Calicopyelocystostomy in auto- and allotransplanted kidneys. Report of 2 cases.

A case of calicopyelocystostomy and one of pyelocystostomy are reported. In the first case, the operation was performed on an autotransplanted kidney. The operation was done in a patient with sponge kidney to facilitate spontaneous excretion of calculi. In the second case, the operation was performed in a patient with long-lasting urinary fistula which developed in a cadaver kidney transplant. Although the authors present only 2 cases, calicopyelocystostomy and pyelocystostomy can be recommended as a valuable method in complicated cases of recurrent nephrolithiasis and in patients with nonheal

Safety and efficacy of supracostal access in percutaneous renal surgery.

BACKGROUND AND PURPOSE: During percutaneous renal surgery, subcostal access is preferred because it carries no risk of injury to either the lungs or pleura. However, in some situations, a supracostal approach may provide more direct access and achieve a more satisfactory result than the subcostal approach. In this prospective study, we evaluated the safety and efficacy of supracostal approaches in percutaneous renal surgery. MATERIALS AND METHODS: Between 2004 and 2006, 30 patients underwent percutaneous renal surgery with a supracostal approach either as the sole or as a secondary access. Th

Fetal pelvi-ureteric junction obstruction: predictors of outcome.

OBJECTIVE: To examine the relationship between pre-natal ultrasonographic findings typical of pelvi-ureteric junction (PUJ) obstruction and post-natal renal function in the affected kidney. PATIENTS AND METHODS: This retrospective study comprised 35 infants in whom pre-natal ultrasonography had identified a unilateral pattern of dilatation of the renal pelvis and calices typical of PUJ obstruction. In each case, the presence of unilateral hydronephrosis had been confirmed post-natally by ultrasonography, and differential function and drainage evaluated by isotope renography. The results of is

Open intervascular nephron-sparing surgery for pyelocaliceal transitional cell carcinoma in solitary kidney planned with contrast-enhanced multidetector CT.

A 69-year-old man presented with a tumor involving the right renal pelvis and the middle and lower calyces in a solitary kidney. The patient was determined to preserve left renal function. Intervascular nephron-sparing surgery (NSS) was planned. A contrast-enhanced multidetector computed tomography (MDCT) was performed, providing 3-D reconstructions of the renal artery and collecting system in regard to the tumor. Two trunks of the anterior branch of the renal artery directed to the lower and middle parenchymal segments were identified. After dissection of the renal vessels, the anterior branc

Does size and site matter for renal stones up to 30-mm in size in children treated by extracorporeal lithotripsy?

OBJECTIVES: To determine the safety and efficacy of extracorporeal shock wave lithotripsy (ESWL) in the treatment of pediatric nephrolithiasis and to analyze the impact of renal stone size and location on stone clearance. METHODS: This was a retrospective case series of pediatric patients (14 years or younger) undergoing lithotripsy using a Dornier MPL 9000 echo-guided lithotriptor. Patients were identified using an international coding and indexing system (9th edition) and ESWL registry. One hundred five patients were treated from January 1990 to December 2001. Stone clearance was assessed a

Percutaneous management of upper-tract transitional cell carcinoma.

Standard management of upper tract transitional cell carcinoma is a nephroureterectomy; however, there is a distinct subgroup of patients who would be rendered dialysis dependent, or at high risk for functionally significant renal insufficiency, and for whom a nephron-sparing approach is warranted. Historically, the options for such "conservative" management have been limited to variations of open pyelotomy or partial nephrectomy. More recently, endourologic techniques have been applied to the treatment of these difficult patients. Though a ureteroscopic approach can occasionally be therapeuti

Urographic changes in homozygous sickle cell disease.

A prospective urographic study of 40 Nigerian homozygous sickle cell patients is presented. The age range was 13-51 years with a mean of 20. The excretory urogram was normal in 30% of the patients. There is a very high incidence of renal papillary necrosis (40%). The other abnormalities observed are non-obstructive caliectasis (47.5%), poor excretion of contrast medium (40%) and renal enlargement. The incidence of urographic abnormality was neither age- or sex-related. There is also no relationship between abnormality and urinary symptoms.

Experience with ureterocalyceal anastomosis.

Experience with 10 cases of ureterocalyceal anastomosis is reported. Most cases involved scleroatrophic scarring of the pelvis after repeated stone surgery, and 1 case each for failed pyeloplasty, tuberculous stricture of the pelvis, transitional cell carcinoma of the pelvis and calyces, and ureteropelvic junction obstruction associated with renal malformation. Three patients had a solitary kidney. End-to-end ureterocalyceal anastomosis was performed in 5 patients; laterolateral in 1 case, and ureteropyelocalyceal anastomosis in the remaining 4. In 3 cases omentoplasty was also performed.

The left ureterocele and stone of calyceal diverticulum in the patient with bilateral incomplete duplex kidneys managed by flexible ureteroscopy: a case report and literature review.

BACKGROUND: Duplex kidneys are one of the most common renal congenital abnormalities, mostly asymptomatic and of no clinical significance. There are little reports about the left ureterocele and stone of calyceal diverticulum in patients with bilateral incomplete duplex kidneys managed by flexible ureteroscopy. CASE PRESENTATION: A 69-year-old Chinese woman was presented with left waist pain for 1 month. A preoperative computed tomography (CT) scan and intravenous pyelogram revealed the left ureterocele which located in the left ureterovesical junction, and stone of calyceal diverticulum whic

Retroperitoneoscopic ureterocalicostomy for congenital proximal ureteral stenosis.

We report the first case of retroperitoneoscopic ureterocalicostomy in a 17-year-old male patient with severe left hydronephrosis caused by a long congenital upper ureteral stenosis. With a retroperitoneoscopic approach, the stenotic segment was resected, the thin renal parenchyma overlying the lower calyx was fenestrated by a modest excision, and the proximal ureter was anastomosed to the lower pole in an end-to-end manner. At 2 years postoperatively, the patient was asymptomatic, with a significant reduction in hydronephrosis and a patent upper ureter. Retroperitoneoscopic ureterocalicostomy

What should the optimal access site be for percutaneous treatment of anterior lower pole calyx stones?

OBJECTIVES: Compared to other stone groups, anterior calyx stones are more challenging for endourologists to treat. This study aims to evaluate the differences between our latest technique and conventional techniques for treating anterior calyx stones in the lower pole. MATERIALS AND METHODS: Patients with isolated anterior lower pole calyx or complex lower pole stones with anterior calyx branching were included in the study. The first group included lower pole access, while the second group included percutaneous treatment through the middle or upper pole. In the first group, the posterior ca

[Urothelial tumors of the kidney].

Urothelial tumors of the kidney arise from the urothelium of the renal pelvis. Most of them are transitional cell carcinomas histologically similar to bladder carcinomas. The new staging system emphasizes the role of kidney parenchyma in the spread of the tumor. Homolateral multifocal lesions are frequent, along with bladder lesions, preceding, simultaneous with or subsequent to upper tract tumor.

Computed tomography of renal tuberculosis and its pathological correlates.

Computed tomographic studies of eight patients with proven renal tuberculosis demonstrated a spectrum of findings related to the extent and the severity of kidney involvement. In early or focal disease, obstruction of a single major calyx or a group of minor calyces was noted (four cases) with no excretion of contrast material into the affected calyces (three cases) and cortical thinning. Tuberculosis of the renal pelvis (five cases) was manifested by either pelvic dilatation secondary to ureteropelvic junction obstruction (three patients) or diffuse pelvic contraction (two patients). In far a
