Pupillary capture following sutureless scleral-fixated intraocular lens in children with Marfan syndrome.

AIM: To compare the clinical outcomes between two approaches for sutureless scleral-fixated intraocular lens (SFIOL) in children with Marfan syndrome (MFS). METHODS: The study included 15 children (26 eyes) with lens subluxation due to MFS. These children underwent lensectomy, anterior vitrectomy, and sutureless SFIOL. According to the position of placement of intraocular lens (IOL) haptics, two study groups were reviewed for best corrected visual acuity (BCVA) and postoperative complications: group A, 14 eyes with haptics fixated at 2.0 mm from the limbus; group B, 12 eyes with the haptics f…

Ultrasound Evaluation of Lens Dislocation with Iridodonesis.

BACKGROUND: Point-of-care ultrasound can be used in the emergency department to quickly diagnose intraocular lens dislocation, a rare but vision-threatening condition. Traumatic dislocated intraocular lenses may also be associated with iridodonesis, which is rapid vibration of the iris with eye movements due to lack of normal iris support from the lens. CASE REPORT: We present the case of a 69-year-old female with traumatic acute lens dislocation sustained after mechanical ground-level fall at her nursing facility. Additionally, she was found to have iridodonesis, which was recorded with ultr

Pseudoexfoliation Glaucoma: Clinical Presentation and Therapeutic Options.

Pseudoexfoliation syndrome (PES) is one of the most common causes of open-angle glaucoma, with a higher risk of vision loss, a higher maximu … les, and iris. PEM deposits on the zonules may explain the clinically observed zonular weakness and lens subluxation or dislocation. An increased incidence of cataract development is also associated with PES. There is growing evidence for systemic associations of PES with peripheral, cardiovascular, and cerebrovascular system diseases, Alzheimer's disease, hearing …

[Vitrectomy and perfluorocarbons in the management of luxated lenses].

PURPOSE: To evaluate the use of vitrectomy with perfluorocarbon liquids (Perfluoro-DK-line) (PFCL) to refloat the luxated lenses of surgical etiology. Follow-up complications are considered. METHODS: The cases with vitreous posteriorly luxated lenses were reviewed between 1989 and 1999. 8 cases were found, all had similar surgery with vitrectomy and PFCL performed by the same surgeon. In two cases an anterior chamber IOL and in 6 cases a posterior chamber IOL sulcus fixated were used. RESULTS: The average age of the 8 cases with unilateral vitreous luxated lenses of no surgical etiology was

Bilateral posterior lens dislocation in Marfan's syndrome.

Complete dislocation of the lens is a rare feature of Marfan's Syndrome. We observed four cases of bilateral complete posterior dislocation of the lens in established cases of Marfan's Syndrome over a one year period. An ophthalmologist may encounter infrequently, unilateral posterior dislocation in one eye and ectopia lentis in the other, but bilateral spontaneous posterior dislocation is rare. All our cases had associated vitreous degeneration with vitreous herniating into the anterior chamber. The pathogenesis of complete posterior dislocation in Marfan's Syndrome is discussed.

[Vascular and ocular complications in a child with homocystinuria].

Homocystinuria, only partially responsive to pyridoxine, was first diagnosed at the age of 14 years in a boy with high myopia, spherophakia and subluxated lenses, when surgical removal of a dislocated lens became necessary. 2 years previously a fracture of the left tibia, treated conservatively, had been followed by ischemia of the leg necessitating amputation. Postoperatively, deep vein thrombosis had developed in the right leg. Plasma homocysteine was lowered to undetectable levels by treatment with betaine in addition to pyridoxine, folic acid and reduction of protein intake. During the two

Late spontaneous posterior capsule rupture with single-piece hydrophobic acrylic intraocular lens dislocation.

In this study, we described and discussed the late onset spontaneous posterior capsule rupture with intraocular lens (IOL) dislocation years after uncomplicated cataract surgery and implantation of hydrophobic acrylic IOLs. Eight patients presented with spontaneous posterior capsule rupture and IOL dislocation 5-20 years after uncomplicated phacoemulsification and IOL (AcrySof, Alcon, US) implantation. None of the patients had undergone posterior capsulotomy in the past. Four of the patients admitted habitual eye rubbing. An intact and well-centered continuous curvilinear capsulotomy edge was

Repositioned versus exchanged flanged intraocular lens fixation for intraocular lens dislocation.

This study aimed to compare the outcomes of flanged intraocular lens (IOL) fixation with new IOL exchange after dislocated IOL removal and repositioned dislocated IOL in patients with IOL dislocation. Eighty-nine eyes that underwent flanged IOL fixation were retrospectively included, with 51 eyes in the exchanged IOL group and 38 eyes in the repositioned IOL group. In both groups, best-corrected visual acuity (BCVA) improved at 1, 3, 6, and 12 months postoperatively and did not differ between the two groups at any of these time points. However, at 1 week postoperatively, BCVA in the reposition

Spontaneous Dislocation of the Lens Nucleus Into the Anterior Chamber Observed in a Patient With Overmature Senile Cataract.

Overmature cataract refers to the advanced stage of cataract where timely surgical intervention is not performed, leading to further progression characterized by decreased water content in the lens, degradation of fibers, and liquefaction within its structure, which can cause a reduction in volume, wrinkling of the capsule, as well as calcification or cholesterol crystallization on its surface. In addition, it may result in deepening of the anterior chamber. If left promptly untreated, these complications may result in visual impairment or even blindness. The occurrence of spontaneous complete

Traumatic lenticele.

A 55-year-old woman presented with sudden diminution of vision in her left eye following trauma with her own finger. Examination revealed a firm subconjunctival swelling superiorly with aphakia and hypotony. Lens spike was absent on B-scan. Ultrasonic biomicroscopy showed thickened conjunctiva, a subconjunctival mass lesion with an underlying uveoscleral discontinuity. A surgical repair was performed with successful restoration of globe integrity. This case demonstrates that ultrasonic biomicroscopy may reveal the site and the extent of occult scleral rupture and help in precise surgical plann

Intra-scleral fixation of the iris hooks for trans-scleral capsular bag fixation in patient with zonular dialysis.

PURPOSE: To report a technique of intra-scleral fixation of the iris hooks for trans-scleral capsular bag fixation in patient with zonular dialysis. METHODS: Two scleral flaps, two scleral tunnels and two sclerotomies were formed. The anterior capsule was captured and pulled by the iris hooks resulting in the IOL adopting an appropriate position at which point the iris hooks extracted from the sclerotomies. The tips of the iris hooks were implanted into the scleral tunnels. Each of them was fixed with a 10.0 nylon suture to the scleral bed. The scleral flaps and conjunctiva were sutured. One

[Ocular refraction in Marfan's syndrome and its surgical problems].

The superior-temporal subluxation of the crystalline lens at the patients with Marfan syndrome makes optic problems who need a surgical solution. It describes one aspiration technique with concomitant support of crystalline lens, underlining the specifics features what are meet here: exessive hardness of crystalloid and the content of crystalline's sack. At eight operated eyes, the postoperatorial visual acuity restored at values over 5/10.
