The multigravid partogram--should it be customised?

Multigravid labour is conducted according to an alert line and an action line derived from primigravid spontaneous labour. Although it is widely recognised that multigravid labour is significantly faster than primigravid labour, the standard of care for both is the 'usual-care' partogram. A series of 1398 partograms of spontaneous multigravid labours resulting in vaginal delivery was audited. Retrospectively, the labours were graphed on a usual-care and on a customised partogram. The customised alert line was built to represent the rate of cervical dilataion of the lower 90 per cent. (after ex

Manual rotation and maternal positioning for managing fetal occiput posterior position during labor: a network meta-analysis.

BACKGROUND: The occiput posterior (OP) foetal position is a global obstetric challenge associated with increased maternal and neonatal complications. Manual rotation and specific maternal positioning (hands-and-knees, lateral position) are potential interventions for managing persistent OP position during labour; however, their comparative effectiveness remains uncertain. This systematic review and network meta-analysis aimed to evaluate the efficacy of these interventions versus standard care. METHODS: A systematic search of multiple databases was conducted from inception to January 3, 2025

Associations between fetal position at delivery and duration of active phase of labor: A historical cohort study.

INTRODUCTION: In clinical experience, occiput posterior (OP) position is associated with longer labor duration than occiput anterior (OA) position, but few studies have investigated the association between labor duration and fetal position. We aimed to compare duration of the active phase of labor in OP deliveries with OA deliveries in a contemporary population using survival methods. Secondary aims were to compare the frequencies of operative interventions, obstetric anal sphincter injuries (OASIS), postpartum hemorrhage, and newborn outcomes in OP with OA deliveries. MATERIAL AND METHODS: W

Pharmacogenetics in obstetric anesthesia.

The 21st century has been billed as the era of "precision/personalized medicine." Genetic investigation of clinical syndromes may guide therapy as well as reveal previously unknown biological or pharmacological pathways that may result in novel drug therapies. Several clinical issues in obstetrics and obstetric anesthesiology have been targets for genetic investigations. These include evaluation of the genetic effects on preterm labor and the progression of labor, spinal anesthesia-induced hypotension and the response to medications used to treat hypotension, and the effect of gene variants on

Delivery in an obstetric birth chair: a randomized controlled trial.

OBJECTIVE: To determine whether nulliparae whose second stage of labour is conducted in an obstetric birth chair have a lower incidence of instrumental delivery than those using a conventional delivery bed. DESIGN: Randomized controlled trial using sealed, opaque envelopes for allocation. SETTING: Delivery ward in a busy teaching hospital. PATIENTS: 1250 nulliparae with a singleton live fetus with cephalic presentation, without epidural anaesthesia, who had achieved full dilatation. INTERVENTION: Intention to conduct second and third stages of labour in either the Birth-EZ chair or the con

The effects of transcutaneous electrical nerve stimulation during the first stage of labor: a randomized controlled trial.

BACKGROUND: Labor pain during childbirth can have devastating effects on the progress of labor, mother, and fetus. Consequently, the management of labor pain is crucial for the well-being of the mother and fetus. Transcutaneous electrical nerve stimulation (TENS) is a non -pharmacological analgesic technique. It uses a low-voltage electrical current to activate descending inhibitory systems in the central nervous system to relieve pain. This study aimed to determine the effects of TENS therapy in the first stage of labor. METHODS: In this single-blind randomized controlled trial, we screened

Obstetric factors in the causation of early periventricular--intraventricular hemorrhage.

On routine fontanelle scanning of neonates weighing less than or equal to 2000 g within the first 24 hours of life, 21% had abnormal findings related to peri/intraventricular hemorrhage. These findings occurred almost exclusively in patients who were in labor, with one exception, and were not observed without labor. Once labor ensued, the performance of cesarean section did not prevent periventricular and intraventricular hemorrhage. Other obstetric factors including fetal heart rate monitoring and umbilical artery pH values were not related to early periventricular and intraventricular hemorr

Induction of labor at second delivery subsequent to a primary cesarean: is stage of labor at previous cesarean a factor?

PURPOSE: Parturients with a history of a cesarean delivery (CD) in the first delivery (P1), undergoing induction of labor (IOL) in the subsequent delivery (P2) are at increased risk for obstetric complications. The primary aim was to study if "the stage of labor" at previous cesarean (elective/latent/first/second) is associated with a successful IOL. The secondary aim was to search for other obstetric characteristics associated with a successful IOL. METHODS: A retrospective longitudinal follow-up study in a large tertiary medical center. All parturients at term who underwent IOL at P2 with a

Anesthetic Management of the Newly Diagnosed or Decompensating Cardio-Obstetric Patient.

Cardiovascular disease (CVD) is a leading cause of maternal mortality. Patients with newly diagnosed or decompensating CVD are at high risk of morbidity and mortality. Pregnancy heart teams (PHTs) allow for multidisciplinary management of the cardio-obstetric patient. Anesthesiologists are key members of the PHT. A cardio-obstetric patient's delivery plan should consider the pathophysiology of their lesion in the context of the hemodynamic changes that occur during pregnancy, labor, delivery, or termination, and the postpartum period. More research is needed to provide evidence-based guidance

Premature rupture of membranes and spontaneous preterm labor: maternal endometritis risks.

Preterm and term deliveries were analyzed at The Johns Hopkins Hospital from January 1, 1976, through December 31, 1977, to determine maternal infection risks associated with premature rupture of the membranes (PROM). Gravidas who delivered preterm infants demonstrated an increased incidence of endometritis with or without PROM. The risk of endometritis attributable solely to PROM was insignificant compared with the risk attributable to preterm delivery. These findings implicate infection in the mechanism of preterm labor, with or without PROM.

Anal sphincter lacerations and upright delivery postures--a risk analysis from a randomized controlled trial.

OBJECTIVE: To evaluate obstetric sphincter lacerations after a kneeling or sitting position at second stage of labor in a multivariate risk analysis model. MATERIALS AND METHODS: Two hundred and seventy-one primiparous women with normal pregnancies and spontaneous labor were randomized, 138 to a kneeling position and 133 to a sitting position. Medical data were retrieved from delivery charts and partograms. Risk factors were tested in a multivariate logistic regression model in a stepwise manner. RESULTS: The trial was completed by 106 subjects in the kneeling group and 112 subjects in the s

Induction Time to Vaginal Delivery: A Comparison of Obstetric Coverage Models.

OBJECTIVE: Induction of labor (IOL) and hospitalist coverage are becoming more common. While hospitalist coverage has been associated with improved maternal outcomes and lower cesarean delivery rates, its impact on IOL remains unclear. The objective of this study was to compare the induction time to vaginal delivery across three obstetric coverage models: hospitalists, faculty generalists, and private practice generalists. STUDY DESIGN: This single-site retrospective cohort study analyzed singleton, term (≥39 weeks), vertex patients undergoing IOL at NYU Langone Hospital-Long Island from Janu

Cocaine abuse is associated with abruptio placentae and decreased birth weight, but not shorter labor.

Many of our patients report having ingested cocaine hoping to decrease the duration of labor. We reviewed the computerized records of 592 women who abused cocaine. Compared with 4687 controls, women who ingested cocaine were older and had higher parity. Birth weight, birth weight percentile, and gestational age at delivery were significantly decreased among their neonates, and the incidence of abruptio placentae was nearly doubled among these women. Although these factors tend to shorten labor, the total duration of labor was not significantly different between the two groups. These data add t

The optimal angle of the mediolateral episiotomy at crowning of the head during labor.

INTRODUCTION AND HYPOTHESIS: The aim of the mediolateral episiotomy incision is to increase the diameter of the soft tissue of the vaginal outlet to facilitate birth and to prevent vaginal tears. Episiotomy angles that are too narrow and close to the midline increase the risk of obstetric anal sphincter injuries. In order to determine the optimal angle of the episiotomy, we assessed the changes in the angles of episiotomy lines marked during the first stage of labor and measured at the time of crowning of the head. METHODS: Incision lines for mediolateral episiotomy were marked on the perinea

Birth weight threshold for postponing preterm birth.

OBJECTIVE: The study was designed to determine the birth weight threshold at which obstetric efforts intended to delay delivery might potentially improve rates of neonatal morbidity and mortality among pregnancies delivered after spontaneous preterm labor or rupture of the membranes. STUDY DESIGN: We studied 1147 singleton infants with birth weights between 1000 and 2499 gm and whose only complication was spontaneous preterm labor or preterm rupture of the membranes. The Mantel-Haenszel chi 2 statistic was used to evaluate trends for neonatal mortality and several indexes of morbidity. RESUL

The use of intrapartum ultrasound to diagnose malpositions and cephalic malpresentations.

Fetal malpositions and cephalic malpresentations are well-recognized causes of failure to progress in labor. They frequently require operative delivery, and are associated with an increased probability of fetal and maternal complications. Traditional obstetrics emphasizes the role of digital examinations, but recent studies demonstrated that this approach is inaccurate and intrapartum ultrasound is far more precise. The objective of this review is to summarize the current body of literature and provide recommendations to identify malpositions and cephalic malpresentations with ultrasound. We p
