Urinary tract infections.

Urinary tract infections (UTIs) are still the precipitating cause for 7 million patient visits per year with total costs exceeding one billion dollars. Diagnostic modalities have become more "friendly" for the smaller laboratory with "dip stick" culture tests providing a rapid method of isolation of pathogens. In many cases, empiric therapy is more cost effective than culture in uncomplicated UTIs in women. The etiologic organisms implicated in UTIs have not changed dramatically over the past two decades, with E. coli still accounting for the majority of cases. Antibiotic susceptibility patter…

[Nosocomial urinary infections].

The concept of nosocomial urinary tract infection now corresponds to a precise definition. It is generally related to bladder catheterization, constitutes the most frequent form of nosocomial infection (30 to 50% of infections), and represents the third most frequent portal of entry of bacteraemia. The organism most frequently isolated is Escherichia coli; but the flora is changing and the ecological distribution is continually modified. Despite their usually benign nature, these nosocomial infections can nevertheless influence hospital mortality; they increase the hospital stay by an average …

Genital mycoplasma infections.

Since 1937, 13 Mycoplasma species, two Acholeplasma species, and one Ureaplasma species have been isolated from humans. Six of these have the urogenital tract as the primary site of colonisation but others, which have the oropharynx and respiratory tract as the primary site, are found occasionally in the urogenital tract because of orogenital contact. Mycoplasma hominis was the first to be isolated and is most strongly associated with bacterial vaginosis (BV), together with a variety of other bacteria. Its involvement in pelvic inflammatory disease (PID) and other conditions may be as part of

Nontuberculous mycobacterial infections.

OBJECTIVE: To review the epidemiology, clinical manifestations, diagnosis, and treatment of nontuberculous mycobacterial infections other than Mycobacterium avium complex (MAC). DATA SOURCES: A MEDLINE search of English-language literature pertaining to nontuberculous mycobacteria other than MAC was performed. Additional literature was obtained from reference lists of pertinent articles identified through the search. STUDY SELECTION AND DATA EXTRACTION: All articles were considered for possible inclusion in the review. Information judged by the author to be pertinent was selected for discuss…

Cervical spine infections.

Cervical spine infections arise from a variety of etiologies including postsurgical, iatrogenic, and hematogenous routes. Clinical history, physical examinations, and diagnostic studies all play an integral role in the diagnosis and treatment of these infections. Successful treatment depends on a proper and timely diagnosis, understanding the etiology, and defining the extent to which the infectious process involves the spinal supporting and neurologic elements. Surgical treatment is required when there is abscess formation, instability, progressive kyphosis secondary to vertebral body collaps…

Supraglottic airway infections.

Supraglottic infections are included among the more serious pediatric infections, although they are less common than other pediatric respiratory infections. Supraglottic infections include epiglottis (supraglottitis), retropharyngeal cellulitis, retropharyngeal abscess, and peritonsillar abscess. A high index of suspicion combined with rapid diagnosis and treatment are crucial to reducing the morbidity and mortality associated with these infections. A review of these infections, including diagnosis and treatment, is presented.

Urinary tract infections.

Urinary tract infections are common clinical problems which result in significant morbidity and even mortality. UTI's can range from minimal disease to life-threatening sepsis and it is important to differentiate between the former which usually involves the lower urinary tract and the latter which invariably involves the upper urinary tract. Diagnosis depends on an abnormal urine microscopy and demonstration of bacteria in the urine. Pre-therapy urine cultures are not mandatory in young women with uncomplicated UTI and many studies support the efficacy of short-course therapy in this groups o…

Neurologic opportunistic infections.

Three common opportunistic infections in patients with the human immunodeficiency virus are being more accurately diagnosed and effectively treated as a result of recent advances. Toxoplasmosis may be prevented in some cases, and can be recognized and treated noninvasively in most cases. Cryptococcal therapy has been enhanced by successful development of oral azole therapy. Cytomegalovirus encephalitis is now an entity that can be diagnosed antemortem, and thus efforts to treat it can now be evaluated.

Listeria Monocytogenes infections.

This article reviews current information regarding human infection with Listeria monocytogenes. Significant advances have occurred in regard to our knowledge of the epidemiology, pathogenesis, immunology, and treatment of this disease which was formerly believed to be of importance mainly to veterinarians. It remains a cause of high mortality in the many different groups of compromised hosts it infects unless diagnosis and treatment are rapidly established.

Soft tissue infections.

Several infectious processes ranging in severity from mild to overwhelming that affect the soft tissues are reviewed. Superficial infections such as impetigo, erysipelas, cellulitis, and subcutaneous bursitis are common and, for the most part, can be easily treated. Because suppurative tenosynovitis has the potential to destroy tendons, consultation with a hand surgeon should be obtained immediately. Mycobacterial and fungal tenosynovitis is usually chronic and patients present with a mass which, when excised and examined with appropriate stain and culture, reveals the unexpected diagnosis. Tr…

Intestinal protozoa infections.

This article reviews important intestinal protozoal infections in cats. Among intestinal protozoa, Giardia and coccidia are the most important infections. Giardiasis is a common problem in catteries, especially where young kittens run free on the floor. Overcrowding and high humidity favour the survival and transmission of Giardia. Diagnosis is made by fecal examination by direct or concentration methods. Metronidazole is the drug of choice for treating giardiasis. Among the coccidial parasites, Isospora felis and Cryptosporidium sp. are the common infections. Virtually all cats become infecte…
