This article provides a summary of renal cancers that can affect adolescent patients. It encompasses both isolated and syndrome-associated cancers and the various renal complications associated with cancer therapy. Prompt recognition and appropriate referral and management, along with periodic long-term follow-up, will enhance both the survival and quality of life of young people afflicted with these serious diseases.

Diffuse atherosclerosis entails a 15-30% risk of plaques on renal arteries (ARAS), with a correlation with coronary atherosclerosis. Ischemi … However, it is the obligatory first step for angioplasty and stent positioning, indicated when the kidney is not atrophic. The indication for revascularization essentially depends on evaluation of the benefits vs risks of angioplasty or surgery. Some publications underscore the frequent stability of renal function and the fact that, revascularized or no …

Schistosomiasis is a parasitic infection that affects 200 million people and is directly responsible for an annual death of 20,000 patients. Three species are responsible for most of the morbidity in humans: Schistosoma hematobium in Africa, S. mansoni in Africa and South America, and S. Japonicum in the Far East. Renal involvement occurs mostly with S. hematobium infection as a consequence of fibrosis and calcification of tissue-trapped ova in the lower urinary tract, leading to obstruction, reflux, infection, and stone formation. The resulting interstitial nephritis may present with tubular

Renal failure is relatively common, but except in association with spina bifida or paraplegia it is unlikely to occur as a result of disease of the CNS. Renal failure, however, commonly affects the nervous system. The effects of kidney failure on the nervous system are more pronounced when failure is acute. In addition to the important problems related to renal failure there are both acquired and genetically determined diseases which may affect the kidney and the brain. Those acquired diseases include the vasculitides, the paraproteinaemias, and various granulomatous conditions (considered in …

Angiomyolipoma of the kidney is a clonal neoplasm, apparently part of a family of neoplasms derived from perivascular epithelioid cells. Early angiomyolipomas are small nodules composed of HMB-45-reactive spindle cells in the renal capsule, cortex, or medulla. Most angiomyolipomas are asymptomatic, and they are more common than previously appreciated, approaching 13 per 10,000 adults. They are much more prevalent in patients with tuberous sclerosis, where they often are accompanied by cysts and occasionally by renal cell carcinoma. That they are rarely diagnosed before puberty in patients with…

The kidney is important not only as a target organ of hypertension but also as a organ that may cause hypertension. For many years, we have studied basic and clinical aspects of the role of the kidney in hypertension, including the renin-angiotensin, kallikrein-kinin, and prostaglandin systems, glomerular hemodynamics, therapeutic modalities, and prognosis. This review summarizes some of our results together with those of other relevant studies in the literature.

The risk for renal insufficiency by uric acid precipitation in medulla of kidney correlates with the degree of uric acid supersaturation in the urine, depending on uric acid concentration and urinary pH. The patients with gout or hyperuricemia have sometimes acidic urine and increased uric acid excretion. Accordingly, these patients frequently accompany by renal insufficiency. Improvement of hyperuricosuria, increasing of urine volume, and alkalinization of urine to pH6 6.5, are effective for the prevention from renal insufficiency. Acute renal failure related to hyperuricemia, can also occure…

A host of abnormalities of renal structure and function accompanies advancing age. An appreciation of methodologic considerations, including population selection, that might confound the assessment of the effects of aging on renal function has prompted a recent reappraisal. Earlier studies assessed the effects of aging by utilizing cross-sectional studies and institutionalized elderly subjects, with their attendant drawbacks. Recent longitudinal studies have utilized appropriate patient cohorts, selected for lock of renal disease, including potential kidney transplant donors. These studies ind…

In humans, urine formation starts with the metanephros at the 10th week of gestation. Nephrogenesis progresses during gestation and is achieved around the 35th week. Clamping of the cord is the signal for a striking increase in renal function which reaches mature levels at the end of the first year of life. The integrity of several hormonal systems (the renin-angiotensin system, the prostaglandins) is mandatory for kidney growth and the development of renal function. The mechanisms underlying renal homeostasis are fragile and can easily be disturbed during respiratory and cardiovascular distre…

The very rare occurrence of ascariasis of the kidney is reported in a 6-year-old boy from Cape Town, South Africa. He was acutely ill with pyonephrosis and passed adult Ascaris lumbricoides per urethra before having a nephrectomy. There was no evidence of a fistula between the intestines and the urinary system, and as the development of larvae into mature helminths within the renal system is not considered to be possible it is likely that mature worms gained entrance through the urethra. It appears that the worms obstructed the right ureter and caused hydronephrosis, pyonephrosis, and depositi…

Aristotle incorrectly observed the absence of the kidney in fish and birds and deduced that it was not essential for the existence of a living organism. This underlies his observations on structure and function of the kidney. From examination of rhesus monkeys he generalized that the right kidney is higher than the left. Aristotle did not consider that the renal pelvis is divided by a filter membrane into 2 chambers, and wrote that no blood reaches the renal pelvis. The theory of the 'filter kidney' cannot thus be attributed to Aristotle. The function of the kidney was described as being to se…

A multifocal cavernous hemangioma was found in the renal pelvis of a young male, in whom intermittent macroscopic hematuria developed. Selective angiography was found to be normal on two occasions. By means of preoperative ureterorenoscopy a presumptive diagnosis of hemangioma was made. Nephrectomy was performed because the location of the lesion precluded partial nephrectomy.

Pathophysiologic and hereditary mechanisms impacting on the kidneys are crucial for the initiation and maintenance of essential hypertension. An appreciation of these renal mechanisms is important for the institution of appropriate antihypertensive therapy. The clinician must develop a physiologic algorithm to control systemic blood pressure while maintaining adequate blood supply to the kidneys. This approach is particularly important in patients at higher risk for progressive renal insufficiency (i.e., older patients, blacks, diabetics, and those with chronic renal failure). Better perfusion…

The aging kidney suffers reduction both in mass and in glomerular filtration rate. These changes may be totally or partially due to atherosclerosis and hypertension, which reduce renal blood flow. Superimposed on these processes, and perhaps responsible for primary loss of renal mass irrespective of renal vascular disease, is glomerular damage and involution that is a consequence of adaptive increases in glomerular perfusion pressure that occurs as the number of nephrons decline with age. The data available at this time do not allow us to distinguish between these two potential mechanisms of r…

The paper is a review of certain aspects of importance of cadmium and the kidney regarding the assessment of risks and understanding of mechanisms of action. The review discusses the following topics: history and etiology of cadmium-induced kidney dysfunction and related disorders; cadmium metabolism, metallothionein and kidney dysfunction; cadmium in urine as indicator of body burden, exposure and kidney dysfunction; cadmium levels in kidney and liver as indicators of kidney dysfunction; characteristics of early kidney dysfunction; the critical concentration concept; critical concentrations o…
