Severe hypophosphatemia: a rare cause of intravascular hemolysis.

A 3-year-old child presented with severe hyperphosphatemia (phosphate 45 mg/dL) secondary to chronic enema use. Following aggressive correction of the hyperphosphatemia, hypophosphatemia ensued (phosphate 1.7 mg/dL). Concurrently, the patient developed severe intravascular hemolysis and RBC morphologic defects. The hemolysis and morphologic defects corrected with return to normal serum phosphate levels. Severe hypophosphatemia is a rare cause of intravascular hemolysis.

Causal assessment of CKD-MBD biomarker alterations on CKD progression through a g-formula analysis in the EQUAL study.

INTRODUCTION: Chronic Kidney Disease-Mineral and Bone Disorder (CKD-MBD) may be both a cause and a consequence of CKD progression. Here, we examine the individual and joint effect of longitudinal circulating phosphate, parathyroid hormone, and calcium levels on estimated glomerular filtration rate (eGFR) decline, the risk of starting dialysis and the composite risk of death or dialysis, in a cohort of elderly patients with advanced CKD not on dialysis. Secondly, we explore whether these effects differ between men and women. METHODS: We used data from 1709 participants in the European Quality

Optimal use of phosphate binders in chronic kidney disease.

INTRODUCTION: Hyperphosphatemia is one of the major factors associated with the development of vascular calcification in patients with chronic kidney disease (CKD). Since phosphate is retained in such patients, pharmacological treatment and other measures are necessary to control hyperphosphatemia. Several phosphate binders (calcium salts, magnesium salts, non-calcium-based binders and aluminium) are available for the treatment of hyperphosphatemia. Nevertheless, none of the above mentioned agents has shown an overall superiority over others, while potency and side effects are quite variable a

Connections between vascular calcification and progression of chronic kidney disease: therapeutic alternatives.

We have shown that renal injury and chronic kidney disease (CKD) directly inhibit skeletal anabolism, and that stimulation of bone formation decreases the serum phosphate. Most recently, these observations were rediscovered in low-density lipoprotein receptor null mice fed high-fat/cholesterol diets, a model of the metabolic syndrome (hypertension, obesity, dyslipidemia, and insulin resistance). We had demonstrated that these mice have vascular calcification (VC) of both the intimal atherosclerotic type and medial type. We have shown that VC is worsened by CKD and ameliorated by bone morphogen

Mineral and bone disorder and management in the China Dialysis Outcomes and Practice Patterns Study.

BACKGROUND: Despite a growing population of patients starting hemodialysis in China, little is known about markers of mineral bone disease (MBD) and their management. We present data on prevalence and correlates of hypocalcemia, hyperphosphatemia, and secondary hyperparathyroidism from the China Dialysis Outcomes and Practice Patterns Study (DOPPS), with evaluation of whether these laboratory markers triggered changes in management. METHODS: We compared the frequency of measurement and prevalence of poor control of MBD markers in China DOPPS with other DOPPS regions. We also used generalized

[Clinical aspect of recent progress in phosphate metabolism. Management of hyperphosphagtemia].

In spite of various cause of hyperphosphatemia, there is a limited number of modality which include restriction of phosphate intake, phosphate binder, extra cellular fluid expansion and blood purification. Each therapy has certain deficit but combination of those may get preferable consequence. Now various new phosphate binders and the inhibitors of sodium-dependent phosphate transporters are being developed. Those agents might promise a better outcome of the treatment of hyperphosphatemia.

Symptomatic hypocalcemia after sodium phosphate preparation in an adult with asymptomatic hypoparathyroidism.

Sodium phosphate is widely used as a bowel cleansing preparation. Its use is, however, not without risk. It can induce serious adverse effects like hypocalcemia, hyperphosphatemia and renal failure. In this case, a 75-year-old woman without known contraindications developed hypocalcemic tetany, hyperphosphatemia and renal failure after oral sodium phosphate. Asymptomatic hypoparathyroidism owing to previous thyroid surgery was identified as a new contributing risk factor for this complication.

[Pseudohypoparathyroidism].

A 5 year old boy with normal phenotype and normal renal function presented tetany. Hypocalcemia and hyperphosphatemia, with increased serum levels of parathyroid hormone (PTH) were detected in serial measurements. Pseudohypoparathyroidism was diagnosed. This disease presents the biochemical abnormalities of hypoparathyroidism (hypocalcemia, hyperphosphatemia) with peripheral resistance to PTH activity. The patient was treated effectively with calcium and vitamin D supplements. The causes of hypocalcemia related to parathyroid gland activity are reviewed and the physiopathology of pseudohypopar

[Mechanism of intracerebral calcification in hypoparathyroidism].

The mechanism of intracranial calcification in hypoparathyroidism, more frequently seen in pseudo--than idiopathic hypoparathyroidism, has not been completely elucidated, but may be related more to the duration of hypocalcaemia and hyperphosphatemia than parathyroid hormone itself. Hyperphosphatemia promotes ectopic calcification, especially in blood vessel and periarticular tissue in renal failure, but in brain tissue in hypoparathyroidism. Participation of PTH receptor2 in the brain and superoxide production by mitochondria in hypoparathyroidism should be explored with reference to intracere

Hypocalcemic coma following two pediatric phosphate enemas.

Coma, tetany, dehydration, hypotension, tachycardia, and hyperpyrexia developed in a 2 1/2-year-old girl following two hypertonic phosphate pediatric enemas. She had marked hyperphosphatemia, hypocalcemia, hypernatremia, and acidosis. Hypocalcemia due to hyperphosphatemia can explain all of these findings. Calculations indicate that about one third of the phosphorus and sodium contents of the enema were absorbed. Physicians should be aware of the potentially lethal complication of this treatment, which is part of everyday practice.

Urinary Fractional Excretion of Phosphorus in Dogs with Spontaneous Chronic Kidney Disease.

The increase of urinary fractional excretion of phosphorus (uFEP) may indicate phosphorus retention before the onset of hyperphosphatemia in the early stages of chronic kidney disease (CKD). The hypothesis of this study is whether uFEP may increase during the early stage of CKD as a compensatory mechanism to prevent hyperphosphatemia as well as whether hyperphosphatemia in the late stages is associated with increase or decrease in uFEP in dogs with naturally occurring CKD; therefore, the aim of this study was to determine the uFEP in CKD dogs with different stages. Forty-nine CKD dogs were inc

CKD-Mineral Bone Disorder in Stage 4 and 5 CKD: What We Know Today?

Patients with CKD stages 4 and 5 experience biochemical derangements associated with CKD-mineral bone disorder. Some of the key abnormalities are hyperparathyroidism, hyperphosphatemia, hypocalcemia, and metabolic acidosis. We review the available treatments for these conditions and the evidence behind the treatments. We conclude that there is greater evidence for treating hyperphosphatemia than hyperparathyroidism. Treatment of metabolic acidosis in small clinical trials appears to be safe. We caution the reader about side effects associated with some of these treatments that differ in patien

[Information about phosphorus additives and nutritional counseling].

Hyperphosphatemia is a common disorder in patients with chronic kidney disease (CKD) , and may result in hyperparathyroidism and renal osteodystrophy. Hyperphosphatemia also may contribute to deterioration vascular calcification and increase mortality. Hence, correction and prevention of hyperphosphatemia is a main component of the management of CKD. This goal is usually approached both by administering phosphorus binders and by restricting dietary phosphorus (P) intake. Dietary intake of phosphorus (P) is derived largely from foods with high protein content or food additives and is an importa

Emerging role for the vitamin D receptor activator (VDRA), paricalcitol, in the treatment of secondary hyperparathyroidism.

BACKGROUND: Secondary hyperparathyroidism (SHPT) is common in chronic kidney disease (CKD) patients. Classically, SHPT is induced by hypocalcemia, hyperphosphatemia, and calcitriol deficiency, that cause not only renal osteodystrophy but also systemic toxicity, particularly cardiovascular disease. OBJECTIVE: Treatment with calcitriol, the active form of vitamin D, reduces serum parathyroid hormone (PTH) levels but may result in both hypercalcemia and hyperphosphatemia, increasing the risk of vascular calcification in CKD. Are the new vitamin D receptor activators (VDRAs) more useful in the tr

Serum phosphate abnormalities in the emergency department.

Abnormalities in serum phosphate levels are more prevalent in certain subsets of Emergency Department patients than in the general population. Patients with diabetic ketoacidosis, chronic obstructive pulmonary disease, alcoholism, malignancy, and renal failure are at increased risk. Multiple factors, including nutritional intake, medications, renal or intestinal excretion, and cellular redistribution, are potential etiologies. The clinical manifestations of mild hypophosphatemia or hyperphosphatemia are typically minor and nonspecific (myalgias, weakness, anorexia). When the imbalance is sever

Iron Sucrose: A Double-Edged Sword in High Phosphate Media-Induced Vascular Calcification.

The high incidence of vascular calcification (VC) in patients with chronic kidney disease (CKD) has become an important clinical subject. Hyperphosphatemia is a primary cause of CKD-related VC. Intravenous iron sucrose (IS) is commonly used to treat anemia in CKD patients, and is effective and well tolerated worldwide. However, the interaction between iron and VC remains controversial, and the underlying mechanisms are yet to be clarified. In the present study, ex vivo normal rat aortic rings were cultured with various concentrations of phosphate and IS, and the levels of calcium and iron depo

The Impact of Tenapanor on Serum Phosphate in Adult Dialysis Patients: A Narrative Review.

Managing hyperphosphatemia remains a challenging feat in the dialysis population. In 2023, tenapanor, a sodium hydrogen exchanger inhibitor, was approved in the United States for treating hyperphosphatemia in adults with chronic kidney disease receiving dialysis. This narrative review evaluated the recent literature on tenapanor, its effects on serum phosphate, and the achievement of target serum phosphate reference ranges in individuals undergoing peritoneal and hemodialysis. In addition to efficacy, this review examined the prevalence of drug-induced diarrhea. Twelve articles met the inclusi

[Kidney and bone update : the 5-year history and future of CKD-MBD. CKD-MBD and phosphotropic hormones].

During the progression of CKD, secondary hyperparathyroidism develops. It has been proposed that low 1,25-dihydroxyvitamin D, hyperphosphatemia and hypocalcemia contribute to the development of secondary hyperparathyroidism. Recent findings indicate that FGF23 begins to increase before the increment of parathyroid hormone. In addition, FGF23 seems to work to decrease 1,25-dihydroxyvitamin D while it also works to prevent the development of hyperphosphatemia by increasing urinary phosphate excretion together with PTH. However, it is not entirely clear what triggers the increase of FGF23 in pati
