[Hepato-cholangiocarcinoma with diffuse metastases and hypercalcemia].

The combined hepatocellular-cholangiocarcinoma (C-HCC) is a very uncommon neoplasm, its diagnosis is difficult and its course is usually very rapid. We report the case of a patient with CHCC type II or transitional tumor, which shows two cellular types in certain zones. Our patient had tumoral hypercalcemia as well, which was very difficult to control. There was a metastatic spread with cholangiolar features almost from the beginning, that made impossible any sort of therapy. It seems that dissemination followed lymphatic pathways.

IgA nephropathy and hypercalcemia in Whipple's disease.

In 2 patients an IgA nephropathy was found 2 and 5 years before gastrointestinal symptoms led to the diagnosis of Whipple's disease. One patient additionally presented with hypercalcemia. Subsequently 1 patient died, whereas treatment with trimethoprim/sulfamethoxazole resulted in an improvement of IgA nephropathy and in a complete recovery from hypercalcemia and all the manifestations of Whipple's disease in the other patient. IgA nephropathy and hypercalcemia may be considered as early manifestations of Whipple's disease.

[Hypercalcemia and neoplasms: recent advances in pathogenesis].

The mechanisms of paraneoplastic hypercalcemic syndromes are heterogeneous. Neoplastic hypercalcemia without bone metastatic disease is caused by parathyroid hormone related protein, whose action is comparable to parathyroid hormone. Growth transforming factors, platelet derived growth factor, tumor necrosis factors and interleukin 1 are also involved in humoral hypercalcemia of malignancy. In addition to these substances, hypercalcemia in bone metastatic disease may be related to PGE. Tumor necrosis factors and interleukin 1 play a major role in multiple myeloma as well as in Adult T cell Leu…

Vitamin-A-induced hypercalcemia: response to corticosteroids.

Hypercalcemia caused by vitamin A toxicity and its prompt response to prednisone is described. Diagnosis was difficult in this 16-year-old patient because she denied recent ingestion of vitamin A and was thought to be bulimic. Intravenous fluids and loop diuretics lowered the serum calcium and ameliorated the symptoms of nausea and vomiting temporarily. During the 10 days of this therapy the serum calcium never fell below 11.5 mg/dl. Oral prednisone was then started and produced a prompt and lasting reduction of the serum calcium to normal. Physicians should be aware of this complication in te…

Experience with hypercalcemia and osteoporosis of neoplastic disease.

Hypercalcemia and hypercalcemic crisis are common and often life threatening complications of neoplastic disease. Even moderately increased mobilization of skeletal calcium can result in significant osteoporosis. This in turn can give rise to pathologic fractures, often followed by pulmonary fat embolism and death. Bone pain is at times the first sign of neoplastic disease and the reason for seeking medical help.

[[Clodronate therapy of hypercalcemia caused by hyperparathyroidism].

The diphosphonates, powerful agents which inhibit osteolysis, have been used parenterally with success to treat malignant hypercalcemia. 4 cases of anatomically confirmed primary hyperparathyroidism (age of patients 58-78 years) were admitted with a serum calcium of over 4 mmol/l in one case, over 3,5 mmol/l in a second case and over 3 mmol/l in the other two. The patients received an infusion of isotonic saline and dichloromethylene-diphosphonate (clodronate) parenterally in a dose of 500 mg a day. Serum calcium began to fall in all four patients 48 hours after the treatment was started. Intr…

Etiology of hypercalcemia in a patient with Addison's disease.

A case is reported of a hypercalcemic patient with primary Addison's disease. A combination of increased calcium input into the extracellular space and reduced calcium removal by the kidney accounted for the hypercalcemia. The mechanisms responsible for the reduction in calcium removal were decreased glomerular filtration and increased tubular calcium reabsorption. Both renal factors were secondary to volume depletion and improved rapidly during rehydration with saline infusion. The enhanced calcium mobilization was probably of skeletal origin. It persisted irrespective of volume status until …

An adenosquamous carcinoma of the male urethra with hypercalcemia.

A case of primary adenosquamous carcinoma of the urethra observed in a 52 year old man is described. The patient had a history of recurrent scrotal abscess for seven and one-half months following trauma to the region. The serum calcium level was found to be elevated when carcinoma was diagnosed. The calcium level returned to normal after resection of the lesion but rose again when local recurrence and metastasis developed. He died three and one-half months after the surgery.

Differential diagnosis of hypercalcemia: a mechanistic approach.

Malignant disease and primary hyperparathyroidism are the most common causes of hypercalcemia, but there are many minor causes. Mechanical or humoral factors, or both, may underlie the increase in bone resorption. Parathyroid hormone (PTH) is a major mediator of bone resorption, but many other humoral agents have the same effect, eg, prostaglandin, osteoclast-activating factor, and thyroid hormone. Serial determination of total calcium concentration is the most important laboratory test in hypercalcemia. Other useful tests include the determination of serum and urinary phosphorus concentration…

Hypercalcemia in children with spinal cord injury.

Eighty-seven children less than 16 years of age with recent spinal cord injury were studied during a 7-year period (1970--1976). Serum calcium concentration was determined at least once in each of 76 of these patients, and in 18 (23.6%) it was above 11 mg%. Fifteen of the 18 patients had quadriplegia and 3 had paraplegia. In 5 patients, the first symptoms of acute hypercalcemia simulated those of an acute abdomen. Two of the 5 patients underwent exploratory laparotomy, with negative findings. The incidence of urinary stones was 55% (10 of the 18), a difference from the control population of on…

Hypercalcemia in patients with known malignent disease.

In 82 patients, a preoperative diagnosis of primary hyperparathyroidism has been established by means of transfemoral neck vein catheterizat … ediastinal search was made. Four normal cervical parathyroid glands were found; three were removed. Hypercalcemia persisted after operation, and the patient died. At postmortem examination, microscopic study revealed that the disease had metastasized to lungs and hilar lymph nodes. There was massive metastasis in the liver; the liver contained a large amount of …

Metastatic carcinoma of penis complicated by hypercalcemia.

A fifty-three-year-old man with epidermoid carcinoma of the penis metastatic to the right inguinal lymph nodes and adjacent areas was found to have persistent hypercalcemia. Associated with this biochemical abnormality was an elevated parathormone activity in the absence of any bony metastases. Other than a transient response to furosemide-inducded diuresis he was refractory to treatment with oral inorganic phosphates and mithramycin. Ablation of the primary tumor did not affect his hypercalcemia. However, when therapy using external irradiation and parenteral bleomycin was directed to the met…

Clinical Insights Into Hypercalcemia of Malignancy in Childhood.

Hypercalcemia of malignancy (HCM) is a rare but life-threatening metabolic emergency in children that occurs in less than 1% of pediatric cancer cases, with a reported incidence ranging from 0.4% to 1.0% across different studies. While it is observed in 10%-20% of adult malignancies, pediatric HCM remains relatively uncommon. Unlike HCM in adults, in which solid tumors are the primary cause, pediatric HCM is most frequently associated with hematological malignancies such as acute lymphoblastic leukemia (ALL). Knowledge regarding the duration of malignancy-related hypercalcemia is obtained by e…

The Calcium Conundrum: Hypercalcemia of Unknown Etiology.

Hypercalcemia is a common clinical finding with a wide variety of clinical presentations and etiologies. The differential is broad and includes hyperparathyroidism, malignancy, medication adverse effects, endocrinopathies, and a variety of granulomatous diseases. We present the case of an 84-year-old woman with severe parathyroid hormone-independent hypercalcemia and elevated 1,25-OH Vitamin D. Non-necrotizing granulomas were identified on bone marrow biopsy. After an exhaustive workup for granulomatous disease, the patient was felt to have sarcoidosis isolated to the bone marrow and started o…
