BACKGROUND: Patients with fibromyalgia (FM) frequently resort to osteopathic or chiropractic treatment, despite very weak supporting evidence. We aimed to assess the efficacy of osteopathic manipulation in FM in a properly controlled and powered randomized clinical trial.
METHODS: Patients were randomized to osteopathic or sham treatment. Treatment was administered by experienced physical medicine physicians, and consisted of six sessions per patient, over 6 weeks. Treatment credibility and expectancy were repeatedly evaluated. Patients completed standardized questionnaires at baseline, durin

INTRODUCTION: Parasympathetic nervous system (PSNS) function can be inferred by heart rate variability (HRV) providing indications about an individual's health. Manual therapy may influence PSNS function, however the research outcomes in this regard are equivocal. This study explored the PSNS effect of a measured breathing technique with suboccipital balanced ligamentous tension, an osteopathic manipulative therapy technique.
METHODS: Healthy adult participants in this crossover study (n = 18) were randomly allocated into two groups with differing order of interventions. A 1:1 breathing rate

BACKGROUND: Individuals who experience migraines often seek out a variety of treatment options including manual or physical therapy. Evidence suggests that manual therapy, including osteopathy, can play a role in the management of migraines. Whilst there is some literature on the role osteopathy therapy plays in migraine management, none describes the treatment approaches used by practitioners.
OBJECTIVES: To explore the demographic, practice and clinical management characteristics of Australian osteopaths who report treating migraine 'often' in clinical practice.
METHODS: Secondary analysis

OBJECTIVE: To establish the effects of manual therapy, chiropractic, or osteopathic treatment of the KISS-syndrome (kinetic imbalance due to suboccipital strain) in infants with positional preference, plagiocephaly, and colic.
DESIGN: Systematic review of the literature.
METHOD: PubMed, Embase and the Cochrane Library were searched for articles on the effects of manual therapy, chiropractic and osteopathy on the KISS-syndrome. Experts in the field of manual medicine and osteopathy were asked to provide relevant articles. The bibliography in a textbook of manual therapy for children was hand-

BACKGROUND: Chronic low back pain (CLBP) is a frequent cause of disability and it represents a medical, social and economic burden globally. Therefore, we assessed effectiveness of osteopathic interventions in the management of NS-CLBP for pain and functional status.
METHODS: A systematic review and meta-analysis were conducted. Findings were reported following the PRISMA statement. Six databases were searched for RCTs. Studies were independently assessed using a standardized form. Each article was assessed using the Cochrane risk of bias (RoB) tool. Effect size (ES) were calculated at post-t

BACKGROUND: Diffuse idiopathic skeletal hyperostosis (DISH) syndrome and lumbar epidural lipomatosis are relatively asymptomatic neurological conditions, with findings often seen incidentally on radiological studies.
OBJECTIVE: The aim of this paper is to present unique findings of concomitant, symptomatic DISH syndrome and lumbar epidural lipomatosis and to discuss the osteopathic diagnosis and treatment implications.
METHODS: Concomitant, symptomatic variants are rare and present challenges to treatment and management, as seen with a 60-year-old African American woman who presented with wo

The estimated prevalence of gastritis in the general US population is approximately 50%. Patients with gastrointestinal disease often present to the primary care practitioner with dyspepsia and abdominal pain. Osteopathic palpatory evaluation suggests that there is an association among gastrointestinal disease, the presence of posterior midthoracic pain, and chronic headache. On the basis of findings from a review of the literature, the author assesses the potential etiologic mechanisms of this clinical association. Possible mechanisms include the physiologic function of the vagus nerve, a neu

BACKGROUND: Meralgia Paresthetica (MP) is a neuropathy of the lateral femoral cutaneous nerve (LFCN). Limited evidence exists to guide therapy selection. Current guidelines recommend conservative treatment as the initial approach. However, this was not implemented in the case reports presented.
OBJECTIVES: These two clinical case reports aim to describe the application of osteopathic techniques (OT) in the management of Meralgia Paresthetica.
CLINICAL FEATURES: The first case involved a 61-year-old woman with a three-week history of symptoms. The second case involved a 48-year-old man with s

Amyotrophic lateral sclerosis (ALS) is an incurable rare neurodegenerative condition, with 45% of cases showing the symptom of dysphagia; its clinical signs are atrophy, weakness, and fasciculations of the facial muscles, tongue, and pharynx. Furthermore, dysphagia is the main cause of aspiration pneumonia. The traditional treatment for dysphagia varies based on the patient's difficulty of swallowing. The initial phase consists of dietary consistency adjustments, progressing to alternatives like nasogastric tubes or percutaneous endoscopic gastrostomy (PEG) in advanced stages. Osteopathic mani

CONTEXT: Osteopathic medical care has been associated with better long-term outcomes in an intention-to-treat analysis of data from a retrospective cohort study involving chronic low back pain (CLBP). The beneficial outcomes attributable to osteopathic medical care were mediated by physician empathy and osteopathic manipulative treatment (OMT) in the subset of patients who consistently utilized osteopathic physicians compared with those who utilized allopathic physicians.
OBJECTIVES: This study utilized data from the previously reported study to conduct a per-protocol analysis to determine th

INTRODUCTION: Adolescent idiopathic scoliosis (AIS) is a deformity that affects the spine in three dimensions. Even though AIS patients are usually asymptomatic, AIS negatively impacts them, affecting their quality of life and restricting their social life. There are many treatment options but no gold standard. Visceral manipulation (VM) is widely used in osteopathic practice for the treatment of several conditions, but it is little known among the medical community. To the best of our knowledge, there are no scientific studies reporting VM as a treatment alternative for AIS.

OBJECTIVE: While positive impacts of osteopathic manipulative treatment (OMT) on the human visual system have long been proposed, even in individuals without visual or oculomotor symptoms, scientific evidence supporting this hypothesis remains insufficient. This randomized, three-arm parallel clinical trial aimed to compare the effects of OMT vs. sham OMT and test-retest improvements on ocular pursuit.
METHODS: 145 participants aged 18-35 years were randomly assigned to one of three groups: an experimental OMT group (n = 53), a sham OMT group (n = 51), or a no-treatment control group that rec

The muscle energy technique for a superiorly subluxed rib, introduced by Fred Mitchell Sr., DO, and later adapted by Philip Greenman, DO, is an effective treatment for a superior rib one somatic dysfunction. Although muscle energy techniques involve various reflexes, Mitchell and Greenman emphasized the postisometric relaxation variation. However, aligning this technique with general postisometric relaxation principles requires certain assumptions to be made about involved nerve reflexes. By analyzing similarly paired muscles to the anterior and middle scalenes as well as the neurologic mechan

BACKGROUND: Ehlers-Danlos syndromes (EDS) are inherited connective tissue disorders, requiring specific care along with a multidisciplinary approach by an expert medical staff, as for both the diagnosis and the treatment management. Chronic generalized pain and musculoskeletal dysfunctions due to joint hypermobility are common traits. Temporomandibular disorders (TMDs) are among the most frequent complaints. The authors report the diagnostic and therapeutic management of an EDS subject.
CASE PRESENTATION: A 33-year old woman with Ehlers-Danlos syndrome, hypermobile type, was clinically evalu-
