Magnetic resonance imaging of the mediastinum.

Magnetic resonance (MR) imaging is becoming a primary modality for evaluating the mediastinum. MR affords multiplanar imaging capabilities without exposing patients to ionizing radiation. The inherent contrast effect of different mediastinal tissues sharply delineates anatomic structures on MR images without contrast enhancement. Gradient-echo and phase-mapping techniques permit noninvasive qualitative and quantitative assessment of mediastinal blood flow. High soft tissue contrast and flow analysis capabilities make MR imaging a valuable modality for evaluating mediastinal vascular disorders.

Magnetic resonance in multiple sclerosis.

Magnetic resonance and various chemical measurements, such as spectroscopy, are being systematically applied to both multiple sclerosis and the experimental model (experimental allergic encephalitis). The dynamic aspects of multiple sclerosis pathology can be visualized on serial magnetic resonance imaging scans, and enhancement techniques help distinguish another type of activity by showing a leaky blood-brain barrier in acute lesions. Quantitative measures applied to the image also appear to provide an index of the burden of disease both in individual patients and in groups. A combination of

Magnetic resonance imaging of meningiomas.

MRI with gadolinium enhancement is the imaging modality of choice in the diagnosis of meningiomas. Various MRI features are described. MRI signal intensity characteristics in meningiomas contain information that could be correlated with the histopathology of the tumor. Hyperintensity on T2-weighted images indicates soft tumor consistency and microhypervascularity and is more often seen in aggressive, angioblastic, or meningothelial meningiomas. MRI is useful in demonstrating dural sinus involvement. Meningiomas in unusual locations and their mimics are also presented.

Thoracic magnetic resonance imaging.

Magnetic resonance imaging has played a role in the diagnosis of thoracic disease; however, the role has been limited compared with that of CT. In the past year, there were a few additional reports of MR imaging in the diagnosis of thoracic disease, which include a multi-institutional study comparing MR imaging with CT and histopathologic correlative studies evaluating gadopentetate dimeglumine-enhanced MR imaging. Furthermore, several excellent reviews of MR imaging of thoracic disease were published. In general, the recent literature supports information previously reported. In the future, d

Magnetic resonance imaging of osteomyelitis.

Early diagnosis and detection of osteomyelitis and differentiation of soft-tissue infection from bone involvement is a difficult clinical and imaging problem. Magnetic resonance imaging has proven to be as sensitive as bone scintigraphy in the early detection of osteomyelitis, and, with its superior spatial resolution, MR is often more specific than planar scintigraphy in differentiating bone from soft-tissue infection and separating arthritis, cellulitis, and soft-tissue abscess from osteomyelitis. In several comparative studies, MR has been more advantageous in detecting the presence and det

[Magnetic resonance in trigeminal neuralgia].

One of the possible causes of the so-called "essential" trigeminal neuralgia is a neurovascular compression of the fifth cranial nerve root at the pons. The demonstration of this hypothesis could orientate the surgical treatment to microvascular decompression. In order to evaluate the role of MRI in the diagnosis of trigeminal neuralgia due to neurovascular compression, the authors present the results of a prospective evaluation of the cranial MR studies of 18 neuralgic patients in comparison with a retrospective evaluation of the cranial MR studies of 50 healthy control subjects. The results

Rare earth magnets and impaction.

Aberration in the eruption process was found to be a prime etiologic factor in inducing impaction. Thus an ideal treatment approach should attempt to mimic the normal eruption modus. However, conventional traction methods have been found to be associated with gingiva inflammation, bone recession, reduced attached gingiva, periodontal pockets, exposed cementoenamel junction, and root resorption of the impacted and adjacent teeth. These side effects are the result of premature exposure of the impacted tooth to the oral cavity through a nonself-cleansing pathway and an uncontrolled force system.

Thoracic magnetic resonance imaging.

In our experience, MR has served largely as a problem-solving device, especially in those cases in which CT has proved equivocal. Magnetic resonance has been especially efficacious in evaluating cardiovascular pathology. Virtually the entire spectrum of aortic disease can be assessed accurately, making MR a reasonable alternative to CT or angiography in most cases. Indications for the use of MR in patients with thoracic neoplasia have also emerged. Magnetic resonance is more accurate than CT in assessing invasion of the chest wall and mediastinum. As a consequence, MR should be considered the

Magnetic resonance imaging of the wrist.

Until recently magnetic resonance imaging of the musculoskeletal system tended to be confined to the larger joints, such as the knee and shoulder. Advances in magnetic resonance technology, especially in the area of improved software and development of surface coils, have made feasible the imaging of smaller joints, such as the wrist. This article reviews the potential applications of magnetic resonance in evaluating disorders of the wrist.

Magnetic resonance imaging of the larynx.

The development of appropriate surface coils has resulted in outstanding depiction of the laryngeal anatomy using MRI. The multiplanar capability of MR allows superior definition of anatomy and tumor extent compared with CT scanning. MR appears to be more effective in detecting cartilage invasion. MR is as effective as CT in defining adenopathy and extension into the carotid sheath. MRI is therefore the primary modality in imaging the larynx.

Magnetic resonance velocity mapping.

Magnetic resonance velocity mapping is achieved by encoding velocity at each point in a tomographic imaging plane in the phase of the magnetic resonance signal. Although this can be achieved with almost any imaging sequence, cine gradient echo sequences are particularly suitable because of the high signal from blood and the ability to repeat the sequence rapidly to form a cine image. The technique has been shown to be accurate by in vitro and in vivo validation, with flow measurements in the great vessels having an accuracy of approximately 6%. A potential problem arises from loss of signal fr

Magnetic resonance in obstructive jaundice.

Twelve cases of obstructive jaundice in whom ultrasound failed to demonstrate the site and/or the cause of obstruction of the biliary tract were examined with magnetic resonance imaging (MRI), correctly diagnosing the site and cause of obstruction in 10 of 12 surgically proven cases. In one case of cholangiocarcinoma, the site of obstruction was well shown on MR but a definite cause could not be ascertained. In another patient who developed intermittent jaundice following surgery for choledochal cyst, MR demonstrated a solitary stone in the common hepatic duct. Surgical confirmation could not
