[Intraoperative use of ultrasound in neurosurgery].

Modern ultrasound technique, as in other specialities, has provided the neurosurgeon with an adjuvant which may be employed intraoperatively and which supplements the neuroradiological methods of investigation. The technique provides the surgeon with the possibility of direct visualization, localization and characterization of the pathological and normal structures intracranially and intraspinally by scanning directly on the surfaces of the dura or brain or medulla. The technique provides a good possibility for planning and controlling the extent of tumour resection. By means of a stand with g

A proposal for intraoperative washout of colon for the endoscopic diagnosis of the lower gastrointestinal tract haemorrhage.

The authors refer about a proposal for an intraoperative wash-out of colon in order to improve the endoscopic diagnosis of the bleeding's precise site in case of lower gastrointestinal tract haemorrhage. The colonic wash-out is obtained by the transanal insertion of a Baker type silicone tube this is guided by the surgeon up to the caecum followed by the insertion of the colonoscope. This method turned out useful in one case of haemorrhagic shock.

[Importance of lateral inspection of the scrotum and value of intraoperative spermatic phlebography in varicocele].

Varicocele appearing on the posterior surface of the scrotum is apt to be overlooked in a routine physical examination. Lateral inspection of the scrotum is important to detect a varicocele of this type. Intraoperative spermatic phlebography on high ligation for varicocele was valuable in confirming the ligation of a single vein or in visualizing the vessel(s) left unligated.

Prevention and management of potential adverse events during transapical aortic valve replacement.

BACKGROUND AND AIM OF THE STUDY: Transapical transcatheter aortic valve replacement (TAVR) is a new minimally invasive technique with a known risk of unexpected intra-procedural complications. Nevertheless, the clinical results are good and the limited amount of procedural adverse events confirms the usefulness of a synergistic surgical/anesthesiological management in case of unexpected emergencies. METHODS: A review was made of the authors' four-year database and other available literature to identify major and minor intra-procedural complications occurring during transapical TAVR procedures

Confirmation of Parathyroid Tissue: Are Surgeons Aware of New and Novel Techniques?

BACKGROUND: Ex vivo aspiration of parathyroid glands for the measurement of intraoperative parathyroid hormone (IOPTH) levels is a rapid point-of-care method to confirm parathyroid tissue during parathyroidectomy and an alternative to frozen section (FS). This study sought to determine the awareness and utilization of this technique among endocrine surgeons. MATERIALS AND METHODS: A de-identified 12-question survey regarding techniques for intraoperative identification/confirmation of parathyroid tissue and the use of IOPTH monitoring was distributed to all 608 members of the American Associa

Different approaches for complete mobilization of the splenic flexure during laparoscopic rectal cancer resection.

PURPOSE: Laparoscopic resection of rectal cancer has already become the standard procedure in many hospitals. The splenic flexure mobilization (SFM) is an important preparational step. Several methods are used for laparoscopic SFM; however, studies comparing different approaches are lacking. In the present study, three different approaches for SFM have been compared to each other. METHODS: Between January 1998 and December 2010, 415 patients with rectal adenocarcinoma underwent laparoscopic rectal resection at one center. Of these, 303 patients received complete splenic flexure mobilization.

[Endoscopic sinus surgery in flowing water].

A balloon has been developed that completely fills the choana, preventing water from leaking into the pharynx even when the water is entering into the nasal cavity at a rate of 1000 ml per minute. The balloon enables endoscopic sinus surgery (ESS) to be safely performed in "flowing water". This surgical technique is similar to that used in transurethral resections of the prostate because the tip of the endoscope is kept clean, and blood, debris and resected tissues are continuously removed by the water flow. In addition, the water pressure helps to suppress bleeding. This technique enables ESS

[Severe hypotension as a complication of intramyometrial injection of vasopressin: a case report].

A thirty-year-old woman was scheduled for laparoscopic myomectomy. After insertion of an epidural catheter at the L4-5 interspace, general anesthesia was induced with thiopental 250 mg followed by vecuronium 8mg intravenously to facilitate tracheal intubation. General anesthesia was maintained with sevoflurane and nitrous oxide. Just after intramyometrial injection of vasopressin, blood pressure decreased from 122/66 to 45/25 mmHg, and heart rate decreased from 52 to 45 beats x min(-1). The patient was ventilated with 100% oxygen, and we administered atropine 0.25 mg and ephedrine 16 mg intrav

[The effect of prophylactic nicorandil infusion to reduce intraoperative myocardial ischemia during abdominal surgery in patients with risk factors of ischemic heart disease].

Three hundred patients who had undergone abdominal surgery with risk factors of ischemic heart disease (IHD), such as hypertension, diabetes mellitus, hyperlipidemia, smoking, advanced age, obesity, familial history, electrocardiographic abnormality, other perivascular disease, and male, were included in this study. Patients older than forty years were included in the study. They were divided into two groups for a randomized double blind trial: nicorandil was administered in one group (group N) and placebo in another (group C), and we investigated the effect of prophylactic nicorandil infusion

Respiratory Depression Following Intraoperative Methadone: A Retrospective Cohort Study.

BACKGROUND: Methadone is used as a perioperative analgesic in the management of postoperative pain. Despite positive outcomes from randomized trials favoring methadone, concerns about its safety persist, particularly regarding respiratory depression (RD) and excessive sedation. In this study, we compared the incidence of naloxone administration between patients administered intraoperative methadone and those administered intraoperative morphine as a measure of severe RD. Time spent at the postanesthesia care unit (PACU) was used as a proxy variable for excessive sedation. METHODS: This was a

Physiology, monitoring, and optimisation of perioperative tissue oxygenation: a narrative review.

Maintenance of tissue oxygenation in patients having surgery is important as tissue hypoxia is a major determinant of organ failure. Tissue oxygenation follows a stepwise physiological pathway involving the macrocirculation, the microcirculation, and the cellular oxygen metabolism. This narrative review endorsed by the Network for the Advancement of Patient Blood Management, Haemostasis and Thrombosis outlines the physiology of tissue oxygenation, evaluates methods for intraoperative tissue oxygenation monitoring, and summarises therapeutic strategies to ensure adequate tissue oxygenation. Perioperative cardiac arrest: a rare event worth measuring.

Perioperative cardiac arrest remains a rare but devastating event, particularly among emergency surgery patients. Work by Anezaki and colleagues confirms that emergency and elective populations represent distinct patient groups requiring different care pathways. Anaesthetists should consider patient- and procedure-level factors to anticipate and mitigate risk, while embracing upstream prevention and strengthening perioperative systems. Healthcare systems must focus on reducing variation in care between institutions to improve overall outcomes.

Routine intraoperative cholangiography during laparoscopic cholecystectomy: application of the 2016 WSES guidelines for predicting choledocholithiasis.

BACKGROUND: Routine use of intraoperative cholangiography (IOC) during laparoscopic cholecystectomy (LC) for detecting common bile duct stones remains controversial. The 2016 World Society of Emergency Surgery (WSES) guidelines on acute calculous cholecystitis proposed a risk stratification for choledocholithiasis. Our present study aimed to (1) examine the findings of common bile duct (CBD) stones in patients underwent LC with routine use of IOC, and (2) validate the 2016 WSES risk classes for predicting choledocholithiasis. METHODS: All patients had LC with IOC routinely performed from Nove

Intraoperative interventions for preventing surgical site infection: an overview of Cochrane Reviews.

BACKGROUND: Surgical site infection (SSI) rates vary from 1% to 5% in the month following surgery. Due to the large number of surgical procedures conducted annually, the costs of these SSIs can be considerable in financial and social terms. Many interventions are used with the aim of reducing the risk of SSI in people undergoing surgery. These interventions can be broadly delivered at three stages: preoperatively, intraoperatively and postoperatively. The intraoperative interventions are largely focused on decontamination of skin using soap and antiseptics; the use of barriers to prevent movem

Supercooled Liquid Serum Physiologic Solution Instantly Crystallized on the Nurse Table Used for Cooling of Periorbital Region During Rhinoplasty.

INTRODUCTION: Formation of less periorbital ecchymosis in post-operative period of rhinoplasty is a popular trend. We present the use of instantly crystallizing supercooled serum physiologic solution for periorbital cooling. PHYSICS OF SUPERCOOLING: There are circumstances in which water temperature drops below its freezing point, but no phase transition happens while water remains in the liquid phase. This is called supercooling. Pure water can be supercooled below the freezing temperature without transforming into ice. Tap water will not supercool because it contains impurities that serve a

How not to bite the bullet: A novel technique for intra operative handling of bullets.

Ballistic trauma caused by bullets can be fatal or cause serious injury. Despite the abundance in literature in management of bullet injuries, there is paucity in describing how the operating surgeon should handle the bullet intra operatively. Bullets are important pieces of evidence and evidence preservation becomes the responsibility of the surgeon. We suggest a novel technique in handling bullets intraoperatively which, to the best of our knowledge, has not been published before. We also suggest general recommendations in handling bullets intraoperatively.
