Avulsion fractures and chronic avulsion injuries of the knee: role of MR imaging.

Avulsion fractures and chronic avulsion injuries of the knee are common lesions in sports-related trauma, especially among adolescents. Magnetic resonance imaging may prove useful in detecting and characterizing such lesions, and has several advantages with regard to other imaging modalities. We review, illustrate, and discuss the MR imaging features of some of the more frequent avulsion fractures and chronic avulsion injuries of the knee, including avulsion fractures of the cruciate ligaments, avulsion fractures of lateral and medial stabilizers, avulsion fractures and chronic avulsion injuri

Arterial injuries in orthopaedics: the posteromedial approach for vascular control about the knee.

Combined orthopaedic and vascular injuries are becoming more common owing to an increasing incidence of high-energy trauma and gunshot wounds. We present our experience using the posteromedial approach in treating eight orthopaedic patients with popliteal arterial injuries. All patients underwent popliteal exploration by a vascular surgeon through a posteromedial approach (releasing the pes anserinus and the medial gastrocnemius). The arterial injuries were reconstructed in five patients using a reverse saphenous vein graft and directly repaired in three patients. Two patients had delayed heal

[Injuries to the knee extensors. Rupture of the patellar tendon and the quadriceps].

Authors discuss, based on the experiences of their 18 cases, the injuries of the knee extensors. 5 ruptures of the quadriceps and 9 of the lig. patellae propr. were observed in the 14 cases controlled. Based on their own experiences and the standpoints of the literature they suggest the operative therapy within 6 hours in the open injury and within 1-5 days following the accident in closed cases. The experiences, the operative technique, the viewponts of the aftercare and the end results are described.

[Epidemiology, treatment and follow-up of acute ligamentous knee injuries in Alpine skiing].

In Switzerland the incidence of sport injuries is twice as high as traffic injuries. Soccer and alpine skiing are the major reasons for such injuries. Nowadays one out of four skiing injuries is an injury of the knee ligaments. Especially in skiing areas favored by beginners, we observed an incidence of knee ligament injuries three times as high compared to other areas. Since 1986 we evaluated 206 patients which were operated for knee ligament injuries. In 44% the direction of instability was unidirectional, most of them in the anterior direction. 115 patients had combined instabilities, 86% o

A review of intra-articular knee injuries in racquet sports diagnosed by arthroscopy.

Because of the recent rapid increase in the number of knee injuries related to racquet sports, the authors undertook a retrospective study of such injuries seen over a 5-year period at the Toronto Western Hospital Sports Medicine Institute. The 121 patients who presented over the study period with a racquet-sports-related knee injury requiring arthroscopy represented 30% of all racquet-sports related injuries seen during that period. The mean age at presentation was 32.8 years and two-thirds of the patients were men. All the major racquet sports were represented. In all, 213 lesions (7 bilater…

Vascular injuries near the knee: an updated series and overview of the problem.

Injury of the popliteal artery results in amputation more often than any other arterial injury. Adjacent injuries, small vessel thrombosis, and muscle necrosis are the major deterrents to limb salvage. This report updates a previously published series, expanded to include 110 injuries treated during a 14-year period. Emphasis is placed on the management of adjacent injuries, errors promoting amputation, and principles of improving limb survival. The hallmark of success is the rapid and complete restoration of arterial and venous flow.

Osseous Injury Associated With Ligamentous Tear of the Knee.

One of the most common knee injuries is ligament tear, which may initially manifest as an osseous injury in radiographs. Radiologists should therefore be able to recognize ligament tears of the knee as osseous abnormalities in images. This review focuses on the imaging features of knee ligament injuries and their related osseous injuries: anterior cruciate ligament (ACL) tear with Segond fracture; associated marrow contusion; ACL avulsion fracture; posterior cruciate ligament (PCL) tear with osseous avulsion of the ligament including arcuate sign; reverse Segond fracture; PCL avulsion fracture…

Accidental burn injury during knee arthroscopy.

A case is described in which hot irrigation fluid used during a routine knee arthroscopy caused severe morbidity. The patient sustained full-thickness skin burns requiring debridement, a muscle flap, and split-skin grafting. The extent of the joint damage required fusion. The underlying factors included equipment failure but also a delay in appreciating the severity of the soft-tissue damage. This case highlights the need for a robust protocol for the management of warming the irrigation fluid, as well as monitoring the actual fluid temperature.

[A mathematical model of determining the knee joint injury].

A mathematical model of determining the injury of human knee joint was established by using structural dynamics method. The results showvered the corresponding shearing displacement and bending angle of the knee joint on the occasion of ligament avulsion, and the shearing force and bending moment which caused tibial condylar fractures under purest shearing and bending condition. The calculated results consisted with the experimental results greatly, suggesting that using mathematical method to determine the injury is feasible and this kind of method can do good to lots of subjects, such as ped

Popliteal artery injury associated with knee dislocations.

Since the Vietnam War experience we have known that there is a high association between knee dislocations and popliteal artery injuries. In an effort to improve the quality of care we asked whether every patient with a knee dislocation needs an arteriogram. This is a retrospective chart review of all injured patients who presented to Louisiana State University Health Science Center with knee dislocations between January 1, 1993 and March 31, 1998. Twenty-one patients met the study criteria. There were no deaths in this series. Twelve patients presented with normal palpable pulses. Nine of thes

Muscle inhibition following knee injury and disease.

It has been observed that knee extensor muscles cannot be fully activated during voluntary contractions following knee injuries. This muscle inhibition has an unknown origin and appears to hinder full rehabilitation of the affected joint. We have investigated muscle inhibition during and following knee injuries in non-athletic and athletic patients and compared their results to non-athletic, unaffected volunteer subjects. There appears to be a small amount of muscle inhibition in the knee extensors of normal subjects; this inhibition increases dramatically following knee injury, and appears to…

How successful is below-knee amputation for injury?

This study analyses how successfully young adults rehabilitate after below-knee amputation for injury. Twenty-five patients referred to Bristol Artificial Limb and Appliance Centre and aged under 45 years at the time of amputation were investigated 2-12 years after operation by a review of their records, a questionnaire and, in a few cases, by interview. Most did extremely well. All without serious disabilities were able to work, most requiring little or no sick leave because of their amputations. Prostheses were worn by 84 per cent of patients for more than 13 h a day and only 32 per cent had

Complete dislocations of the knee with popliteal vascular injury.

Complete dislocation of the knee is rare, but is frequently associated with popliteal artery disruption. Prompt recognition and early revascularization are paramount for a successful and functional result. Arteriography is recommended in all cases of complete dislocation of the knee. Excision of the damaged arterial segment with saphenous vein bypass grafting is the treatment of choice. Associated venous and nerve injuries should be recognized and repaired early. Liberal use of compartment decompression procedures give better long-term results.

Extensor apparatus of the knee: anatomy and injury patterns.

The extensor mechanism of the knee is essential for maintaining normal daily function and for activities such as walking and athletic performance. It comprises the quadriceps muscles and tendon, patella and peripatellar fat pads, patellofemoral joint and retinacula, patellar tendon, and tibial tuberosity. Disruption of any component can result in pain, instability, or loss of active extension. Owing to its superficial location and continuous biomechanical loading, it is particularly vulnerable to acute trauma, repetitive microtrauma, chronic degeneration, and traction apophysitis in the pediat

Early Open Repair of a Multiligamentous Knee Injury.

This review discusses the utility of early open repair of multiligamentous knee injuries in patients with high-energy mechanisms. The surgical technique for anterior cruciate ligament, posterior cruciate ligament, and medial collateral ligament repair was demonstrated on a polytraumatized patient using the medial parapatellar approach. The posterior cruciate ligament was repaired first with suture through a drill tunnel in the femoral insertion followed by the anterior cruciate ligament in a similar fashion. The medial collateral ligament was repaired last with a staple. With some exceptions, …

Cartilage Injury in the Knee: Assessment and Treatment Options.

Cartilage injuries in the knee are common and can occur in isolation or in combination with limb malalignment, meniscus, ligament, and bone deficiencies. Each of these problems must be addressed to achieve a successful outcome for any cartilage restoration procedure. If nonsurgical management fails, surgical treatment is largely based on the size and location of the cartilage defect. Preservation of the patient's native cartilage is preferred if an osteochondral fragment can be salvaged. Chondroplasty and osteochondral autograft transfer are typically used to treat small (<2 cm) cartilage defe
