Pathological and surgical examination of the handling of the lacerated eyeball.

A survey of 85 lacerated eyeballs pointed to six elements that had led to destruction of the globe: poor wound closure, hypotony, hemorrhage, lens remnants, fibrosis and antigen exposure. Guidelines for the repair of such globes were established on the basis of the pathological findings. The techniques necessitate great care and require the use of a microscope, a vitreous aspiration cutter and fine sutures. Adequate time must be set aside for the surgery, since anything less than careful and complete repair can lead to disaster. The quality of the primary repair is likely to be what defines th

The Role of Topical Anesthetics in the Treatment of Pediatric Facial Lacerations.

PURPOSE: Pediatric facial lacerations (PLACS) are a common emergency, often requiring a general anesthetic (GA) or sedation if infiltration of local anesthetic (LA) cannot be tolerated. Topical anesthetics are a well-established and accepted alternative to infiltrations for facial lacerations which can reduce the need for general anesthesia. We report our experience following the introduction of topical 5% cocaine w/v and 1:2000 adrenaline gel (TCA) in a single center. METHODS: A secondary data analysis of 11 months attendances with PLACS following the introduction of TCA as part of a service…

PARTICIPANTS: Eighteen women from the United States, the United Kingdom, Australia, and Canada who sustained fourth-degree perineal lacerations during childbirth. Evaluation and Management of Sports-Related Lacerations of the Head and Neck.

Soft tissue injuries to the head and neck are a common occurrence in sports. These anatomical regions are somewhat predisposed because of th … al knowledge and skill in regard to wound care, and awareness of potential poor outcomes related to lacerations of unique structures, such as the mouth, eye, or ear.

Is Ritgen's maneuver associated with decreased perineal lacerations and pain at delivery?

Purpose: Different techniques have been studied to prevent the risk of perineal trauma during labor and post-partum pain. Limited informatio … oach. The primary outcome was severe perineal laceration, defined as either third- or fourth-degree lacerations. Meta-analysis was performed using the random-effects model of DerSimonian and Laird to produce summary treatment effects in terms of either a relative risk (RR) with 95% confidence interval (CI).Results: Three trials including 1589 women were analy …

Racial/Ethnic Differences in Perineal Lacerations in a Diverse Urban Healthcare System.

OBJECTIVES: The aim of the study was o determine whether variations exist between races/ethnicities in perineal laceration at first vaginal … ingly. RESULTS: A total of 1179 charts met criteria. When comparing none, first-, or second-degree lacerations versus third or fourth degrees (obstetric anal sphincter injuries) race was not a significant factor. We also compared lacerations among the following three groups: none or first degree versus second degree versus obstetric anal sphincter injuries. …

Sealing of Corneal Lacerations Using Photoactivated Rose Bengal Dye and Amniotic Membrane.

PURPOSE: Watertight closure of perforating corneoscleral lacerations is necessary to prevent epithelial ingrowth, infection, and potential loss of the eye. Complex lacerations can be difficult to treat, and repair with sutures alone is often inadequate. In this study, we evaluated a potentially sutureless technology for sealing complex corneal and scleral lacerations that bonds the amniotic membrane (AM) to the wound using only green light and rose bengal dye. METHODS: The AM was impregnated with rose bengal and then sealed over lacerations using green light to bond the AM to the deepithelial…

Does meperidine analgesia affect the incidence of obstetric lacerations at vaginal delivery?

PURPOSE: To study whether meperidine analgesia affects the incidence of obstetric lacerations at normal vaginal deliveries. MATERIALS AND METHODS: A retrospective cohort study of all women with term vertex singleton pregnancies, who underwent normal vaginal deliveries, in a single tertiary hospital, between 2011 and 2015, was performed. The incidence of various obstetric lacerations was compared between deliveries with meperidine analgesia and deliveries with no analgesia. Deliveries with epidural analgesia and instrumental deliveries were excluded. An intravenous infusion of 75 mg of meperid…

A rare case of steering wheel injury causing coronal-plane pancreatic lacerations.

The incidence of pancreatic injury after blunt abdominal trauma is extremely low. A timely and accurate diagnosis is important, as a delay could be life-threatening. In this case, a 70-year-old driver crashed a car into a concrete wall at low speed. He was transported to the emergency hospital but died about 9.5h later with the cause of death unknown. An autopsy revealed that his pancreas was lacerated in the coronal plane and there was mesenteric contusion. Cause of death was determined to be blood loss resulting from pancreatic and mesenteric contusion. The mechanism of the injury was consid

Pretibial lacerations: experience from a lower limb trauma centre and systematic review.

BACKGROUND: Pretibial lacerations are an important and neglected problem among the elderly. Poor management leads to prolonged hospitalisation and terminal decline. This study summarises our experience and evidence from the literature to ascertain an evidence-based rationale for referral and management. METHODS: Our data were obtained from review of a prospectively gathered database. Additionally, Pubmed, Embase, Medline, and the Cochrane Database of Systematic Reviews were searched through July 2013, with eligible studies evaluated using standard methodology. RESULTS: Seventy-three pretibia…

A modified bicanalicular intubation procedure to repair canalicular lacerations using silicone tubes.

PURPOSE: To explore a modified technique for silicone intubation for the repair of canalicular lacerations. METHODS: The surgery was performed on 35 eyes in 35 adult patients from October 2007 to September 2009. Using a modified soft probe, silicone tubes were inserted through the lacrimal punctum and left in the bicanaliculi for 3-10 months. RESULTS: The surgery was performed successfully in all cases. The tubes were removed after 3-10 months (mean 5.3±1.8 months). The mean follow-up time after tube removal was 13.8 months (range, 6-22 months). Lower punctum splitting occurred in one case (…

Management of uncomplicated nail bed lacerations presenting to a children's emergency department.

OBJECTIVE: This study examined the mechanisms of injury and the pattern of care for children who presented to the emergency department with uncomplicated nail bed lacerations. METHODS: A retrospective chart review was conducted from January 2004 to December 2007 for all children younger than 18 years who presented to a tertiary children's hospital with an uncomplicated nail bed laceration. RESULTS: There were 84 cases of uncomplicated nail bed injuries for more than a 4-year period. Sixty percent of the subjects were males. The mean age was 5.3 (SD, 4.1) years. Most injuries occurred at home…
