[Surgical treatment of primary hyperparathyroidism].

OBJECTIVE: To investigate the causes of misdiagnosis and the surgical treatment of primary hyperparathyroidism (PHPT). METHODS: The clinical data of 26 patients with PHPT from July 2008 to January 2013 in The Affiliated Hospital of Chengde Medical College and The Fourth Hospital of Hebei Medical University were retrospectively analyzed, including preoperative diagnosis and operative method. The level of serum calciumion and serum parathyroid hormone (PTH), Ultrasonography, CT, (99)mTc-methoxy isobutylis onitrile ((99)mTc-MIBI) were used in the diagnosis before operation. All patients accepted

Nontraditional manifestations of primary hyperparathyroidism.

Classical primary hyperparathyroidism (PHPT) was previously a multisystemic symptomatic disorder not only with overt skeletal and renal complications but also with neuropsychological, cardiovascular, gastrointestinal, and rheumatic effects. The presentation of PHPT has evolved, and today most patients are asymptomatic. Osteitis fibrosa cystica is rarely seen today, and nephrolithiasis is less common. Gastrointestinal and rheumatic symptoms are not part of the clinical spectrum of modern PHPT. It remains unclear whether neuropsychological symptoms and cardiovascular disease, neither of which ar

Turner syndrome with primary hyperparathyroidism.

Turner syndrome has multiple comorbidities such as osteoporosis, obesity, diabetes, hypothyroidism, and hypertension. As they are treatable conditions in Turner syndrome, early recognition and proper treatment should be needed. We report on a 23-year-old woman with Turner syndrome who presented with severe osteoporosis and hypercalcemia. Laboratory tests showed elevated levels of serum calcium and parathyroid hormone. Dual-energy X-ray absorptiometry showed severe osteopo-rosis (z score, -3.5). Ultrasound and (99m)Tc scintigraphy of parathyroid glands showed an adenoma in the right inferior gl

Primary hyperparathyroidism: imaging to pathology.

The aim of this review is to describe the multimodal imaging (ultrasound, magnetic resonance, computed tomography, and nuclear medicine) of primary hyperparathyroidism and its correlation to the pathological findings. In the last decades, imaging science has progressed a great deal. Accurate preoperative localization of the involved glands is essential for surgical success.

[Primary hyperparathyroidism: diagnosis and management].

The prevalence of primary hyperparathyroidism (HPT) varies between 1 and 4/1.000 in the general population. HPT is nowadays most often asymptomatic. The classical bone disease has been replaced by osteopenia or osteoporosis with a preferential bone loss in cortical sites. The incidence of nephrolithiasis has been considerably lowered, but renal lithiasis is still the most frequent complication of HPT. The diagnosis is most often made by chance or during the workup of an abnormal bone mass. Hypercalcemia and an elevated PTH concentration, or at least a PTH level in the upper part of the normal

Lithium use and primary hyperparathyroidism.

OBJECTIVE: To review suspected causes of lithium-induced hyperparathyroidism, disease presentation, underlying pathology, and current recommendations and trends in medical and surgical treatment. METHODS: Relevant literature was reviewed. RESULTS: Lithium carbonate therapy has continued to be a mainstay of treatment for bipolar disease and schizoaffective disorder since its introduction into clinical use. Several metabolic consequences are associated with its long-term use, including hypercalcemia and hyperparathyroidism. CONCLUSIONS: Until further data become available, the surgeon should

Surgical treatment of primary hyperparathyroidism.

OBJECTIVE: To review the surgical treatment options for primary hyperparathyroidism with a focus on recent refinements in minimally invasive techniques and endoscopic and video- or robot-assisted parathyroidectomy. METHODS: We review the relevant surgical treatment options for primary hyperparathyroidism. RESULTS: Parathyroidectomy is the standard therapy for patients with primary hyperparathyroidism. Advancements in imaging, including technetium Tc 99m-sestamibi single-photon emission computed tomography and ultrasonography, have improved preoperative localization, while intraoperative para

Subtotal parathyroidectomy for primary hyperparathyroidism.

OBJECTIVE: To discuss the etiology of multiple gland disease in the context of primary hyperparathyroidism, as well as indications for surgery, operative management and technical considerations of subtotal parathyroidectomy, and postoperative/long-term management. METHODS: We conducted a systematic review of the literature using evidence-based criteria. RESULTS: Approximately 15% of patients with primary hyperparathyroidism have multiple gland disease, and a small subset of these cases is due to a familial syndrome. Subtotal parathyroidectomy is one operative approach to the management of mu

Outpatient management of primary hyperparathyroidism.

Primary hyperparathyroidism is the most common cause of hypercalcemia in an outpatient setting. Most patients are asymptomatic with mild hypercalcemia and only require periodic follow-up. Symptomatic patients and those with end-organ damage should undergo surgery according to the National Institutes of Health guidelines. No medical therapy has been approved for treatment of this disorder. Treatment with bisphosphonate, calcimimetic drugs, or alcohol ablation techniques has been used in select patients.

Sporadic and hereditary primary hyperparathyroidism.

OBJECTIVE: To evaluate the influence of reproductive health training given in Turkish Armed Forces on recruits' knowledge. METHODS: This interventional study was conducted at the Basic Military Training Centre, Kutahya, Turkey, from September to October 2009, and comprised recruits of armed forces. Data was collected and assessed before and after the training sessions. The intervention programme consisted of education focusing on anatomy of the reproductive system, contraceptive methods, sexually transmitted infections, responsible parenthood, gender and violence against women. [Renal function in primary hyperparathyroidism].

The kidney is one of the classical target organs of PTH action. Symptomatic primary hyperparathyroidism (PHPT) is nowadays less frequent but mostly occurs with renal symptoms, in particular kidney stones. Nephrocalcinosis and polyuria, the latter closely related to the severity of hypercalcemia, are uncommon. Parathyroidectomy leads to a marked and long-lasting improvement of nephrolithiasis, whereas it has a limited effect on nephrocalcinosis. Kidney failure is one of the long-term complications of PHPT and is associated with a more severe clinical expression of the disease and a higher risk

[Secondary osteoporosis UPDATE. Primary hyperparathyroidism].

It has been clarified that even mild primary hyperparathyroidism without typical symptoms ie, nephrolithiasis, bone disease, influences bone status and reduced bone density and increased fracture risk can be improved with parathyroidectomy. Therefore, parathyroidectomy should be recommended for patients with asymptomatic primary hyperparathyroidism. Guideline and recommendation by an international work shop on primary hyperparathyroidism has presented for the surgical indications. Non invasive localization studies including ultrasound and sestamibi scan, and intraoperative parathyroid hormone

[Primary hyperparathyroidism: ultrasonography and scintigraphy].

Primary hyperparathyroidism is nowadays frequently diagnosed from screening biological work-up: it is due to a single parathyroid adenoma in nearly 90% of cases. There is increasing interest in the minimally invasive surgical approach with unilateral neck exploration. For such approaches preoperative localizing studies are mandatory. Ultrasound and radionuclide imaging are the most widely used procedures for this purpose. Ultrasound offers detailed anatomic imaging of the adenoma but may not be not able to detect some ectopic glands. Sonographic imaging features, as well as pitfalls and diagno
