Newborn Bilirubin Screening for Preventing Severe Hyperbilirubinemia and Bilirubin Encephalopathy: A Rapid Review.

UNLABELLED: According to the 2004 American Academy of Pediatrics guideline on the management of hyperbilirubinemia, every newborn should be assessed for the risk of developing severe hyperbilirubinemia with the help of predischarge total serum bilirubin or transcutaneous bilirubin measurements and/or assessments of clinical risk factors. The aim of this rapid review is 1) to review the evidence for 1) predicting and preventing severe hyperbilirubinemia and bilirubin encephalopathy, 2) determining the efficacy of home/community treatments (home phototherapy) in the prevention of severe hyperbil

Bilirubin production and the risk of bilirubin neurotoxicity.

Neonatal jaundice usually occurs in the transitional period after birth, presenting as an elevation of circulating bilirubin. Bilirubin neurotoxicity can occur if the levels of bilirubin become excessive (hyperbilirubinemia). This pathologic phenotype of newborn jaundice can develop because of excessive bilirubin production or impaired conjugation, with the risk for developing bilirubin-induced neurologic dysfunction, depending on the degree of the resultant bilirubin load. The plasma bilirubin level thus can be used to assess an infant's risk for developing bilirubin neurotoxicity relative to

Neonatal Indirect Hyperbilirubinemia.

Neonatal indirect hyperbilirubinemia (IHB) is caused by an imbalance in bilirubin production and elimination. Approximately 60% of term and 80% of preterm infants develop jaundice in the first week of age. This review seeks to provide the reader with a thorough understanding of the physiology of bilirubin, etiology of IHB, and management of severe IHB. Phototherapy and exchange transfusion remain the mainstays of treatment for severe IHB. Noninvasive screening tools, innovative treatments, and a better understanding of how prematurity and genetics contribute to severe IHB have improved our und

Liver Fibrosis Associated With Crigler-Najjar Syndrome in a Compound Heterozygote: A Case Report.

Crigler-Najjar syndrome is a hereditary unconjugated hyperbilirubinemia. Two forms of the disease are recognized. Type I is more severe and results in kernicterus if left untreated, and Type II is less severe and responds to phenobarbital. While Crigler-Najjar syndrome is thought by many to have normal liver histology, few reports of the liver pathology exist. Herein, we present a 19-year-old patient with Crigler-Najjar who underwent liver transplantation. The liver showed marked canalicular cholestasis with portal and variable, delicate, bridging fibrosis. Correlation of the patient's genetic

Hyperbilirubinemia in the 2000s: what should we do next?

Changes in healthcare policy mandating early discharge of healthy, full-term infants and the higher prevalence of breast-feeding of newborns have contributed to an increased number of term infants being readmitted to the hospital for hyperbilirubinemia. At high levels, total serum bilirubin causes kernicterus, which is associated with severe neurological sequelae. This article provides a review of prior studies that have examined whether and at what levels total serum bilirubin affects neurodevelopmental outcome in term infants. In addition, the article reviews some approaches in the literatur

Dyskinetic and dystonic cerebral palsy and birth.

Two hundred and nineteen cases of the dyskinetic and dystonic forms of cerebral palsy which were seen in the course of three decades at a single clinic have been analysed. Fifty-seven patients had kernicterus. In the remaining 162, 71% of whom were born at term, birthweight was below the expected mean in two-thirds. There was no relationship between birth weight, or abnormal birth, or asphyxia, and the ultimate clinical severity of the children. We conclude that abnormal birth and asphyxia are not direct causes of the cerebral damage, but are expressions of a pre-existing condition resulting i

[Pathogenesis of bilirubin encephalopathy and subsequent therapeutic consequences].

At present the classic "kernicterus" has nearly disappeared, but toxic effects of bilirubin, nevertheless, play an important role in the etiology of late neurologic sequelae in VLBW infants. Even in very low concentrations of serum bilirubin CNS damages have to be expected when other metabolic disturbances induce the opening of blood-brain-barrier. Since safe criteria for predicting toxic effects of bilirubin do practically not exist, in VLBW infants with perinatal metabolic disorders early preventive phototherapy is indicated to keep the serum bilirubin level as low as possible. Experimental

Estimation of bilirubin binding capacity of neonatal serum.

One aim of laboratory investigation in neonatal jaundice is to prevent kernicterus from occurring. Estimation of the bilirubin-binding capacity of serum is an important measurement along with total bilirubin determination. With a convenient Sephadex competitive binding procedure, KERNLUTE provides information on whether or not the neonate's bilirubin-binding capacity has been exceeded. Further testing of those serum specimens which show some residual binding capacity estimates whether the remaining bilirubin capacity is equivalent to 0 to 2.5 mg, 2.5 to 5 mg or more than 5 mg bilirubin per dl.

Glucose-6-phosphate dehydrogenase deficiency in Tunisian jaundiced neonates.

BACKGROUND AND OBJECTIVES: Glucose-6-phosphate dehydrogenase (G6PD) deficiency is the most common human enzymopathy worldwide associated with hemolysis as well as neonatal jaundice, kernicterus, and even death. The goal of this study is to determinate the prevalence of G6PD deficiency in icteric neonates and to investigate its biochemical, hematological and molecular characteristics. PATIENTS AND METHODS: This cross sectional study was carried out on 154 icteric newborns admitted to the Bechir Hamza Children's Hospital in Tunisia. Laboratory evaluations included complete blood count, total se

[Increasing prevalence of G6PD deficiency in the Netherlands].

Glucose-6-phosphate dehydrogenase (G6PD) deficiency is an X-linked enzyme deficiency with a high prevalence in Sub-Saharan countries and countries in the Middle East. Due to increased migration in the past decennium, the prevalence of this disorder has increased in the Netherlands. Here we present three patients with acute haemolysis due to G6PD deficiency: a male neonate with severe hyperbilirubinaemia leading to kernicterus; a 3-year-old girl with a severe acute haemolysis due to an infection; and a 5-year-old boy with acute haemolysis with no clear cause. These cases serve to trigger the aw

[Hereditary spherocytosis. Review. Part II. Symptomatology, outcome, complications, and treatment].

Hereditary spherocytosis must always be suspected in children with anemia, hyperbilirubinemia, splenomegaly or cholelithiasis, in the asymptomatic individual with an affected relative, and in the neonate with hyperbilirubinemia with no blood group incompatibility; its early detection is key to avoid kernicterus. Follow-up of these patients is based on periodical control and supply of information on the adequate management of hemolytic or aplastic crisis, and early detection of cholelithiasis. The decision to perform splenectomy is usually associated with quality of life rather than life-threat

Should we resuscitate? Ethical dilemmas.

Over a 2-year period, there were 312 perinatal deaths. A total of 144 (46.2%) of these deaths were due to lack of or delayed resuscitation at birth. This group comprised 45 infants with multiple congenital abnormalities, 31 with severe birth asphyxia, 25 with meningomyelocele, 20 with infections and 15 with kernicterus. Taking the decision not to resuscitate may be difficult, perplexing and agonizing for the families and health professionals. The decision may be easier if one can prognosticate as to intact survival and development. The physician, though he is not God, has to be firm and decisi

[The bronze-baby-syndrome: a complication of phototherapy (author's transl)].

A case of intense grey-brown discoloration of the skin, serum and urine in a premature twin is reported when phototherapy was used to reduce hyperbilirubinemia. All features made the diagnosis of bronze-baby-syndrome most probable. We were able to detect an increased level of coproporphyrin in serum. The patient did not show any signs of a kernicterus. Possible speculations about the pathogenetic and prognostic value of elevated levels of coproporphyrins in newborn infants under treatment with phototherapy are discussed. In these cases we must be aware of possible neurotoxic effects, since inc

Community-based monitoring of neonatal jaundice using the point-of-care Bilistick® System 2.0.

BACKGROUND: Neonatal jaundice often peaks after discharge, creating the need for reliable post-discharge monitoring. This study evaluated the feasibility and safety of a hospital-to-community pathway using the Bilistick® System 2.0 within routine Community Health Family Services (CHFS). METHODS: This retrospective observational study was conducted within a Local Health Authority in northeastern Italy. Newborns eligible for out-of-hospital bilirubin testing according to a Diagnostic-Therapeutic Assistance Pathway (DTAP) were referred either to CHFS or to hospital follow-up based on bilirubin r

Systematic review of the safety of trimethoprim-sulfamethoxazole for prophylaxis in HIV-infected pregnant women: implications for resource-limited settings.

Daily prophylaxis with trimethoprim-sulfamethoxazole (TMP-SMZ) significantly decreases morbidity and mortality among people living with HIV. Some clinicians are reluctant to use TMP-SMZ in pregnant and breastfeeding HIV-infected women because of concerns about the possible teratogenicity when used in the first trimester and about its potential to induce hyperbilirubinemia near term and during early breastfeeding. We systematically reviewed evidence regarding the toxicity of TMP-SMZ prophylaxis in pregnant and breastfeeding women to help guide practice in resource-limited settings. We identifie

High- versus low-dose phototherapy for neonatal jaundice.

RATIONALE: Phototherapy is the mainstay of treatment for neonatal hyperbilirubinaemia. Increasing the dosage or intensity of phototherapy by various means is commonly practised for infants with high levels of serum bilirubin. It is unclear whether these measures of increasing phototherapy dosage are beneficial and safe for neonates. OBJECTIVES: To assess the effects of high-dose versus low-dose phototherapy on bilirubin level and associated clinical outcomes that constitute the major conditions in kernicterus spectrum disorder (KSD), such as acute bilirubin encephalopathy and kernicterus, as
