Experimental models of hypersensitivity pneumonitis are important tools for the study of the pathogenesis of this disease. In this paper we review the characteristics of the main animal models developed until now. A 59-year-old man with Duhring's disease (herpetiform dermatitis) developed undulating fever and pulmonary infiltrates soon after sulfapyridine treatment was started. When the drug was discontinued, a positive provocation test unequivocally established the diagnosis of hypersensitivity pneumonia. There was also a cross-reaction with sulfonyldianiline (dapsone). This case illustrates the need of considering a hypersensitivity reaction as the cause of fever and (or) pulmonary infiltration during sulphonamide administration.

Seven patients with type I hypersensitivity reactions to rubber are reported. Radioallergosorbent testing with crude latex antigen was positive in 6/7 patients. Five patients had coexisting spina bifida, and all had undergone surgery before the development of rubber allergy. Rubber allergy is a new phenomenon that should be in the differential diagnosis of idiopathic anaphylaxis and urticaria.

The mercurial compounds are known to be a common cause of allergic contact dermatitis. Immediate hypersensitivity is rarely induced by mercurial organic derivatives. A 32 year old female developed a severe hepatitis (serum bilirubin 544 mumol/l) one week after a halothane anaesthetic. Six months later a general anaesthetic was administered via a halothane free circuit without incident. A year later a further non halothane anaesthetic was administered this time utilising the routine circuit after briefly flushing the rubber tubing with oxygen. That evening she became febrile and the following day abnormal liver function tests were documented. She remained asymptomatic.

Patients referred from the Houston, Texas, metropolitan area were evaluated for allergic reactions to insect stings. Forty-eight persons reported at least one systemic reaction caused by a Polistes paper-nest wasp sting. Honey bees, imported fire ants, and other types of Hymenoptera were identified in that order by 19 other subjects with systemic allergic reactions. The terms milk allergy, milk sensitivity, and milk intolerance are used in the medical literature with different meanings. Fungal extracts that precipitate with human C-reactive protein caused immediate erythema on subdermal injection into marine flatfish. Only species with calcium-dependent serum precipitins to these fungi showed skin reactions. Immediate hypersensitivity in a nonreactive species could be induced after injection with serum from reactive species. The transferable serum factor (or factors) was heat sensitive.
