A massive mandibular keloid with severe infection: What is your treatment?

A case report of massive mandibular keloid with severe infection induced by acne achieved resolution of skin lesions after combined treatment with surgery and high concentration single-dose 5-aminolevulinic acid photodynamic therapy (5-ALA PDT). The patient achieved satisfactory effects, after receiving combined treatment with radiotherapy, secondary healing, intralesional injection of glucocorticoids, and other treatments. The scar didn't exhibit growth in a follow-up check after a year. This case provides evidence that photodynamic therapy is effective in the treatment of massive mandibular …

Botulinum toxin type A versus 5-Fluorouracil in treatment of keloid.

Most therapeutic approaches for keloids remain clinically unsatisfactory. In the last years, intralesional botulinum toxin-A (IL BTX-A) was proposed for treatment of keloids. Our aim of the study was to compare the clinical efficacy of IL BTX-A and IL 5-Fluorouracil (IL 5-FU) in treatment of keloids. A total of 50 patients with keloids were included in the study, 22 patients (with 26 keloids) were treated with IL BTX-A monthly for up to 6 months and other 22 patients (with 27 keloids) were treated with IL 5-FU weekly for up to 6 weeks, while the remaining 6 patients, each having multiple keloi…

Keloid Formation after Circumcision: A Case Report and Current Literature Review.

Although penile keloid formation can be seen after major penile surgeries, it is rarely reported after circumcision and there is no standard method for the treatment of this complication. We present a patient who was admitted with a penile keloid mass that occurred after circumcision surgery and discuss the treatment we administered in light of the current literature review. A 7-year-old white boy was admitted to our clinic with a swollen stiff mass on the foreskin six months after circumcision. The parents indicated that no complication occurred in the early postoperative period. Physical exa…

The "Sandwich Therapy": A Microsurgical Integrated Approach for Presternal Keloid Treatment.

BACKGROUND: Keloid therapy remains a great challenge for plastic surgeons, especially when the defect cannot be closed primarily, necessitating tissue transplantation. Here, we introduce a new treatment modality, called the sandwich therapy, for presternal keloids; the sandwich therapy incorporates preradiotherapy, superficial circumflex iliac artery perforator (SCIP) flap transplantation, and postradiotherapy. METHODS: From December 2012 to October 2013, 12 patients received the "sandwich therapy." For the protocol, all patients went through 5 days of specific events: the precut procedure, p…

Corneal Myofibroma (Keloid) in a Young Patient with Neurofibromatosis Type 2.

We present a 27-year-old male patient with neurofibromatosis type 2 (NF2), facial palsy, and lagophthalmos following acoustic neuroma removal and an impressing vascularized corneal tumor, which was excised. Histology showed a fibrous tumor with small vessels, and immunohistochemistry was positive for vimentin and negative for smooth muscle actin, S100, and GFAP. We assume a corneal myofibroma (keloid), which in this case rather represents a reactive lesion. This entity has not been described before in NF2 or in facial palsy-associated lagophthalmos in general.

Custom made pressure appliance for presurgical sustained compression of auricular keloid.

Keloid is dermal lesion characterized by nodular fibroblastic proliferation, which is considered an aberration of wound healing process. It is believed to be the confused scar that does not know when to stop growing. Pressure therapy using clips or splints is widely used for the treatment of keloids; however, it is often very difficult to control the amount and direction of pressure applied. Among the most common complications of this therapy is ulceration due to excessive pressure. A case of presurgical size reduction for a large ear keloid with a custom made pressure appliance is presented. …

Treg-enriched CD4+ T cells attenuate collagen synthesis in keloid fibroblasts.

Keloid is an inflammatory and fibrotic disease with an unknown pathogenesis. Regulatory T cells (Tregs) of CD4+ lineage can suppress other effector CD4+ T cells and modulate the immune response. A relative decrease in the number of Tregs may be involved in the pathogenesis of inflammatory and fibrotic diseases. We therefore investigated the number of Tregs in keloids using immunohistochemistry and examined the interaction between Tregs and keloid fibroblasts (KFs) using a coculture system. It was found that the ratio of Tregs/CD4+ T cells was lower compared with that in other common inflammato…

Immunohistochemical analysis of Fas protein expression in hypertrophic scar and keloid.

OBJECTIVE: To find an efficient diagnostic method of hypertrophic scar and keloid. METHODS: Immunohistochemistry was employed to detect the expression and distribution of Fas protein in keloids, hypertrophic scars and their surrounding normal skins. Image analysis was performed to quantitatively compare the differences of the protein expression in these tissues. RESULTS: The expression level of Fas antigen was much higher in the keloids and their surrounding normal skins than in hypertrophic scars, and no significant difference in the expression level between the former 2 tissues was found. …

Analysis of Risk Factors and Exploration of Treatment for Infected Keloid.

OBJECTIVE: To identify independent risk factors for keloid infection and explore optimal therapeutic strategies for infected keloids. METHODS: A retrospective study included 131 keloid patients (26 infected, 105 non-infected) from two hospitals (July 2021-January 2025). Univariate and multivariate logistic regression analyses screened infection-related risk factors. Efficacy was compared among infected patients treated with complete surgical excision + radiotherapy (n= 12), intralesional injection + radiotherapy (n= 10), and local resection of infectious focus + injection + radiotherapy (n= 4…

Cytokines, chemokines and growth factors involved in keloids pathogenesis.

Keloid is a common fibrotic disease, which is difficult to treat. It often causes itching and pain, which greatly disturbs patients in their work and daily life and causing difficulties in social interaction. Its pathogenesis is not clear, but may be related to several aspects: genetic susceptibility, environmental, immunological and endocrine factors, trauma and tension. The central point of its pathogenesis is the excessive proliferation of fibroblasts, with excessive synthesis and secretion of extracellular matrix such as collagen. However, the cause of fibroblast excessive proliferation an…

A Prospective Study of Patients Presenting With Pre-sternal Keloids.

INTRODUCTION: Keloid represents a pathological form of scarring. They are very common in the anterior chest area; nearly 50% of all keloids occur in this location. One of the reasons for this is that folliculitis and acne, known for triggering the development of keloids, are common on the anterior chest. The other reason is the tension load in this area due to the frequent movements of the upper limbs and the respiratory movements. These movements stretch the skin of the anterior chest horizontally. When this cyclical tension is imposed on the anterior chest wounds, there is an exacerbation an…

The effects of systemic diseases, genetic disorders and lifestyle on keloids.

Keloid are a fibroproliferative disorder caused by abnormal healing of skin, specifically reticular dermis, when subjected to pathological or inflammatory scars demonstrating redness, elevation above the skin surface, extension beyond the original wound margins and resulting in an unappealing cosmetic appearance. The severity of keloids and risk of developing keloids scars are subjected to elevation by other contributing factors such as systemic diseases, general health conditions, genetic disorders, lifestyle and natural environment. In particular, recently, daily physical work interpreted in…

Ear Keloids Treated with Postoperative Electrons: A Case Series.

INTRODUCTION: Multiple modalities exist for treating keloids ranging from surgical, medical, chemotherapeutic, and radiation therapy. Different techniques of radiation therapy have been used to treat keloids as an adjuvant to surgical excision. With this case series, we report our experience of using electron beam radiation therapy in this setting. MATERIALS AND METHODS: We retrospectively analyzed 16 ear keloids treated in 10 patients from January 2013 to October 2015 with surgical excision followed by electron beam to a dose of 10 Gy in two fractions over two consecutive days in immediate p…

Efficacy and Safety of Contact Cryosurgery in the Treatment of Earlobe Keloids.

Earlobe keloids, although not a life-threatening disease, can severely impair a patient's quality of life. Contact cryosurgery is a safe technique for the management of earlobe keloids with tolerable side effects. The aim of this study was to assess the safety and efficacy of contact cryosurgery in the treatment of earlobe keloids through a 1-year prospective study. We treated 15 Caucasian women ranging in age from 8 to 54 years with a total of 18 earlobe keloids. Excellent improvement was noted in 11 earlobe keloids, with a 75%-100% reduction in the volume of the keloids. An improvement rangi…

Inhibitory Effect of the LY2109761 on the Development of Human Keloid Fibroblasts.

Keloids are scars characterized by abnormal proliferation of fibroblasts and overproduction of extracellular matrix components including collagen. We previously showed that LY2109761, a transforming growth factor- (TGF-) β receptor inhibitor, suppressed the secretion of matrix components and slowed the proliferation of fibroblasts derived from human hypertrophic scar tissue. However, the exact mechanism underlying this effect remains unclear. Here, we replicated the above results in keloid-derived fibroblasts and show that LY2109761 promoted apoptosis, decreased the phosphorylation of Smad2 an…

Scoping Review of Therapeutic Strategies for Keloids and Hypertrophic Scars.

BACKGROUND: Keloids are an abnormal proliferation of scars that can involve large areas of tissue beyond the original injury site. Hypertrophic scars are similar clinically, but do not exceed the original scar limits. These scarring abnormalities can cause noxious symptoms such as pain, tenderness, itching, and ulcerations. The aim of this review is to discuss current therapies for both types of abnormal scarring, and to determine if guidelines can be provided for excisional treatment with adjuvant therapies versus non-excisional methods. METHODS: A systematic literature search was performed …
