Protocol for rapid onset of mobilisation in patients with traumatic spinal cord injury (PROMPT-SCI) study: a single-arm proof-of-concept trial of early in-bed leg cycling following acute traumatic spinal cord injury.

INTRODUCTION: Activity-based therapy (ABT) is an important aspect of rehabilitation following traumatic spinal cord injury (SCI). Unfortunately, it has never been adapted to acute care despite compelling preclinical evidence showing that it is safe and effective for promoting neurological recovery when started within days after SCI. This article provides the protocol for a study that will determine the feasibility and explore potential benefits of early ABT in the form of in-bed leg cycling initiated within 48 hours after the end of spinal surgery for SCI. METHODS AND ANALYSIS: PROMPT-SCI (pr

Cross-Leg Free Flap: Crossing the Border Zone of Ischemic Limb-A Case Report of Limb Salvage Procedure following a Delayed Diagnosis of Popliteal Artery Injury.

Vascular injury following traumatic knee injury quoted in the literature ranges from 3.3 to 65%, depending on the magnitude and pattern of the injury. Timely recognition is crucial to ensure the revascularization is done within 6 to 8 hours from the time of injury to avoid significant morbidity, amputation, and medicolegal ramifications. We present a case of an ischemic limb following delayed diagnosis of popliteal artery injury after knee dislocation. Even though we have successfully repaired the popliteal artery, the evolving ischemia over the distal limb poses a reconstruction challenge. Mu

The distally based neurocutaneous sural flap: a good choice for reconstruction of soft tissue defects of lower leg, foot and ankle due to fourth degree burn injury.

OBJECTIVE: Debridment of fourth degree burn wound usually leads to soft tissue loss. If these wound are on distal one-third of lower leg and ankle, a simple wound changes to a complex problem. Options are available for these conditions, but each of them have advantages and disadvantages. The distally based neurocutaneous sural flap is one recent flap available for this problem which has excellent results and decreased disadvantages. METHODS: Between 2000 and 2004, all patients with acute fourth degree burns or unstable scar on the distal third of lower leg, foot or ankle that referred to our

Increased knee valgus alignment and moment during single-leg landing after overhead stroke as a potential risk factor of anterior cruciate ligament injury in badminton.

BACKGROUND: In badminton, knees opposite to the racket-hand side received anterior cruciate ligament (ACL) injuries during single-leg landing after overhead stroke. Most of them occurred in the backhand-side of the rear court. Comparing lower limb biomechanics during single-leg landing after overhead stroke between the forehand-side and backhand-side court may help explain the different injury rates depending on court position. HYPOTHESIS: The knee kinematics and kinetics during single-leg landing after overhead stroke following back-stepping were different between the forehand-side and backh

Non-invasive spinal cord neuromodulation enables volitional anti-gravity leg movements after motor-complete spinal cord injury: responders vs. non-responders.

BACKGROUND AND OBJECTIVES: Transcutaneous spinal cord stimulation (tSCS) is an emerging treatment for motor recovery following spinal cord injury (SCI). However, the extent of motor recovery with tSCS and the reasons why some individuals with motor-complete SCI respond less effectively, despite having the same injury classification, remain unclear. Here, we demonstrate that lumbosacral tSCS can enable anti-gravity voluntary movement following motor-complete SCI, and identify markers that distinguish responders from non-responders. METHODS: Ten individuals with chronic cervical and upper thora

Other clinical uses for apomorphine: Sedation and sleep disorders, withdrawal of oral dopaminergic medication, palliative care, restless legs syndrome, traumatic brain injury, sexual dysfunction.

Apomorphine is now recognised as the oldest antiparkinsonian drug on the market. Though still underused, it is increasingly prescribed for patients with advanced Parkinson's disease (PD) with motor fluctuations in Europe, Asia and more recently on the other three continents. In light of its most recent uses and newest challenges, this paper focuses on a number of indications in Parkinson's disease and beyond, which are currently under development or which would benefit from development, given the generally high levels of evidence: sedation and sleep disorders, withdrawal of oral dopaminergic m

Early Reconstruction of Distal Leg and Foot in Acute High-Voltage Electrical Burn: Does Location of Pedicle in the Zone of Injury Affect the Outcome of Distally Based Sural Flap?

BACKGROUND: Distally based fasciocutaneous sural flap is popular in the reconstruction of distal leg and foot burns. However, utilization of this technique in high-voltage electrical injury has been challenging. AIMS: The present study aimed to compare the outcome of early aggressive debridement and coverage of contact point of acute high-voltage electrical injury using distally based fasciocutaneous sural flap between high-risk and low-risk patients defined by the anatomic proximity of the flap pedicle to the zone of injury. METHODS: A total of 51 patients with contact point of high-voltage

Sagittal plane body kinematics and kinetics during single-leg landing from increasing vertical heights and horizontal distances: implications for risk of non-contact ACL injury.

PURPOSE: This study identified kinematic and knee energetic variables that reduce the risk of non-contact anterior cruciate ligament (ACL) injury during single-leg landings from increasing vertical heights and horizontal distances. METHODS: Nine subjects performed single-leg landings from takeoff platforms with vertical heights of 20, 40, and 60 cm onto a force plate. Subjects also performed single-leg landings from a 40 cm high takeoff platform placed at horizontal distances of 30, 50 and 70 cm from a force plate. Kinematic and kinetic data were measured. RESULTS: Vertical height had a sign

The relationship between performance of a single-leg squat and leap landing task: moving towards a netball-specific anterior cruciate ligament (ACL) injury risk screening method.

Field-based screening methods have a limited capacity to identify high-risk postures during netball-specific landings associated with anterior cruciate ligament (ACL) injuries. This study determined the biomechanical relationship between a single-leg squat and netball-specific leap landing, to examine the utility of including a single-leg squat within netball-specific ACL injury risk screening. Thirty-two female netballers performed single-leg squat and netball-specific leap landing tasks, during which three-dimensional (3D) kinematic/kinetic data were collected. One-dimensional statistical pa

Reliability of 3-Dimensional Measures of Single-Leg Cross Drop Landing Across 3 Different Institutions: Implications for Multicenter Biomechanical and Epidemiological Research on ACL Injury Prevention.

BACKGROUND: Anterior cruciate ligament (ACL) injuries are physically and financially devastating but affect a relatively small percentage of the population. Prospective identification of risk factors for ACL injury necessitates a large sample size; therefore, study of this injury would benefit from a multicenter approach. PURPOSE: To determine the reliability of kinematic and kinetic measures of a single-leg cross drop task across 3 institutions. STUDY DESIGN: Controlled laboratory study. METHODS: Twenty-five female high school volleyball players participated in this study. Three-dimensiona

Elevated gastrocnemius forces compensate for decreased hamstrings forces during the weight-acceptance phase of single-leg jump landing: implications for anterior cruciate ligament injury risk.

Approximately 320,000 anterior cruciate ligament (ACL) injuries in the United States each year are non-contact injuries, with many occurring during a single-leg jump landing. To reduce ACL injury risk, one option is to improve muscle strength and/or the activation of muscles crossing the knee under elevated external loading. This study's purpose was to characterize the relative force production of the muscles supporting the knee during the weight-acceptance (WA) phase of single-leg jump landing and investigate the gastrocnemii forces compared to the hamstrings forces. Can two-dimensional video analysis during single-leg drop vertical jumps help identify non-contact knee injury risk? A one-year prospective study.

BACKGROUND: Previous studies showed that the amount of hip flexion and the combination of knee valgus and lateral trunk motion, measured with two-dimensional video analysis, were related to three-dimensional measured knee joint moments during single-leg drop vertical jumps, but it remains unclear whether these measurements can be used to identify non-contact knee injury risk. METHODS: Fifty injury-free female athletes participated in the study. Two-dimensional video analysis was used to measure hip flexion, knee valgus and lateral trunk motion angles during single-leg drop vertical jumps. Tim

Relation between peak knee flexion angle and knee ankle kinetics in single-leg jump landing from running: a pilot study on male handball players to prevent ACL injury.

OBJECTIVES: The relationship between knee kinematics and knee-ankle kinetics during the landing phase of single leg jumping has been widely studied to identify proper strategies for preventing non-contact ACL injury. However, there is a lack of study on knee-ankle kinetics at peak knee flexion angle during jumping from running. Hence, the purpose of this study is to establish the relationship between peak knee flexion angle, knee extension moment, ankle plantar flexion moment and ground reaction force in handball players in order to protect ACL from excessive stress during single leg jumping.

Effects of early acute in-bed leg cycling initiated from the intensive care unit following severe traumatic spinal cord injury: final results from the PROMPT-SCI trial.

BACKGROUND: Preclinical studies suggest that initiating activity-based therapy (ABT) within days of an acute spinal cord injury (SCI) best enhances recovery, but may also increase the risk of inducing spinal cord damage. This study assesses the effects of early ABT initiated within 48 h of spinal surgery (or 3 days of the SCI), while patients are in the intensive care unit (ICU) for hemodynamic management. METHODS: This single-arm proof-of-concept trial included 45 adults with a severe traumatic SCI receiving daily 30-minute sessions of in-bed leg cycling for 14 consecutive days, in comparison
