Effect of ethnicity on mortality of very low birthweight infants in Israel.

BACKGROUND: Multiple demographic, genetic, and environmental factors differ between Muslim and Jewish infants in Israel. OBJECTIVE: To evaluate whether, after adjustment for perinatal factors associated with mortality, excess mortality occurs in very low birthweight (VLBW) Muslim compared with Jewish infants. DESIGN: The Israel National VLBW infant database includes data on 99% of all VLBW births in Israel. The study population comprised 5015 Jewish and 1549 Muslim VLBW infants of more than 23 weeks gestation, born between 1995 and 1999. The Mantel-Haenszel test was used for stratified analy

Prophylactic synthetic surfactant for preventing morbidity and mortality in preterm infants.

BACKGROUND: This section is under preparation and will be included in the next issue. OBJECTIVES: To assess the effect of prophylactic administration of synthetic surfactant in preterm newborns at risk for developing respiratory distress syndrome (RDS). SEARCH STRATEGY: Searches were made of the Oxford Database of Perinatal Trials, Medline (MeSH terms: pulmonary surfactant; limits: age groups, newborn infants; publication type, clinical trial), previous reviews including cross-references, abstracts, conference and symposia proceedings, expert informants, and journal hand searching in the Eng

Nursery neurobiologic risk score as a predictor of mortality in premature infants.

Severity-of-illness scales have been proven to be valuable in assessing clinical outcomes and several risk scores have been used to evaluate neonatal illness severity. At present, we use nursery Neurobiologic Risk Score (NBRS) to evaluate the short term outcome of preterm newborns and their correlation to length of stay and morbidity. The NBRS was determined at the neonatal intensive care unit (NICU) of Changhua Christian Hospital in 183 preterm infants with birth weight < 2500 gm, gestational age < 37 weeks and age less than 7 days between July 1991 and June 1992. The NBRS correlates signific

[Mortality and morbidity among infants with extremely low birth weight].

We reviewed outcomes in 65 infants with extremely low birth weight who were treated at our perinatal center from January 1981 through December 1989. Included in the study were infants whose gestational age was less than or equal to 26 weeks and whose birth weight was less than or equal to 750 grams. The outcomes were compared for three periods, 1981-83 (n = 16), 1984-86 (n = 20) and 1987-89 (n = 29). There was no significant difference in survival rate during the study period (19 vs 25 vs 31%). However, significantly more children now survive the first 24 hours of life.

Surgical mortality and morbidity in extremely low-birth-weight infants.

Twenty extremely premature infants (birth weight, less than 1,100 g) underwent 49 surgical procedures for acquired conditions, including necrotizing enterocolitis, other forms of gastrointestinal perforation, gastroesophageal reflux, patent ductus arteriosus, and hydrocephalus. Few congenital anomalies requiring operation were encountered in this group. The overall survival rate was 58%. Using a computerized data base to compare surgical patients with a weight-matched group of nonsurgical patients, it was shown that survival was similar and that adverse prenatal and perinatal influences were e

Differences in Mortality Among Infants With Neonatal Opioid Withdrawal Syndrome.

INTRODUCTION: Prior studies are mixed regarding whether infants diagnosed with neonatal opioid withdrawal syndrome have a higher risk of mortality than other infants. However, these studies have not accounted for whether mothers of infants with neonatal opioid withdrawal syndrome received medication for opioid use disorder in pregnancy. METHODS: Linked data from 2016‒2018 North Carolina birth certificates, maternal and infant Medicaid claims, and infant death certificates were analyzed in summer 2021 to compare mortality and causes of mortality before age 1 year among infants diagnosed with n

Relationship between the mode of delivery, morbidity and mortality in preterm infants.

BACKGROUND: The aim of the study, to evaluate the relationship between mode of delivery and preterm morbidities and mortality, who born ≤34 weeks of gestation within 1 year. MATERIALS AND METHODS: Babies were divided into two groups as who were born by cesarean section (CS) and vaginal delivery (VD) between March 2019 and March 2020. Infants born at ≤28 weeks were also analyzed. RESULTS: The rate of CS delivery was 76% (378) in the whole group and 73% (115) in the babies of ≤28 gestational weeks. The most common maternal factor causing CS was preeclampsia (25%). The antenatal corticosteroid

Mortality by mode of delivery among infants with spina bifida in Texas.

BACKGROUND: It is hypothesized that cesarean delivery may reduce mortality among infants with spina bifida (e.g., by reducing trauma to the open lesion); however, few studies have assessed this relationship. METHODS: We used the Texas Birth Defects Registry to identify neonates with spina bifida born between 1999 and 2014. The mode of delivery (main exposure) was abstracted from each subject's birth certificate. The vital status (main outcome) was determined based on the presence or absence of a death certificate. When a death certificate was present, survival time was calculated by subtracti

Mortality factors in infants with congenital diaphragmatic hernia: A systematic review.

BACKGROUND: Congenital diaphragmatic hernia (CDH) is a malformation of the diaphragm accounting for 8% of all major congenital anomalies. Although many clinical factors of survival in children with CDH have been established, limited research exists on the role of sociodemographic and other factors. We aimed to systematically identify and summarize all available international literature, published from January 2000 to July 2017, evaluating specific mortality factors for children with prenatally diagnosed, isolated, left-sided CDH. METHODS: MEDLINE, PROSPERO, EMBASE, Scopus, The Cochrane Librar

Cause-specific mortality of very preterm infants and antenatal events.

OBJECTIVE: To assess the relationship between antenatal factors and cause-specific risk of death in a large area-based cohort of very preterm infants. STUDY DESIGN: The ACTION (Accesso alle Cure e Terapie Intensive Ostetriche e Neonatali) study recruited during an 18-month period all infants 22-31 weeks' gestational age admitted to neonatal care in 6 Italian regions (n=3040). We analyzed the data of 2974 babies without lethal or acutely life-threatening malformations. Cause-specific risks of death adjusted for competing causes were calculated, and region-stratified multiple Cox regression ana

Changes in the neonatal and infant mortality rate and the causes of death in Korea.

Neonatal mortality rate (NMR) or infant mortality rate (IMR) are the rate of deaths per 1,000 live births at which babies of either less than four weeks or of one year of age die, respectively. The NMR and IMR are commonly accepted as a measure of the general health and wellbeing of a population. Korea's NMR and IMR fell significantly between 1993 and 2009 from 6.6 and 9.9 to 1.7 and 3.2, respectively. Common causes of infantile death in 2008 had decreased compared with those in 1996 such as other disorders originating in the perinatal period, congenital malformation of the heart, bacterial se…

Prophylactic intravenous indomethacin for preventing mortality and morbidity in preterm infants.

BACKGROUND: Persistent patent ductus arteriosus (PDA) is associated with mortality and morbidity in preterm infants. Prostaglandin synthetase inhibitors such as indomethacin promote PDA closure but also have potential side effects. The effect of the prophylactic use of indomethacin, where infants who may not have gone on to develop a symptomatic PDA would be exposed to indomethacin, warrants particular scrutiny. OBJECTIVES: To determine the effect of prophylactic indomethacin on mortality and morbidity in preterm infants. SEARCH STRATEGY: The standard search strategy of the Cochrane Neonatal
