BACKGROUND: Transforaminal epidural injections have been used since the late 1990s to treat lumbar radicular pain. They have been the subject of considerable attention, with varying conclusions from systematic reviews as to their efficacy. Transforaminal injections have been associated with rare but major complications. Further, the use of transforaminal injections has increased since the passage of the Affordable Care Act. Finally, with the SARS-CoV-2 pandemic, there has been heightened concern regarding the risk associated with steroid injections.
OBJECTIVES: To evaluate and update the effe

BACKGROUND: Epidural steroid injections (ESIs) may be beneficial for lumbar spinal stenosis (LSS) symptoms. Past studies found interferon-gamma, fibronectin-aggrecan complex, and electromyography (EMG) to predict patients' response to ESIs for other spinal pathologies, but no similar studies have been done for LSS.
OBJECTIVE: To explore the ability of biomarkers and EMG to help predict outcome after ESI in LSS.
DESIGN: Prospective observational cohort.
SETTING: The physical medicine & rehabilitation spine clinic at a tertiary care center.
PARTICIPANTS: Eleven patients with LSS were recruit

CONTEXT: Epidural steroid injections (ESIs) are a common, effective treatment of lumbar radiculopathy and sciatica. Although the negative skeletal effects of oral glucocorticoids are well established, little is known about the impact of ESI on bone quality.
OBJECTIVE: To investigate the relationship between ESI exposure and volumetric bone mineral density (vBMD) at the lumbar spine (LS) using central quantitative CT.
DESIGN: Retrospective study.
SETTING: University hospital outpatient facility.
PATIENTS: All patients had CT scans of the LS between 2011 and 2016. Cases received at least thr

BACKGROUND: Various types of lumbar dural punctures may contribute to neurological injury. The etiologies of dural injury include; inadvertent dural punctures due to epidurals placed for labor anesthesia, epidural steroid injections (ESI/transforaminal TESI; approximately 9 million ESI performed in the US per year), deliberate placement of intradural pain devices, and spontaneous cerebrospinal fluid (CSF) fistulas. Resulting neurological complications may include; spinal headaches/intracranial hypotension, subdural hematomas, and 6th nerve cranial palsies. Furthermore, uniquely in the cervical

OBJECTIVE: Response to lumbar epidural steroid injection in lumbar radicular pain varies. The purpose of this study is to characterize the changes in quantitative sensory testing (QST) phenotypes of subjects and compare the QST characteristics in patients who do respond to treatment of radicular pain with a lumbar epidural steroid injection (ESI).
DESIGN: Prospective, observational pilot study.
SETTING: Outpatient pain center.
METHODS: Twenty subjects with a lower extremity (LE) radicular pain who were scheduled to have an ESI were recruited. At the visit prior to and four weeks following a

Objective: A cervical epidural steroid injection is one of the most commonly performed interventions to manage chronic neck pain and cervical radiculopathy. Despite its many severe complications, cervical transforaminal epidural steroid injection (CTFESI) is a clinically necessary modality for managing neck pain and cervical radiculopathy. We aimed in this study to find a safer optimal needle entry angle to decrease the chance of an accidental vertebral artery (VA) puncture even with a proper needle entry angle and to visualize the target of the needle tip. Methods: This retrospective study in

OBJECTIVES: Recently, most lumbar spine injections have been administered under ultrasound (US) guidance; however, there is no standard method for US-guided lumbar transforaminal epidural injection (TFEI). In this study, we evaluated the accuracy, effect on pain relief, and safety of US-guided lumbar TFEI.
METHODS: A total of 80 patients with low back pain and radicular pain were enrolled. The patients were randomly assigned to either the fluoroscopy (FL) group or the US group. The FL-guided approaches were performed under standardized procedures using the C-arm, whereas the US-guided injecti

STUDY DESIGN: Prospective study OBJECTIVE.: The aim of this study was to evaluate variations in the sacral hiatus based on ultrasonographic images.
SUMMARY OF BACKGROUND DATA: Caudal epidural injection is commonly used for the treatment of lower back pain, but blind injections commonly lead to mistakes made at the injection site due to anatomic variations in the sacral hiatus.
METHODS: A total of 339 patients with low back pain or sciatica were studied using a linear-array ultrasound transducer. The transducer was placed at the sacral cornua and the distance between the bilateral cornua and

BackgroundThe purpose of this study was to determine the use of Central Epidural Steroid Injections (ESI) and Selective Nerve Root Blocks (SNRB) along with the crossover rate to lumbar discectomy in patients with a lumbar disc herniation using retrospective records database search. Butterman et al found a crossover rate for patients with symptomatic disc herniations treated with ESI of 54% (27/50), while Riew similarly found a 53% (29/55) crossover patients receiving SNRB.MethodsThe database was searched in a sequential Boolean style for patients with the diagnosis of a lumbar disc herniation

Lumbosacral radicular syndrome is characterized by radiating pain into a part of the leg typically served by one nerve root in the lumbar or sacral spine. The most common cause of radicular syndrome is a herniated disk. The estimated annual incidence of radicular syndrome in The Netherlands is 9 cases per 1000 adults per year. The course of radicular syndrome is favorable, with resolution of leg pain within 3 months from onset in the majority of patients. During the first few weeks post-onset treatment focusses on pain relief. Besides pain medication, transforaminal, fluoroscopic injections wi

BACKGROUND: Existing guidelines and systematic reviews provide inconsistent recommendations on epidural corticosteroid injections for sciatica. Key limitations of existing reviews are the inclusion of trials with active controls of unknown efficacy and failure to provide an estimate of the size of the treatment effect.
PURPOSE: To determine the efficacy of epidural corticosteroid injections for sciatica compared with placebo.
DATA SOURCES: International Pharmaceutical Abstracts, PsycINFO, MEDLINE, EMBASE, Cochrane Central Register of Controlled Trials, and CINAHL.

The authors report a case of epidural and extraforaminal calcification caused by repetitive triamcinolone acetonide injections. A 66-year-old woman was admitted presenting with lower extremity weakness and radiating pain in her left leg. Ten months before admission, the patient was diagnosed as having an L4-5 spinal stenosis and underwent anterior lumbar interbody fusion followed by posterior fixation. Her symptoms had been sustained and she did not respond to transforaminal steroid injections. Repetitive injections (10 times) had been performed on the L4-5 level for six months. She had been t
