The paper reports on 78 cases of fatal choking observed in the Institute of Legal Medicine, University of Duesseldorf, since 1947. The age median of all victims was 53 years. There were 24 females among the victims. Only in one child a solitary occlusion of the glottis by a collar-button was found. In all other cases bolus material was distributed over more than one region, e.g. larynx, glottis and trachea. In 50 victims single pieces of meat, ham, herring etc. were found. In the remaining 28 cases the bolus material was roughly chewed. More than 80 per cent of cases occurred in restaurants an…

The development of appropriate surface coils has resulted in outstanding depiction of the laryngeal anatomy using MRI. The multiplanar capability of MR allows superior definition of anatomy and tumor extent compared with CT scanning. MR appears to be more effective in detecting cartilage invasion. MR is as effective as CT in defining adenopathy and extension into the carotid sheath. MRI is therefore the primary modality in imaging the larynx.

Adenoid cystic carcinoma of the larynx is rare. Only 7 cases have been treated in our hospital from 1959 to 1986, which comprised 0.7% of all laryngeal malignancies in the same period. Of the 7 cases, there were 5 males and 2 females with the average age of 47. The symptoms at the time of diagnosis varied according to the size and location of the tumor. Three lesions were considered as supraglottic and one as subglottic. Six cases were treated by surgery and one by radiotherapy alone. The most common site of distant metastasis was the lung. The average survival was long even in those with pulm…

Superficial extending carcinoma of the hypopharynx is a recently recognized form of carcinoma regarded as a possible pharyngeal counterpart to early gastric cancer and superficial esophageal carcinoma. In this study we report the radiological, clinical, and pathological features of three carcinomas of the larynx, which show architectural and histopathological features similar to those previously described in superficial extending carcinoma of the hypopharynx. These cases were singled out from a series of 37 consecutive laryngectomy specimens of primary infiltrating laryngeal carcinomas that we…

We report the ninth case of plexiform neurofibroma of the larynx, which occurred in a 2-year-old with multiple café au lait spots and obstructive sleep apnea. In discussing this clinical problem, we have attempted to make the following points. A patient's having more than six café au lait spots of greater than 1.5-cm diameter is diagnostic of von Recklinghausen's disease. It is much more difficult to completely excise plexiform neurofibroma than nonplexiform neurofibroma. The association of juvenile xanthogranuloma with von Recklinghausen's disease may be a risk factor for the later developmen…

Recent papers have shown a high degree of disagreement between observers in the assessment of physical signs. In order to determine the interobserver variation in assessment of the larynx, 63 patients were each examined by 2 ENT registrars. The examiners agreed on the assessment of the voice in 80% of cases, and on the indirect laryngoscopy appearance in 76%. These results compare favourably with those reported previously for other forms of clinical examination.

A 74 year old woman presented with a primary subglottic tumour. Neither cutaneous mastocytosis (urticaria pigmentosa) nor spread to the bone marrow, liver, or spleen were detected. About two years after initial manifestation of the tumour nodular skin metastases appeared, as well as local recurrence in the larynx. Despite chemotherapy and radiation the disease progressed and was fatal. The terminal phase was characterised by generalisation of the mast cell tumour with diffuse infiltration of bone marrow and, shortly before death, leukaemic transformation. The patient died four years after onse…

Aneurysmal bone cyst is a rare lesion of the head and neck, and the mandible and maxilla are the most common sites of occurrence. To the authors' knowledge, two cases of aneurysmal bone cyst of the larynx described here are the first to be reported. Although the exact pathogenesis of this lesion is unknown, several theories of its origin are discussed. Complete surgical excision is the treatment of choice. Radiation therapy for this benign lesion is not indicated because of a possible association with the development of sarcoma. Histologic misdiagnosis of carcinoma or sarcoma of the larynx lea…

Fifty-one laryngectomy specimens were histopathologically studied in serial sections. The incidence of thyroid cartilage invasion was 10% in supraglottic, 48% in glottic, and 67% in subglottic carcinomas. The incidence of cricoid cartilage invasion was 0% in supraglottic, 22% in glottic, and 33% in subglottic carcinomas. The sites of cartilage invasion were the anterior commissure portion and an ossified lower portion of the thyroid cartilage, and an ossified upper portion of the cricoid cartilage. Fixation of the vocal fold in glottic carcinoma was accounted for by carcinomatous invasion into

This presentation has dealt with the question of when to perform conservation surgery of the larynx for cancer. The authors included a recommendation for the use of radiotherapy because it is an obvious extension of the issue. It should be clear that this is a complex problem involving relatively few patients compared with the number of radiation therapists and head and neck surgeons throughout the country. Eligibility for conservation surgery of the larynx is dependent upon many factors in addition to the gross extent of the tumor. Since radiation will be one of the choices of therapy, it is …

Coccidioidomycosis, while endemic to the American Southwest, may occur in travelers on return to their homes. Usually asymptomatic in human hosts, approximately 40% of infected subjects will have pulmonary signs and symptoms and 2% will have dissemination from the pulmonary foci. The larynx may be secondarily involved or may be the site of sputum inoculation.

Histology, histogenesis, biology and treatment of the rare spindle cell carcinoma of the larynx are presented. The histogenesis of spindle cell tumours has been much debated, but electron microscopic investigations have shown that most of the laryngeal lesions are squamous epithelial carcinomas that have undergone transformation to a spindle cell pattern. As confusion with supporting tissue neoplasms may occur in the light microscope, the use of the electron microscope in differential diagnostics is recommended.

Opening and closing of the larynx are determined by the intrinsic and extrinsic muscles acting on the elastic forces in the tongue, pharynx, larynx, and trachea. The pharynx is opened or closed by two mechanisms: (1) Contractions of the cricothyroid and of the intrinsic muscles of the larynx open and close the vocal cords. (2) The false cords, ventricle, and true cords accordion open or close in a bellows mechanism. We conclude that the posterior cricoarytenoid opens the laryngeal airway. The cricothyroid together with the posterior cricoarytenoid accentuates this opening. The larynx is also o…

A saccular or congenital cyst of the larynx is a mucus-filled dilatation of the laryngeal saccule and can distort the aryepiglottic fold, the false cord or the laryngeal ventricle. The most common symptom of the laryngeal cyst is respiratory distress with inspiratory stridor and the diagnosis can usually be made by laryngoscopy. Treatment includes incision and drainage together with extirpation of the cranial wall of the cyst through a laryngoscope. Sometimes tracheostomy is also needed. Usually there are no complications afterwards and the development of these infants is perfectly normal. Our…

The muscles of the larynx function as a part of the respiratory system before birth, and like other respiratory muscles, have experienced considerable use by the moment of birth. In late fetal life the larynx appears to influence the outward flow of pulmonary liquid and thus may play a role in lung development. Immediately after birth and in cases of neonatal lung disease, elevated pressures within the airways during expiration, probably a result of laryngeal adduction, are involved in the maintenance of FRC. This mechanism is also present, to a lesser degree, in normal ovine (and probably hum…

Adenoid cystic carcinoma, or cylindromas, constitute a large and distinct group of neoplasms with characteristic microscopic features recognizable in tissues as varied as the nasal passages, tongue, and bronchi. Cylindromas of the larynx are rare and only 60 cases have been reported in a review of the literature in 1977. In this report, a case of adenoid cystic carcinoma originating in the subglottic area of the larynx is discussed, with emphasis on the clinical and pathologic characteristics. In the past, the histogenesis of cylindroma has been the subject of considerable controversy, but rec…
