Mizoribine-induced severe hyperglycemia in a patient with microscopic polyangiitis.

A 77-year-old woman had been receiving prednisolone (PSL) for 5 months as induction therapy for microscopic polyangiitis. Because of repeated elevation of the antineutrophil cytoplasmic autoantibody titer, we added mizoribine (MZR), and, 2 months later, the patient developed severe hyperglycemia with low serum and urinary C-peptide reactivity (CPR). The MZR was, therefore, withdrawn and insulin therapy was started. One month later, the serum and urinary CPR had increased and postprandial hyperglycemia had improved. Previous in vitro studies have shown that MZR can induce hyperglycemia through …

Relationship of hyperglycemia and surgical-site infection in orthopaedic surgery.

BACKGROUND: The impact of perioperative hyperglycemia in orthopaedic surgery is not well defined. We hypothesized that hyperglycemia is an independent risk factor for thirty-day surgical-site infection in orthopaedic trauma patients without a history of diabetes at hospital admission. METHODS: Patients eighteen years of age or older with isolated orthopaedic injuries requiring acute operative intervention were studied. Patients with diabetes, injuries to other body systems, a history of corticosteroid use, or admission to the intensive care unit were excluded. Blood glucose values were obtain…

Worsening of hyperglycemia due to atorvastatin in a renal transplant patient.

New-onset diabetes mellitus post-renal transplantation [post-transplantation diabetes mellitus (PTDM)] and impaired glucose tolerance are among the most serious adverse metabolic disturbances of kidney transplants. We report a renal transplant patient whose mild post-transplant hyperglycaemia considerably worsened upon substituting atorvastatin for pravastatin. The patient was a 58-years-old Caucasian man who underwent living, non-related kidney transplantation. The mean blood sugar level (BSL) following transplantation was 113.8 mg/dl. In an attempt to reduce LDL cholesterol, atorvastatin 40

Infants of diabetic mothers: the effects of hyperglycemia on the fetus and neonate.

News that a woman with diabetes is about to deliver brings up images of a macrosomic infant. This infant may experience birth injuries, asphyxia, respiratory distress, hypoglycemia, hypocalcemia, hyperbilirubinemia, polycvthemia/hyperviscosity syndrome, asymmetric sepral hypertrophy, and other congenital malformations. Uncontrolled diabetes has profound effects on embryogenesis, organogenesis, and fetal and neonatal growth, and evidence increasingly indicates that some of these effects are lifelong and may contribute to adult obesity. Preconception control of diabetes and monitoring throughout

Hyperglycemia and mortality in elderly patients with Staphylococcus aureus bacteremia.

OBJECTIVE: To investigate the association between hyperglycemia and in-hospital mortality in elderly patients with Staphylococcus aureus bacteremia (SAB). METHODS: We reviewed the medical records of 135 elderly patients with SAB admitted to two tertiary medical centers from January 2003 until December 2004. Patients were stratified into two groups: those with a 7-day post-SAB mean blood glucose < 170 mg/dL and those with a 7-day post-SAB mean blood glucose > or = 170 mg/dL. A stepwise logistic regression analysis was performed to determine whether the degree of hyperglycemia was a significant…

Cerebral injury in association with profound iatrogenic hyperglycemia in a neonate.

The adverse effect of hyperglycemia on neurological outcome following cerebral ischemia has been established in both experimental and adult human studies. However, there is a paucity of data to assess this risk in the human infant. This case is the first to describe severe and prolonged iatrogenic hyperglycemia in association with ischemia in an infant who sustained a major cerebral lesion. The topography of the predominant cerebral injury, i.e., bilateral parieto-occipital cortex and subcortical white matter, shown by magnetic resonance imaging was similar to that documented in the hypoglycem…

[Acute repetitive giratory seizures as a manifestation of nonketotic hyperglycemia].

This 71 years old women without any history of epilepsy had diabetes mellitus. She was admitted for repetitive giratory seizures in relation with non-ketotic hyperglycaemia. The EEG showed right centro-parietal paroxysmal slow activity. Symptomatology disappeared within 48 hours after insulin therapy. One month later, she presented with a left hemiplegia in relation with a right sylvian infraction. The role of focal transitory ischaemia in connection with hyperglycaemia is discussed.

The effect of urapidil and ramipril on hyperglycemia in streptozotocin diabetic rats.

Angiotensin-converting enzyme inhibitors and alpha1-adrenoceptor antagonists improve glucose disposal in diabetes mellitus. We compared the effect of the antihypertensive hybrid drug urapidil [alpha1-adrenoceptor antagonist serotonin 1A (5-hydroxytryptamine 1A, 5-HT1A) receptor agonist] on hyperglycemia in streptozotocin diabetic rats with the angiotensin-converting enzyme inhibitor ramipril. 5-HT1A receptor agonists induce hyperglycemia. This could be an important disadvantage during treatment of diabetes mellitus with urapidil. Diabetes was induced by streptozotocin (70 mg/kg i.p.). Treatmen…

Spontaneous hypertriglyceridemic obesity and hyperglycemia in an inbred line of rats.

A genetically mild obesity syndrome of pubertal onset in a highly inbred line of rats differentiated as beta (beta) has been described. It was discovered in both sexes fed a stock diet for rodents. Hyperphagia was not noticeable. Total fat content reached 31 percent of total body weight in mature males. Obesity was associated with normal plasma cholesterol values and hypertriglyceridemia. Fasting blood sugar levels at maturity were within the normal range for rats, but significantly higher than in lean alpha (alpha) controls. This syndrome developed into a mild glucose intolerance and glucosur

Role of H2-receptors in gastrogenic hyperglycemia and hyperinsulinemia in dogs.

Recent studies have demonstrated that an acidified liver meal stimulates insulin release and raises plasma glucose levels. To examine the mechanism of these changes, a liver extract test meal at either pH 7 or at pH 2 was instilled into the stomach of dogs with a bisected pylorus and a gastric fistula during the infusion of either cimetidine, a specific H2-receptor antagonist, or a saline control. In response to the meal at pH 2 insulin, glucagon and glucose levels rose significantly and were not significantly changed by the infusion of cimetidine. In response to the liver meal at pH 7 a late

A liver phosphatase reprograms gut stem cells to drive hyperglycemia.

Why is fatty liver disease associated with hyperglycemia? In this issue, Ye, Wan, Liu, Deng, Zhang et al.1 propose an unexpected mechanism: hepatic alkaline phosphatase released from diseased liver suppresses intestinal stem cell differentiation into GLP-1-secreting L-cells. This study reveals a new pathogenic route of liver-intestine communication.

Diabetic Hyperglycemia Induces Region-Specific Transcriptomic Remodeling in the Cochlea.

BACKGROUND/OBJECTIVES: Diabetes mellitus is associated with sensorineural hearing loss, but the cochlear molecular alterations related to this complication remain unclear. This pilot study aimed to characterize auditory function and region-specific transcriptomic changes in the diabetic cochlea. METHODS: A streptozotocin-induced type 1 diabetic mouse model was established. Auditory function was assessed by auditory brainstem response (ABR) testing. Next-generation RNA sequencing was performed on micro-dissected cochlear regions, including the modiolus, lateral wall, and organ of Corti, follow

How hyperglycemia promotes ocular diseases: An overview of molecular mechanisms.

Ocular diseases result from a complex interaction of environmental and metabolic factors. The increasingly prevalent sedentary lifestyle and dietary patterns characterized by high sugar and ultra-processed food intake substantially elevate the risk of hyperglycemia. Hyperglycemia induces dysregulation of core metabolic pathways, such as overactivation of the polyol and hexosamine pathways, aberrant activation of protein kinase C, and accumulation of advanced glycation end products. Hyperglycemia further triggers tissue-specific molecular and cellular events in various ocular structures. These …

Acetate Administration Ameliorates Streptozotocin-Induced Hyperglycemia and Adipose Tissue Loss.

Short-chain fatty acids (SCFAs) are products of gut microbiota through fermentation of soluble fibers. Recent studies have highlighted the beneficial roles of SCFAs in various physiological and pathological conditions, including diabetes. In this study, we applied streptozotocin (STZ)-induced type 1 diabetes mellitus (T1DM) to investigate the potential role of SCFAs in the pathogenesis of T1DM. We found a significant increase in the abundance of SCFA metabolism-related bacteria in mice surviving after 5 month post-STZ injection (mpi), coupled with improved hyperglycemia. While the expression l…

EGLN1 inhibition reverses angiogenesis impairment in hyperglycemia by activating autophagy.

Cardiovascular complications have emerged as the predominant cause of mortality in diabetes mellitus (DM), with diabetic cardiomyopathy (DCM) representing a critical clinical challenge due to its distinct pathogenesis independent of coronary artery disease or hypertension. DCM is characterized by left ventricular diastolic dysfunction, hypertrophy, and fibrosis, with impaired myocardial angiogenesis contributing to adverse cardiovascular outcomes. Despite this, targeted therapies for angiogenesis restoration in DCM remain elusive. Through bioinformatic analysis of public databases, we identifi

Sex-Specific Alterations in Placental Proteomics Induced by Intrauterine Hyperglycemia.

Gestational diabetes mellitus (GDM) with intrauterine hyperglycemia induces a series of changes in the placenta, which have adverse effects on both the mother and the fetus. The aim of this study was to investigate the changes in the placenta in GDM and its gender differences. In this study, we established an intrauterine hyperglycemia model using ICR mice. We collected placental specimens from mice before birth for histological observation, along with tandem mass tag (TMT)-labeled proteomic analysis, which was stratified by sex. When the analysis was not segregated by sex, the GDM group showe…
