BACKGROUND: Hyperparathyroidism-jaw tumor (HPT-JT) syndrome is a rare autosomal dominant multiple tumor syndrome characterized by hyperparathyroidism due to single or multiple-gland parathyroid tumor(s). Since it was first described in 1990, the genetics underlying the syndrome have been elucidated and typical clinical presentations are becoming clarified as literature describing this rare entity amasses.
METHODS AND RESULTS: A 22-year-old man presented with a 2-year history of fatigue, weight loss, nausea, and vomiting. Anemia workup indicated severe hypercalcemia. Investigations were consis…

Primary hyperparathyroidism is the third most common endocrine disorder after diabetes and thyroid disease, and women are affected twice as often as men. Hyperparathyroidism in pregnancy was first reported in 1931. Maternal complications in patients with hyperparathyroidism can be as high as 67%. We present a case of a pregnant patient with chronic hypertension that was exacerbated throughout the course of her pregnancy with a concomitant diagnosis of primary hyperparathyroidism and its sequelae for both the mother and fetus.

PURPOSE OF REVIEW: Primary hyperparathyroidism (PHPT) is a constantly evolving disease. As more evidence on the cardiovascular and neuropsychiatric manifestations of apparently asymptomatic disease accumulates, and since medical therapeutic options and surgical indications are being further investigated, it is important to review this disease entity to gain a fresh, updated perspective on the current disease phenotype and treatment approach.
RECENT FINDINGS: The pathogenesis of PHPT has not been completely elucidated yet, but some potential culprits, such as gene mutations, growth factors, ca…

BACKGROUND: Hyperparathyroidism (HPT), the result of excessive secretion of the parathormone, is one of the most common endocrine disorders. In most forms of HPT, surgical parathyroidectomy is the best choice.
AIM: This paper aims to examine patients with hyperparathyroidism operated in First Surgical Unit Iaşi, in terms of indications, surgical technique and postoperative results.
MATERIAL AND METHOD: We performed a retrospective study, in First Surgical Unit Iaşi, during 2000-2008. Clinical, laboratory, intraoperative and histopathological data were included in a MS Access Office XP databa…

DESIGN: Case report.
SETTING: University hospital of Udine, Italy.
PATIENT(S): A 32-year-old black pregnant woman with primary hyperparathyroidism.
INTERVENTION(S): Hospitalization with observation, nuclear magnetic resonance of the neck, and right parathyroidectomy of the patient in the 15th week of gestation (WG). Monitoring during pregnancy until the delivery.

OBJECTIVE: Sialolithiasis is a frequent condition seen and treated by otolaryngologists. Salivary stones typically affect the submandibular and parotid glands and can arise from electrolyte disturbances, stasis, or a chronic infectious origin. Primary hyperparathyroidism (pHPT) is associated with 'stone disease', but this has been universally considered to refer to kidney stones. Few case reports exist reporting the relationship between pHPT and sialolithiasis.
DESIGN: A retrospective patient series from a single surgical practice.
SETTING: A single-specialty referral clinic with a practice …

Surgery of primary hyperparathyroidism. Primary hyperparathyroidism is usually caused by a single parathyroid adenoma, rarely by multiple adenomas or hyperplasia and in 1-2% of cases by carcinoma. The definitive cure of the disease can be achieved only by surgical means. Unfortunately, only 10% of expected cases based on the number of population are diagnosed in Hungary. The main reason is that the disease has no specific symptoms and it causes only a few cases present with clinical entities such as nephrolithiasis, osteoporosis-osteopenia, pancreatitis, hypertension, peptic ulcer disease, dep…

Impetigo herpetiformis, first described by Hebra, is a rare pustular disorder that primarily affects pregnant women and it is often complicated by an increased risk of spontaneous abortion. A commonly associated hypocalcemia often appears with hypoparathyroidism. Here we report a case of complicated impetigo herpetiformis without hypocalcemia paired with a compensatory hyperparathyroidism.

We present three cases of primary hyperparathyroidism (PHPT) in pregnancy. The clinical presentation of PHPT is not altered by pregnancy; however, the disease constitutes a serious risk for the foetus and the newborn. Although rare, hypercalcaemia should be suspected in pregnant women presenting with polydipsia, polyuria and fatigue as well as hypertension or preterm labour.

More than 95% of patients with primary hyperparathyroidism (HPT) will be cured at initial operation by an experienced surgeon. Despite this success rate, persistent and recurrent HPT remain challenging clinical entities. The most cost effective and safest treatment for persistent and recurrent HPT is avoidance by successful first operation. The contributors to treatment failure can be categorized into factors related to the initial surgical procedure, anatomic variability, and the biology of disease. An understanding of the factors that commonly contribute to treatment failure can help prevent…

OBJECTIVE: Evaluation of the influence of hypercalcemia and primary hyperparathyroidism on the course of pregnancy and recommendations for the best management of this condition.
DESIGN: Case report.
SETTING: Department of Obstetrics and Gynecology, Regional Hospital, Pardubice.
SUBJECTS AND METHODS: The author analyses her experience with the course of pregnancy in a patient with primary hyperparathyroidism diagnosed at the beginning of the third trimester. The patient was successfully operated on, the solitary adenoma of parathyroid gland was removed and she delivered vaginally a healthy f…

Primary hyperparathyroidism (PHPT), in addition to cancer, represents an important cause of hypercalcaemia in the general population. Furthermore, hypercalcaemia, in the course of uraemic HPT, represents the late stage of chronic renal failure refractory to therapy. Neck surgery is still the only curative approach for these forms of HPT and medical treatment rarely exhibits an effective control on HPT and HPT-dependent hypercalcaemia. Moreover, some HPT patients may not undergo neck surgery due to the presence of other concomitant disorders. Therefore, more effective therapeutic approaches are…

There has been a shift in the referral patterns in recent years that has resulted in increasing numbers of patients being referred to surgeons with a diagnosis of hypercalcemia rather than primary hyperparathyroidism. The surgeon must perform a thorough history, including medications, and laboratory assessment, including serum calcium and parathyroid hormone measurements. A 24-hour urinary calcium excretion should be routinely ordered to exclude FHH. After the diagnosis of primary hyperparathyroidism is made, preoperative localization studies will benefit 78% to 90% of patients, with sestamibi…

The cause and effect relationship between acute pancreatitis and primary hyperparathyroidism (pHPT) still evokes controversy. Our paper reviews the debate in the medical literature. In this controversy we add a case of a 49-year old non-alcoholic man presenting with recurrent attacks of acute pancreatitis. His raised serum calcium was realized rather late. Eventually, high intact parathyroid hormone levels led to open neck exploration and finding of a solitary parathyroid adenoma. Post-surgery, serum calcium returned to normal and abdominal symptoms disappeared. The case report and the accompa…
