[Respiratory muscle weakness after prolonged use of hydrocortisone and pancuronium bromide].

A 30-year-old man was admitted because of status asthmaticus. He required 7 days of artificial ventilation and was treated with hydrocortisone 1.2 g.day-1 and bronchodilaters. Pancuronium bromide 0.08 mg.kg-1.hr-1 was given for 64 hours for the ease of artificial ventilation. On day 3, severely elevated airway pressure resulted in left pneumothorax and isoflurane 1% was given for the following 2 days. Respiratory muscle weakness was evident 24 hours after discontinuation of pancuronium infusion on day 5 while full 4 twitches of TOF on the adductor pollicis muscle were seen at the time. The res

Lung phospholipids during recovery from oxygen toxicity are altered by hydrocortisone.

Corticosteroids affect type II alveolar epithelial cells, increase the lung's content of lipids associated with pulmonary surfactant, and have been used to treat the adult respiratory distress syndrome. We used hyperoxia to produce diffuse alveolar damage in rats and then determined the contents of lung lipids and the incorporation of precursors into these lipids while the rats recovered in air and received either hydrocortisone or saline injections. With saline injections (control) the total lipid content after hyperoxia and 48 hours recovery in air was nearly twice the lipid content of lungs

Retrograde spread of hydrocortisone containing foam given intrarectally in ulcerative colitis.

A method is described of estimating retrograde spread through the colon of a 10% hydrocortisone acetate foam by labelling the foam with technetium-99m sulphur colloid and observing spread after intrarectal administration by serial gamma-camera pictures. The recommended 51 ml dose of foam reached the mid-sigmoid colon in all of the nine patients who had active ulcerative colitis. Furthermore, in seven the foam reached the proximal sigmoid colon. Foam spread less extensively in five patients with quiescent disease than in those with active disease. Increasing the volume of enema to 50 ml did not

Critical illness-related corticosteroid insufficiency after acute brain hemorrhage surgery: a prospective cohort study with a randomized trial of hydrocortisone : HYdrocortisone theraPy in nEurocRitical illness; HYPER study.

BACKGROUND: Critical illness-related corticosteroid insufficiency (CIRCI) is a potentially underrecognized complication in postoperative patients with acute brain hemorrhage. Its incidence, associated risk factors, and the therapeutic role of corticosteroids in this population remain unclear. METHOD: This multicenter study combined a prospective observational cohort with a single-blinded randomized controlled trial. Adult patients aged 18–80 years who underwent neurological surgery for acute brain hemorrhage within 48 h of admission at tertiary care hospitals were eligible for inclusion. Acute

Effect of Vitamin C, Hydrocortisone, and Thiamine vs Hydrocortisone Alone on Time Alive and Free of Vasopressor Support Among Patients With Septic Shock: The VITAMINS Randomized Clinical Trial.

IMPORTANCE: It is unclear whether vitamin C, hydrocortisone, and thiamine are more effective than hydrocortisone alone in expediting resolution of septic shock. OBJECTIVE: To determine whether the combination of vitamin C, hydrocortisone, and thiamine, compared with hydrocortisone alone, improves the duration of time alive and free of vasopressor administration in patients with septic shock. DESIGN, SETTING, AND PARTICIPANTS: Multicenter, open-label, randomized clinical trial conducted in 10 intensive care units in Australia, New Zealand, and Brazil that recruited 216 patients fulfilling the

[Studies on toxicity of hydrocortisone 17-butyrate 21-propionate -1. Acute toxicity of hydrocortisone 17-butyrate 21-propionate and its analogues in mice, rats and dogs (author's transl)].

The acute toxicity of hydrocortisone 17-butyrate 21-propionate (HBP), hydrocortisone 17-butyrate (HB17) and hydrocortisone 21-butyrate (HB21) were investigated by three administration routes (s.c., i. p. and p. o.) in mice, rats and dogs. In the case of HBP, LD50 by oral administration was the highest, and followed by subcutaneous and intraperitoneal administration in mice and rats. And LD50 of HB17 and HB21 were not different from HBP in mice by subcutaneous administration. The depression of spontaneous movement and respiratory rate, ptosis, larcrymation and the collapse were commonly observe

Hydrocortisone administration in preterm infants is not associated with adverse cardiovascular outcomes in childhood.

BACKGROUND: To assess whether administering hydrocortisone in the perinatal period is associated with subsequent adverse cardiovascular outcomes. METHODS: The children/adolescents enrolled in the PREMILOC trial underwent resting blood pressure (BP) measurement, tonometry evaluation (pulse wave velocity (PWV), aortic systolic BP), continuous BP and ECG measurements (supine and standing), and ambulatory BP monitoring. Heart rate variability (HRV) indices, baroreflex sensitivity (BRS), and orthostatic systolic BP (SBP) response were calculated. RESULTS: Fifty-two subjects (median [25th; 75th pe

Hydrocortisone for Secondary Prevention of Post-Traumatic Stress Disorder: A Case Report.

Post-Traumatic Stress Disorder (PTSD) is prevalent among both the civilian and military populace, causes significant burden to the healthcare system, and diminishes the quality of life of those afflicted by it. Secondary pharmacologic prevention strategies with glucocorticoids have been studied in ICU patients and those who have experienced traumatic events with some effect. This case report follows one Active-Duty Marine who was exposed to a traumatic event during an emergency medical technician shift. Following this event, he was diagnosed with Acute Stress Reaction and treated with a 10-day

Low-Dose Hydrocortisone and Cognition in Women With HIV: A Randomized Clinical Trial.

IMPORTANCE: Women with HIV experience high rates of trauma, chronic stress, and depression, which are linked to cognitive impairment. Dysregulation of the hypothalamic-pituitary-adrenal (HPA) axis, glucocorticoid receptor function, inflammation, and HIV persistence are candidate mechanisms, yet no interventions directly target these pathways. OBJECTIVES: To determine whether low-dose hydrocortisone (LDH), a pharmacologic modulator of HPA axis function, improves cognition in women with HIV and to examine neuroendocrine, immune, and HIV-related mechanisms underlying treatment effects. DESIGN,

Evaluation of Hydrocortisone Discontinuation Strategies in Septic Shock: A Retrospective Cohort Study.

IMPORTANCE: While corticosteroid administration in septic shock has been shown to reduce vasopressor requirements and accelerate shock reversal, the optimal discontinuation strategy remains unexplored. OBJECTIVES: The purpose of this study was to assess whether rates of hemodynamic instability differ among patients with septic shock undergoing abrupt hydrocortisone discontinuation compared with gradual tapering. DESIGN, SETTING, AND PARTICIPANTS: A retrospective cohort study conducted in five medical and surgical ICUs at a tertiary care hospital, involving adult patients (≥ 18 yr) with septi

Interaction of hydrocortisone and illness severity on head growth in cohort of ELBW infants.

BACKGROUND: Extremely low birth weight (ELBW) infants comprise a fragile population at risk for neurodevelopmental disabilities (NDD). Systemic steroids were previously associated with NDD, but more recent studies suggest hydrocortisone (HCT) may improve survival without increasing NDD. However, the effects of HCT on head growth adjusted for illness severity during NICU hospitalization are unknown. Thus, we hypothesize that HCT will protect head growth, accounting for illness severity using a modified neonatal Sequential Organ Failure Assessment (M-nSOFA) score. METHODS: We conducted a retros

Successful management of feline pemphigus foliaceus with pentoxifylline and topical hydrocortisone aceponate.

The treatment regimen for feline pemphigus foliaceus (PF), an autoimmune disease caused by auto-antibodies against proteins of the desmosome junction, usually includes high doses of oral or parenteral immunosuppressive drugs, typically glucocorticoids. This case adds to a growing body of evidence that topical hydrocortisone aceponate is effective for the treatment of feline PF, and demonstrates the practical use of a non-invasive diagnostic method for histopathology when owners refuse a biopsy to support a clinical diagnosis of PF. Finally, this case highlights an international trend of owner-

[The controversy and value of the combination of hydrocortisone, ascorbic acid and thiamine in treating sepsis].

Sepsis is a common cause of death in intensive care unit. Some retrospective studies had shown the combination of hydrocortisone, ascorbic acid and thiamine (HAT therapy) could lower mortality rate of patients with sepsis or septic shock. Recently, multiple randomized controlled trials(RCTs) related to the efficacy of combination therapy don't reach a consensus. Though one meta-analysis indicated beneficial effect of HAT regimen on decreasing mortality rate of patients with sepsis or septic shock, improving SOFA score and reducing use of vasopressor. However, controversy still exist in the tar

[Metabolic assessment of hydrocortisone replacement therapy in patients with primary adrenocortical insufficiency].

Primary adrenocortical insufficiency (Addison's disease) requires lifelong steroid substitution. Although the patients are both at risk of under-replacement and excessive glucocorticoid exposure, there is no consensus on monitoring this therapy. The aim of the study was to assess the substitution therapy in Addison's disease in regard to metabolic balance, glycaemic effects and bone mineral density. Seventy two subjects with primary adrenal insufficiency (52 women, 20 men) were evaluated. Mean disease duration was 15.6 years. All patients were supplemented with hydrocortisone (10-60 mg/day), 4

The first case of drug-induced immune hemolytic anemia due to hydrocortisone.

BACKGROUND: Drug-induced immune hemolytic anemia (DIIHA) is a well-known complication of drug treatment. Sensitization can occur, due to interaction of the drug and/or its metabolites with cells of the immune system, after the first drug administration, while the hemolytic crisis generally occurs after repeated administration of a drug. This event occurred in the case described here of acute hemolysis due to the administration of corticosteroids. STUDY DESIGN AND METHODS: To define the etiopathogenesis of the hemolytic crisis, immunohematologic screening and specific tests were performed to i
