Nocturnal frontal lobe epilepsy: new pathophysiological interpretations.

Since the first descriptions of Nocturnal Frontal Lobe Epilepsy (NFLE) many theories have been proposed to explain its pathophysiological mechanisms. The aim of this paper is to formulate a tentative hypothesis designed to unify the clinical, anatomo-physiological, and genetic aspects underlying this condition. According to this hypothesis, NFLE is due to a disorder in the thalamocortical circuit involved in the arousal mechanism. Other cortical-networks involving the limbic system may explain, for instance, primitive behaviors. The role of the cholinergic system and related pathways in the pa

[Tourette syndrome].

Tourette Syndrome is not as rare as many health professionals may think. On the contrary it is very common, especially among the Afrikaans population. It is possibly the most common inherited condition. Tourette Syndrome is a neurological variation characterized by "tics" or mannerisms, which can also be combined with one or all of the following: frontal lobe involvement particularly the limbic system; learning problems; anxiety; obsessive-compulsive behaviour; sexual deviations. Most authorities regard TS as a dominant inherited condition although Comings describes it as semi-dominant--semi-r

Klüver-Bucy syndrome and amyotrophic lateral sclerosis: a case report with biochemistry, morphometrics, and Golgi study.

We studied a patient with amyotrophic lateral sclerosis and the Klüver-Bucy syndrome. At autopsy there was extensive degeneration of the limbic system with the brunt of the changes in the medial temporal lobe, especially the entorhinal cortex and subiculum. Degenerative changes were also seen in the substantia nigra and lower motor neurons. Morphometric and biochemical studies implied a disease process that affected small, possibly somatostatinergic, cortical neurons. These latter findings and the lobar distribution of cortical atrophy were consistent with Pick's disease, but Pick bodies and b

MR imaging of autopsy-proved paraneoplastic limbic encephalitis in non-Hodgkin lymphoma.

We report the case of a 26-year-old man with precursor T-cell acute lymphoblastic leukemia who developed paraneoplastic limbic encephalitis that was diagnosed on the basis of MR imaging findings and was proved post mortem. In our MR imaging studies, fluid-attenuated inversion recovery images and diffusion-weighted echo-planar images clearly depicted bilateral involvement of the medial temporal lobes and multifocal involvement of the brain, whereas T2-weighted turbo spin-echo images failed to show the changes.

Brain FDG PET Findings Mimicking Limbic-predominant Age-related TDP-43 Encephalopathy.

Limbic-predominant age-related TDP-43 encephalopathy (LATE) is a recently recognized neurodegenerative disorder that frequently affects elderly patients and is often misdiagnosed as Alzheimer's disease. It typically presents with mild memory impairment and disproportionate hippocampal atrophy. FDG PET commonly shows distinct medial temporal lobe hypometabolism, which differs from Alzheimer's disease. We report a case with FDG PET findings suggestive of LATE; however, CT and MRI revealed hippocampal sulcus remnant cysts instead of hippocampal atrophy. This highlights the critical importance of

Functional connectivity of hippocampal networks in temporal lobe epilepsy.

OBJECTIVE: Temporal lobe epilepsy (TLE) affects brain areas beyond the temporal lobes due to connections of the hippocampi and other temporal lobe structures. Using functional connectivity magnetic resonance imaging (MRI), we determined the changes of hippocampal networks in TLE to assess for a more complete distribution of abnormality. METHODS: Regions of interest (ROIs) were defined in the right and left hippocampi in three groups of participants: left TLE (n = 13), right TLE (n = 11), and healthy controls (n = 16). Brain regions functionally connected to these ROIs were identified by corre

Frontotemporal dementia: a clinicopathological review of four postmortem studies.

Frontotemporal dementia is a progressive dementing illness characterized clinically by personality change, disinhibition, and apathy. Neuropathologically, neuronal cell loss, astrogliosis, and microvacuolation are present in the superficial frontotemporal cortical layers, with variable involvement of subcortical and limbic structures. The clinical picture and anatomical distribution of the degenerative changes, as well as motor neuron involvement, differentiate four neuropathological groups: 1) frontal lobe type, 2) thalamostriatal type, 3) motor neuron type, and 4) asymmetrical type. The auth

Exploring the Neural Pathways of Faith: A Review and Case Study on Hyperreligiosity in Epilepsy.

Religious experiences represent a universal and timeless phenomenon that has accompanied humanity since its origins. In recent decades, neuroscience has explored the relationship between temporal lobe epilepsy (TLE) and hyperreligiosity phenomena, describing sudden convictions, states of ecstasy, and spiritual conversions associated with epileptic seizures. This article offers a narrative review of the literature on the relationship between epilepsy and religion, including its clinical manifestations (ictal, postictal, and interictal) and the main neurobiological models proposed to explain it,

Stress-induced seizure susceptibility ("Psychoepileptogenesis"): From psychological trauma to epileptogenic network reorganization.

Stress exposure, whether acute or chronic, is now recognized to be a determinant of epileptogenic vulnerability. Psychological stress or trauma may not only precipitate seizures but also actively contribute to the development of epilepsy, a concept that in the clinical setting could be termed "psychoepileptogenesis". Recent evidence from both animal models and clinical studies supports the role of emotional stress in facilitating epileptogenesis, particularly within limbic structures such as the amygdala and hippocampus. In rodent models, chronic stress has been shown to lower seizure threshol

Diffusion tensor imaging in borderline personality disorder showing prefrontal white matter alterations.

Borderline personality disorder (BPD) has repeatedly been linked to alterations in fronto-limbic dysfunction. In this study, we tested the hypothesis of disturbed structural connectivity in underlying fibre tracts and their relation to symptom profiles. We analysed diffusion tensor imaging (DTI) data from 18 female BPD patients and 38 female healthy controls. Group comparisons showed significant (p < .05, FDR adjusted) increase of radial diffusivity (RD) in the right frontal lobe, including the uncinate fasciculus, anterior thalamic radiation, and inferior fronto-occipital fasciculus, as well

Psychiatric symptoms in anti glutamic acid decarboxylase associated limbic encephalitis in adults: a systematic review.

Autoimmune Limbic Encephalitis (LE) is a relatively new category of immune-mediated diseases with a wide range of neuropsychiatric symptoms. LE associated with Glutamic Acid Decarboxylase (GAD) antibodies is difficult to diagnose due to its possible atypical presentation with neuropsychiatric and behavioral features. We performed a systematic review of literature and retrieved 21 cases of anti GAD-associated LE with neuropsychiatric signs. Median age at onset was 27 years with a female predominance (81.0 %) and median diagnostic delay of 6 months. Clinical presentation included typical LE symp

Convergence of spatially dispersed atrophy into functional networks in temporal lobe epilepsy: glial-immune signature associated with circuit dysfunction and treatment response.

Temporal lobe epilepsy (TLE) exhibits spatially heterogeneous cortical atrophy, yet how these distributed patterns drive unified pathophysiology remains unknown. Here, we test the hypothesis that locally discrete atrophic regions coalesce into functionally coherent networks associated with unified molecular mechanisms. Using large-scale normative modeling of patient-specific regional atrophy and functional connectivity mapping, we demonstrate that marked variability in atrophic loci coalesce into remarkable functional convergence within temporal-limbic circuitry in TLE patients. This convergen

Persistent Klüver-Bucy syndrome after bilateral thalamic infarction.

OBJECTIVE: To describe a patient who exhibited a partial Klüver-Bucy syndrome after small bilateral ischemic lesions in the thalami. BACKGROUND: Previously reported patients with Klüver-Bucy syndrome had very large, mostly bilateral lesions in the limbic system and could not provide sufficient information about its anatomo-functional correlate. METHOD: Behavioral assessments and clinical examinations, including magnetic resonance imaging and positron emission tomography, were conducted. RESULTS: The patient was severely amnestic, distractible, hyperoral, and affectively dyscontrolled, and s

Transcranial magnetic stimulation (TMS) of the brain in patients with mesiotemporal epileptic foci.

Transcranial magnetic stimulation (TMS) of the human brain is mainly used for the diagnosis of diseases with disturbed central motor conduction. Recent studies revealed controversial results concerning the possibility of a TMS-induced specific activation of epileptogenic foci in patients with localization-related epilepsies, which would make TMS an additional diagnostic tool for the presurgical localization of the primary epileptogenic zone. We applied TMS to 19 patients with complex-partial seizures and investigated its effects and safety. In 12 patients we performed TMS during scalp electroe

Neural correlate of the projection of mental states on the not-structured visual stimuli.

Projection is a spontaneous and complex mental activity responsible for the subjective meaning attribution. The hypotheses of this study were that the neural correlate of projection may involve frontal, parietal, and temporal brain areas, and that alexithymia may be negatively associated with intensities in limbic and paralimbic areas during projection. EEG data were recorded continuously at 250 Hz using NetStation 4.5.1 with 256-channels HydroCel Geodesic Sensor Net in 20 healthy subjects during the presentation of structured and not-structured visual stimuli. The tasks were paying attention
