Insulin infusion pump therapy is a treatment option that facilitates achieving improved blood glucose control and lifestyle flexibility. These advantages are derived from the physiologic mode of insulin delivery and the pharmacologic advantages of using rapid-acting insulin. Patients who are sufficiently motivated and capable can learn to use the pump so that insulin adjustments can be made to compensate for changing circumstances. We studied two obese type II diabetic patients before, during, and after 3 yr of continuous iv insulin infusion, delivered by means of totally implanted insulin infusion pumps. The totally implantable infusion pump (Infusaid pump) for treating metastatic disease to the liver is a new treatment that is gaining widespread acceptance. However, as might be expected for any new major surgical procedure, there are increasing reports of complications. We report a complication in which duodenal atony was caused by the infusion of the chemotherapeutic agent (FUDR) into the stomach and duodenum. PURPOSE: To assess the efficacy of continuous bupivacaine infusion at the iliac crest donor site in reducing postoperative pain and inpatient hospital stay.
METHODS: Forty consecutive cleft lip/palate patients who underwent alveolar bone grafting at a single institution between 2003 and 2008 were identified, and their charts were retrospectively reviewed. Opioid analgesic use, self-reported pain indices, and length of hospital stay were analyzed. Comparisons were made between those patients who received a bupivacaine infusion pump at the iliac crest harvest site and those who did not.
