Histological evaluation of symptomatic ossification of the anterior longitudinal ligament treated with etidronate disodium: a case report.

BACKGROUND: Here we report the first autopsied case involving pathological examination after two resections of symptomatic ossification of the anterior longitudinal ligament with anterior osteophytes and etidronate treatment with more than 8 years of follow-up. CASE PRESENTATION: A 51-year-old Japanese man complained of severe dysphagia due to esophageal compression by ossification of his anterior longitudinal ligament with anterior cervical osteophytes. Although surgical removal of the anterior cervical osteophytes was performed following etidronate treatment (800 mg/day for 6 months), dysph

Benefit of degenerative posterior longitudinal ligament removal during anterior decompression in cervical spondylotic myelopathy.

It remains controversial whether degenerative posterior longitudinal ligaments should be removed during anterior decompression procedures for cervical spondylotic myelopathy. Few data are available from studies that have compared removing and retaining the degenerative posterior longitudinal ligament. The goal of this retrospective study was to evaluate the benefit of degenerative posterior longitudinal ligament removal during such operations. Clinical data on 130 patients with confirmed degenerative posterior longitudinal ligament who underwent anterior cervical decompression surgery were ret…

A genome-wide association study identifies susceptibility loci for ossification of the posterior longitudinal ligament of the spine.

Ossification of the posterior longitudinal ligament of the spine (OPLL) is a common spinal disorder among the elderly that causes myelopathy and radiculopathy. To identify genetic factors for OPLL, we performed a genome-wide association study (GWAS) in ∼8,000 individuals followed by a replication study using an additional ∼7,000 individuals. We identified six susceptibility loci for OPLL: 20p12.3 (rs2423294: P = 1.10 × 10(-13)), 8q23.1 (rs374810: P = 1.88 × 10(-13)), 12p11.22 (rs1979679: P = 4.34 × 10(-12)), 12p12.2 (rs11045000: P = 2.95 × 10(-11)), 8q23.3 (rs13279799: P = 1.28 × 10(-10)) and

Combined ossification of the posterior longitudinal ligament at C2-3 and invagination of the posterior axis resulting in myelopathy.

PURPOSE: Spinal stenosis at the C2-3 segment is a rare occurrence, and when it occurs myelopathy infrequently results. Furthermore, only a handful of cases involving congenital abnormalities of the posterior arch of the axis have been described resulting in cervical myelopathy many of which described simultaneous congenital abnormalities at adjacent levels and none of which identified ossification of the posterior longitudinal ligament (OPLL) at the same level. We report a case of a previously undescribed combination of abnormalities at the C2-3 segment resulting in clinical myelopathy. METHO

Surgical technique: Hemilaminectomy and unilateral lateral mass fixation for cervical ossification of the posterior longitudinal ligament.

BACKGROUND: Surgical approaches for cervical ossification of the posterior longitudinal ligament (OPLL) include anterior, posterior, or combined decompression with or without fusion. The goal of surgery is to decompress the spinal cord while maintaining the stability and sagittal alignment of the cervical spine. C5 palsy has been reported as a postoperative complication of cervical laminectomy or laminoplasty characterized as motor weakness of the muscles supplied with C5 nerve roots. Several studies have shown this phenomenon was partially attributable to posterior shift of spinal cord. DESC

Ossification of the posterior longitudinal ligament in non-Asians: demographic, clinical, and radiographic findings in 43 patients.

OBJECT: Ossification of the posterior longitudinal ligament (OPLL) is a disorder afflicting as many as 2% of East Asians. However, reports of OPLL in non-Asians have been sporadic in the medical literature. This study describes clinical and radiographic findings with OPLL in non-Asians at a tertiary care center treating a diverse multiethnic population. METHODS: Over a 6-year period, 43 patients not of East Asian descent presented to an urban tertiary medical center with OPLL. Patient data, including ethnicity, spinal cord function, Nurick grade, radiographic findings, OPLL subtype, and degre

Radiographic predictors for the development of myelopathy in patients with ossification of the posterior longitudinal ligament: a multicenter cohort study.

STUDY DESIGN: A multicenter cohort study was performed retrospectively. OBJECTIVE: To identify radiographic predictors for the development of myelopathy in patients with ossification of the posterior longitudinal ligaments (OPLL). SUMMARY OF BACKGROUND DATA: The pathomechanism of myelopathy in the OPLL remains unknown. Some patients with large OPLL have not exhibited myelopathy for a long periods of time. Predicting the course of future neurologic deterioration in asyptomatic patients with OPLL is difficult at their initial visit. METHODS: A total of 156 OPLL patients from 16 spine institut…

Proprioception after rehabilitation and reconstruction in knees with deficiency of the anterior cruciate ligament: a prospective, longitudinal study.

We assessed proprioception in the knee using the angle reproduction test in 20 healthy volunteers, ten patients with acute anterior instability and 20 patients with chronic anterior instability after reconstruction of the anterior cruciate ligament (ACL). In addition, the Lysholm-knee score, ligament laxity and patient satisfaction were determined. Acute trauma causes extensive damage to proprioception which is not restored by rehabilitation alone. Three months after operation, there remained a slight decrease in proprioception compared with the preoperative recordings, but six months after re

[Can knee joint proprioception by reconstruction of the anterior cruciate ligament be restored? A prospective longitudinal study].

Knee proprioception was assessed in 20 healthy volunteers, 10 patients with acute anterior instability and in 20 patients with chronic anterior instability preoperatively as well as after 3 and 6 months postoperatively. To determine proprioception, an angle reproduction test was performed. There were no differences between the right and the left knee joint or between men and women. Best proprioception was measured near full extension. The acute trauma causes massive damage of the proprioception, which could be improved significantly by rehabilitation. However, rehabilitation could not restore

Topography and distribution of nerve fibers in the posterior longitudinal ligament of the rat: an immunocytochemical and electron-microscopical study.

The distribution and immunocytochemical characterization of nerve fibers and their terminals in the posterior longitudinal ligament of the rat lumbar vertebral column was studied in whole-mount preparations and serial semithin and ultrathin sections. Differences in the localization, distribution pattern and density of peptidergic and catecholaminergic nerve fibers were found in the vertebral and intervertebral regions of the posterior longitudinal ligament. For immunocytochemistry, free floating specimens were incubated with primary antibodies against protein gene product 9.5, substance P, cal

The effects of intervertebral fusion in patients with myelopathy due to ossification of the posterior longitudinal ligament of the cervical spine.

Fifty patients with ossification of the posterior longitudinal ligament of the cervical spine were treated by anterior intervertebral body fusion and compared with 30 other patients treated conservatively. We found that patients with narrowing of the cervical canal to 0.5 of the normal diameter were improved by simple intervertebral fusion without excision of the ossified ligament. Patients with more extensive ossification and narrowing of the spinal canal to less than 0.4 of the normal diameter might have either severe symptoms or unexpectedly minor symptoms and the latter were attributed to

Posterior circumferential decompression for multilevel thoracic ossification of the posterior longitudinal ligament: a case report and literature review.

Multilevel thoracic ossification of the posterior longitudinal ligament (T-OPLL), especially when it involves more than five consecutive vertebrae, remains surgically challenging. In recent years, a strategy focused on "spinal cord de-tension" has been proposed for such extensive involvements. This report describes a 52-year-old woman who presented with progressive thoracic myelopathy due to continuous OPLL from T3 to T10, causing severe canal stenosis. The surgical approach included a posterior laminectomy from T4 to T10, circumferential decompression at the most compressed level (T8-9), and
