Evaluation of Surgical Options for Supernumerary Teeth in the Anterior Maxilla.

AIM: The purpose of this study was to evaluate the surgical options for supernumerary teeth (SNT) in the premaxillary region of children. MATERIALS AND METHODS: A total of 69 patients with 82 partially or completely formed SNT in the anterior maxillary region were identified over a 3-year period. All selected patients were assessed for the number, location, and family history of SNT, damage to adjacent roots, and associated symptoms. The surgical approaches used for removal were recorded. Postoperative clinical sequelae (loss of vitality, periodontal problems, pain, bleeding, and enanthema) w…

Sarcomatoid Carcinoma of the Maxilla: A Case Report with Literature Review.

Sarcomatoid carcinoma is a very rare malignant and aggressive tumor that can involve the maxillary sinus. We report the case of a 46 y/o male who presented a tumor of the right maxillary sinus with extension to the right nasal cavity. The diagnosis of sarcomatoid carcinoma was set by histology and immunohistochemistry. The patient received concomitant cisplatin based chemoradiotherapy with no response after 3 months of treatment. He died 6 months later. Through the present case and the review of literature we discuss all aspects of this entity: clinical presentation, differential diagnoses, pa…

[Treatment of a unilateral Brodie's syndrome by surgical contraction of the maxillae].

INTRODUCTION: Brodie's syndrome consists in a transverse occlusal discrepancy in relation with an excessive width of the maxilla, a narrow mandible or a combination of both, leading to lateral scissors bite. This kind of infrequent malocclusion is usually treated by orthodontics alone in children. In adults, additional orthognathic surgery id often required. We present a case of unilateral Brodie's syndrome treated by surgical contraction of the maxillae. OBSERVATION: A 22-year-old patient presented with a left scissors bite and severe unilateral molar overbite. After failure of an attempt to…

Novel Transoral Approach to the Posterolateral Maxilla and Infratemporal Region.

Surgical access to tumors involving the posterolateral maxilla and infratemporal region remains a considerable challenge for surgeons. Various surgical approaches for treating posterior maxillary lesions require transcutaneous incisions, such as an upper or lower cheek flap, but they often result in substantial morbidity. This article describes a novel transoral approach involving a curvilinear incision, temporalis myotomy, and coronoidectomy to allow direct visualization of the posterolateral maxilla and infratemporal region. This surgical technique may be used for the surgical resection of m…

Rehabilitation of an Edentulous Maxilla in a Patient with Isolated Cleft Palate.

This article aims to discuss the use of short dental implants in cleft patients, the construction of an implant-retained obturator and the use of a Createch milled titanium bar with three Locator overdenture attachments incorporated within the bar. CPD/Clinical Relevance: Implant-retained obturators offer a possible solution for treating patients with isolated cleft palate who are struggling with dentures due to the unfavourable soft and hard tissue profile. This paper demonstrates how to manage such patients and shows all the clinical and laboratory stages involved.

Multidisciplinary approach for management of a patient with fibrous dysplasia of maxilla.

Fibrous dysplasia is a non-neoplastic hamartomatous developmental fibro-osseous lesion of bone. Monostotic fibrous dysplasia is more common than the polyostotic form and usually involves jaw bones, ribs and femur. Maxillary lesions may extend to involve the maxillary sinus, zygoma, sphenoid bone and floor of the orbit and require surgical intervention resulting in an acquired defect of the involved site. A multidisciplinary team approach involving an oral pathologist, oral surgeon, oral medicine expert and maxillofacial prosthodontist is required for successful treatment and rehabilitation of …

Maxillectomy for Cementifying Osseous Dysplasia of the Maxilla: A Case Report.

OBJECTIVES: Osseous dysplasias are the most common subtype of fibro-osseous lesions of the maxillofacial bones. They are benign and often present as incidental asymptomatic lesions. Diagnosis can be made with clinical and radiographic examination. CASE REPORT: This article reports the case of a 47-year-old man with a huge cementifying osseous dysplasia of the maxilla that presented with secondary infection after teeth extraction and repair of oroantral fistula. A subtotal maxillectomy had to be done after conservative treatment failed to resolve the infection. DISCUSSION/CONCLUSION: Biopsy, …

Cherubism With Bilateral Mandible and Maxilla Involvement: A Case Report.

Cherubism is a rare, nonneoplastic, self-limiting fibro-osseous that occurs in children. Affected children usually appear normal at birth. Lesions are characterized by the replacement of bone with fibrovascular tissue containing many multinucleated giant cells. Most studies have reported cherubism to be familial and with bilateral involvement of the mandibles. The authors describe a nonfamilial case of cherubism, involving both the mandible and the maxilla, in a 4-year-old female child with slowly enlarging, painless, symmetrical swelling of both cheeks.Cherubism is a rare disease that is usua…

[Anesthesia in a neonate with capillary hemangioma of the maxilla: a case report].

A 21-day-old female neonate weighing 3.2 kg was scheduled for surgical excision of a maxillary tumor under general anesthesia. The lesion was present since birth, with gradual enlargement since then. Preoperatively, the lesion measured 25 mm in diameter, although it was not known whether it was benign or malignant. The oral surgeon anticipated that the surgery would require 30 minutes for completion and would involve minimal bleeding. Hence, blood products were not prepared preoperatively. A good intravenous access was secured before induction of general anesthesia, which was achieved with IV …

A complex odontoma of the anterior maxilla associated with an erupting canine.

Many developmental anomalies of the jaw are accidentally diagnosed during radiographic investigations for other symptoms. Complex odontomas are odontogenic tumours, relatively rare in the anterior maxilla and generally asymptomatic. Frequently the odontoma may interfere with the eruption of teeth. This paper describes a case of complex odontoma in an 8-year-old girl, which resulted in the palatal displacement of the lateral incisor and an erupting permanent canine. A calcified mass was seen on the radiograph and was provisionally diagnosed as an odontoma which was surgically enucleated. Routin…

Dual endoscopic approach of intradiploic epidermoid cyst of the maxilla.

Intradiploic epidermoid cysts are relatively rare tumors that arise between the 2 tables of cranial bones. We report a very rare case of intradiploic epidermoid cyst of the maxilla, which was managed through dual endoscopic approach. We recommend the dual endoscopic approach, maxillary anterior wall trephination, and middle meatal antrostomy for removal of intradiploic epidermoid cyst of the maxilla. The endoscopic approach through maxillary anterior wall trephination and middle meatal antrostomy allows for good visualization, removal of the intradiploic epidermoid cyst, prompt spontaneous dra…

Malignant lymphoma in maxilla with cystic involvement: a case report.

INTRODUCTION: Most radiolucent jaw lesions are benign and resolve with proper diagnosis and treatment. However, a small percentage of radiolucent jaw lesions are malignant and more difficult to manage. METHODS: A biopsy of the radiolucent jaw lesion associated with swelling after nonsurgical root canal therapy was submitted. RESULTS: The lesion had the combined diagnosis of diffuse large B-cell lymphoma with cystic involvement. The patient has since undergone radiation and chemotherapy treatments. Recall appointments indicate healing of the lesion, and no other areas of lymphoma have been fo

Unusual presentation of peripheral primitive neuroectodermal tumor of the maxilla.

The peripheral primitive neuroectodermal tumor (pPNET) is a rare and highly malignant soft tissue neoplasm in children and young adults. Fewer than 10 reported cases of pPNET of maxilla are available in the English literature. A 28-year-old woman was presented with the pPNET of the maxilla and metastasis. Two years after diagnosis, she experienced diplopia, and then magnetic resonance imaging was done, which showed a mass in the optic chiasma and parasellar region. The typical appearance resembled large noncalcified soft tissue masses in the magnetic resonance image and computed tomographic sc…

Multiple supernumerary teeth and odontoma in the maxilla: A case report.

Most supernumerary impacted teeth are located in the anterior maxillary region. They are classified according to their form and location. Their presence may give rise to a variety of clinical problems. The detection of supernumerary teeth is best achieved by thorough clinical and radiographic examination. Their management should form part of a comprehensive treatment plan. This article presents an overview of the diagnostic problems associated with multiple supernumerary impacted teeth and includes a discussion of the classification, diagnosis, and management of this difficult clinical entity.

Reconstruction of the maxilla with prefabricated scapular flaps in noma patients.

BACKGROUND: Noma (cancrum oris) is a devastating gangrenous disease that leads to severe tissue destruction in the face and is associated with a high rate of mortality. When untreated, it results in disfiguring midface defects and severe scarring. Ideal reconstruction of complex maxillofacial defects requires the restitution of bone, dental implants, soft tissue, and the thin and durable lining of the oral and paranasal cavities. Prefabricated composite grafts from the scapula are used to restore the maxilla in patients with this disease. METHODS: A new concept of osteosynthesis involving tit…

[Aneurysmal bone cyst of the maxilla. Report of one case].

Aneurysmal bone cysts are benign fibro osseous lesions, rarely present in maxillary region. We report the CT and morphological findings of aneurysmal bone cyst of the maxilla in a 15 years old female. The patient was subjected to a maxillectomy, preserving the orbit floor. The postoperative evolution has been uneventful. The tumor mainly involved the maxillary sinus with extension to the orbit wall and osseous palate. Very few cases of aneurysmal bone cysts of the maxilla have been reported in the literature.
