The trigeminal nerve provides corneal sensitivity and trophic supply to corneal tissues. The impairment of corneal innervation leads to development of neurotrophic keratitis (NK). NK is a rare, degenerative corneal disease characterized by corneal hypo/anesthesia and development of nonhealing corneal epithelial defects and ulcers. NK is a challenging condition with high medical need due to the lack of approved treatments that can restore corneal integrity. Current treatment of NK aims at stimulating corneal healing and preventing disease progression. Cenegermin is a recombinant human nerve gro…

OBJECTIVE: To determine the characteristics of infectious keratitis caused by Kocuria spp.
METHODS: Retrospective case series. Information included demographic data, medical history, risk factors associated with infectious keratitis, clinical characteristics, microbiological results and drug sensitivity, clinical course, management, and visual outcomes.
RESULTS: Eight patients were included (7 females and 1 male; mean age, 66.2 years; age range, 42-84 years). All patients presented risk factors for infectious complications, such as filamentous keratitis, dry eye, blepharitis, and persistent …

Pattern recognition receptors (PRRs) are critical to the early detection and innate immune responses to pathogens. In particular, the toll-like receptor (TLR) system and its associated adaptor proteins have essential roles in early host responses to infection. Epidemic keratoconjunctivitis, caused by the human adenovirus, is a severe ocular surface infection associated with corneal inflammation (stromal keratitis). We previously showed that adenovirus capsid was a key molecular pattern in adenovirus keratitis, with viral DNA having a lesser role. We have now investigated the role of the adapto…

PURPOSE: To analyse predictors of clinical outcome in fungal keratitis.
METHODS: Data was collected during a prospective, randomized, controlled, double-masked clinical trial of treatment for fungal keratitis. Clinical features at presentation and demographics were collected at the enrollment visit for all patients. Pre-specified clinical outcomes included 3-month visual acuity and infiltrate/scar size, time to re-epithelialization, and corneal perforation. A separate multivariable model with each outcome as the dependent variable included all predictor variables.
RESULTS: Predictors for wor…

Peripheral ulcerative keratitis is a destructive process involving perilimbal corneal inflammation and an overlying epithelial defect. It can be a manifestation of systemic autoimmune diseases or can be secondary to an infectious process. We present a case of a 7-year-old girl with peripheral ulcerative keratitis of unknown etiology. To the best of our knowledge, this is the first such case reported in a child.

PURPOSE: To identify microbiologic characteristics of infectious keratitis and predisposing factors.
METHODS: The microorganisms isolated from patients with infectious corneal ulcers during the 5 years between January 1999 and December 2003 were retrospectively examined. Herpetic and other viral infections were excluded from this study. Patient age, sex, risk factors, and isolated organisms were collected.
RESULTS: In 123 eyes of 122 patients, organisms were isolated from 72 (58.5%) eyes. In total, 99 different organisms were isolated. Among these, 77 (77.8%) were gram-positive bacteria; 18 …

PURPOSE: To evaluate the ability of ophthalmologists to predict the laboratory results of presumed microbial keratitis and to explore which findings may influence diagnostic prognostication.
DESIGN: Prospective, cross-sectional study.
METHODS: Fifteen ophthalmologists completed study forms at the initial presentation of patients with presumed microbial keratitis. After predicting the category of microbial recovery, clinicians submitted corneal scrapings for masked laboratory processing. The relative effects of ocular inflammatory signs on correct microbial diagnosis were explored with Poisso…

A paracentesis is performed in glaucoma procedures as a flat, beveled tract to allow access into the anterior chamber after the filtration fistula is made. Complications related to the paracentesis are infrequent because it is a self-sealing wound. The authors report a case of infectious keratitis that developed in a paracentesis tract.

We present three cases of acne rosacea keratitis that developed in childhood. All three children were prepubescent and demonstrated characteristic dermatologic findings involving the nose, cheeks, and/or chin. The patients had complained of ocular irritation for at least 6 months, and in one case symptoms were reported by the family to have occurred intermittently since age 4 years. All three children showed evidence of meibomian gland inflammation; two patients demonstrated bilateral keratitis, the third had only unilateral involvement. In each case, oral tetracycline hydrochloride or doxycyc…

Vaccinial epithelial keratitis was produced in rabbits. When the therapeutic effect of vidarabine on the experimentally-induced disease was evaluated and compared to the effect of idoxuridine, vidarabine was found to be highly effective, substantially more effective than idoxuridine, and nontoxic to the eye.

A 31-year-old female presented with a 3-week history of right eye redness, ocular pain, photophobia and decreased vision after tap water exposure. Slit-lamp examination revealed corneal lesions with pseudopodia and hill-shaped hypopyon. In vivo confocal microscopy (IVCM) detected filament-like structures in the corneal stroma. The patient was initially suspected of having fungal keratitis, but the condition deteriorated despite standardized antifungal treatment. Repeated corneal scraping culture and targeted DNA sequencing confirmed the infection of Pythium insidiosum. After switching to linez…

Fungal keratitis is a major cause of corneal blindness, particularly in tropical and low-resource settings, and remains challenging to manage due to delayed diagnosis, limited drug penetration, emerging resistance, and host-mediated tissue damage. Conventional antifungal therapies, including natamycin and voriconazole, are often inadequate in deep, severe, or refractory infections, underscoring the need for alternative strategies. This narrative review summarizes and critically appraises emerging treatments in fungal keratitis, with a focus on recent pharmacological advances, innovative drug d…

Herpes simplex keratitis (HSK) caused by herpes simplex virus type 1 (HSV-1) is a major cause of infectious corneal blindness in developed countries. The disease reflects a variable combination of viral replication, immune-mediated inflammation and neurotrophic dysfunction, whose respective contributions determine both the clinical presentation and therapeutic strategy. Standard management relies on antiviral agents to suppress viral replication, corticosteroids to control immune stromal involvement and ocular surface optimisation to address chronic inflammation and neurotrophic impairment. Re…

Mycotic keratitis is a rare but serious infection of the cornea that occurs particularly in wearers of contact lenses and in patients with systemic diseases, such as diabetes or immunosuppression. It is often caused by filamentous fungi such as Fusarium spp. and Aspergillus spp. or by yeasts, in particular Candida spp. The diagnosis of mycotic keratitis is challenging, as clinical signs are often non-specific. Diagnosis includes in vivo confocal microscopy, fungal cultures and histopathological examinations. Early and targeted diagnosis is crucial, in order to avoid serious complications. Trea…
