Bypassing mandibular osseous defect using the high oblique sagittal split osteotomy: a case report and review of literature.

The presence of a mandibular osseous defect significantly complicates revision orthognathic surgery by increasing the risk of unfavorable fractures and inferior alveolar nerve (IAN) injury. In such instances, the high oblique sagittal split osteotomy (HOSSO) serves as a strategic alternative to the conventional bilateral sagittal split osteotomy. We report a case involving a 31-year-old Chinese female presenting with skeletal relapse twelve years after the primary bimaxillary surgery. Preoperative imaging identified a 1.5×1.5 cm circular osseous defect in the right mandibular body, communicati

Mandibular Angle Osteotomy via Intraoral Approach: A 15-Year Experience from a Single Plastic Surgery Center in China.

BACKGROUND: Mandibular angle osteotomy (MAO) using an intraoral approach is a common procedure for correcting square facial contours in the Asian population. While previous retrospective studies have primarily focused on complications rates, limited research has examined time-related trends and the epidemiological characteristics of the surgical population. A comprehensive study with a large sample size and long-term follow-up is crucial to enhance outcomes, minimize complications, provide evidence-based insights, and consolidate clinical experience. METHODS: This retrospective study reviewed

Swallowing Functions after Sagittal Split Ramus Osteotomy with Loose Fixation for Mandibular Prognathism: A Retrospective Case Series Research.

Sagittal split ramus osteotomy (SSRO) is a standard surgical technique for patients with mandibular prognathism. However, the appropriate position of the proximal fragment is not strictly defined, and rigid fixation can induce early postoperative skeletal relapse and temporomandibular (TMJ) disorders. Loose fixation can be expected to seat the proximal bone fragments in a physiologically appropriate position, thereby reducing adverse events. Although long-term skeletal stability has been achieved using SSRO without fixation, the evaluation of preoperative and postoperative eating and swallowin

Maintaining the space between the mandibular ramus segments during bilateral sagittal split osteotomy does not influence the stability.

BACKGROUND/PURPOSE: Three-dimensional computer-assisted orthognathic surgery allows to simulate the space between the mandibular ramus segments, i.e. intersegmental gap, for the correction of facial asymmetry. The purposes of the study were to estimate the screws- and mandible bone-related changes from the early postoperative period to the period after the debonding and to measure the association between the intersegmental gap volume and the screws- and mandible bone-related changes. METHODS: This cone-beam computed tomography (CBCT)-assisted retrospective study assessed the stability of the

Stability of fixation methods in large mandibular advancements after sagittal split ramus osteotomy: an in vitro biomechanical study.

Sagittal split ramus osteotomy (SSRO) with large mandibular advancements is a common surgical procedure and could be indicated for patients with sleep apnoea. As a large variety of fixation methods is used for the stabilisation of SSRO, a biomechanical test model was used to analyse which fixation technique was most stable. For this in vitro study, 80 polyurethane hemimandibles with a prefabricated SSRO were used as substrates. Loads in Newtons were recorded at displacements of the mandibular incisive edge at 1mm, 3mm and 5mm. The samples were divided into two groups: mandibular advancements o

Comparable aesthetic outcomes of a hybrid modified intraoral vertico-sagittal ramus osteotomy technique and conventional bilateral sagittal split ramus osteotomy for asymmetric mandibular prognathism in a youthful population.

Conventional bilateral sagittal split ramus osteotomy (BSSRO) for asymmetric mandibular prognathism carries risks to the temporomandibular joints and inferior alveolar nerve. A hybrid technique, combining a modified L-shaped intraoral vertico-sagittal ramus osteotomy (mIVSRO) with a contralateral SSRO, may mitigate these risks, but its aesthetic outcomes are unproven. The aim of this study was to compare the aesthetic efficacy of the hybrid versus conventional BSSRO techniques, adjusting for baseline asymmetry. This retrospective cohort study compared 30 patients with asymmetric mandibular pro

Mandibular ramus remodeling after vertical osteotomy: a three-dimensional quantitative and qualitative assessment with sequential segmental-marrow superimposition.

BACKGROUND: The intraoral vertical ramus osteotomy (IVRO) is a surgical technique for prognathic or asymmetric mandible. We wanted to quantitatively and qualitatively analyze the three-dimensional (3D) remodeling pattern of the mandibular ramus one year after IVRO. METHODS: We reconstructed 3D mandibular models using 3D CT data from 108 IVRO hemi-mandibles of 54 subjects. The immediate postoperative (Imm) proximal and distal segments were separated and individually superimposed to one-year postoperative (Post) mandible by employing a direct segmental-isolated marrow superimposition. RESULTS:

Does a mandibular access osteotomy improve survival in pT2 oral tongue cancers? Retrospective study at a single institution.

The surgical approach to the resection of oral tongue cancers can involve transoral resection (TOR) or a temporary mandibulotomy access (TMA). There are no relevant guidelines, and the oncological safety of TOR needs consideration. The objective of this study was to investigate TMA and TOR in pT2 oral tongue cancer surgery with regard to cancer outcomes. Demographic, surgical, and histology data from primary pT2 tongue cancers were recorded and evaluated through multivariate Cox regression for local recurrence (LR), disease-free survival (DFS), and overall survival (OS). A total of 166 patient

Surgical and social implications of extraction of mandibular third molars as a separate procedure before bilateral sagittal split osteotomy.

In some units, lower wisdom teeth are extracted in a separate procedure before bilateral sagittal split osteotomy (BSSO), whereas in others they are removed at the time of the osteotomy. We identified 57 patients who had BSSO at Salisbury Hospital between October 2013 and September 2015, 40 of whom had their wisdom teeth extracted at the same time. The remaining 17 did not have wisdom teeth. Patients who have these teeth extracted as a separate procedure require at least one day off work, which can result in a loss of earnings. Our findings showed that the removal of third molars at the time o
