Prevention of postoperative deformation of the nose after bone repair surgery (transposition of the maxillary complex forward) is an important problem. The proposed method for osteotomy of the maxillary complex prevents or minimizes the postoperative deformation of the nasal cartilage after transposition of the maxillary complex. The method was used with good results in 35 patients with upper microretrognathia.

Although the osteotome is positioned in close vicinity to the maxillary artery and its branches during ptergomaxillary separation in a Le Fort I osteotomy, postoperative complications from vascular injuries are rare. The following report describes an unusual occurrence of a maxillary artery pseudoaneurysm following a Le Fort I and bilateral sagittal-split osteotomies for correction of mandibular and maxillary asymmetries in a patient with Goldenhar syndrome. This was recognized 8 months after the procedure when the patient developed acute facial swelling and required an emergent angiogram for

Endodontic microsurgery (EMS) techniques have increased success rates over traditional approaches. Despite surgical advances, anatomically challenging scenarios can preclude EMS in certain cases. The aim of this article was to introduce targeted EMS, which uses 3-dimensional-printed surgical guides (3DSGs) and trephine burs to achieve single-step osteotomy, root-end resection, and biopsy in complex cases. In each of 3 cases, a 3DSG with a trephine port was printed using computer-aided design/computer-aided manufacturing implant planning software. The osteotomy site, angulation, and depth of pr

PURPOSE: Many facial bone contouring surgeries, including reduction malarplasty, are performed in Asian countries to give the face an oval shape. When performing reduction malarplasty, a few large vessels can be injured, including the deep facial vein. The aim of this study was to determine the anatomy of the deep facial vein and to investigate the best method to prevent injury of deep facial vein during reduction malarplasty.
PATIENTS AND METHODS: A retrospective chart review was performed for all patients who underwent reduction malarplasty with L-shaped osteotomy between March 2009 and Feb

Amelogenesis imperfecta (AI) is an enamel defect and is often associated with the anterior open bite (AOB) and transverse maxillary deficiency. It is known that in such cases when AI and AOB appeared together, posterior maxillary impaction with or without bilateral mandibular ramus osteotomies is a frequently preferred treatment option. Virtual planning is more reliable rather than the conventional model surgery planning, especially for complicated cases. Usage area of virtual 3D anatomical models reconstructed from Cone Beam Computed Tomography (CBCT) data is expanding day by day for both dia

INTRODUCTION: Patients with Craniofacial Microsomia (CFM) mandibles Types I/IIa benefit from combined LeFort 1 osteotomy and Mandibular Distraction Osteogenesis (LeFort + MDO); Type IIb from LeFort + MDO or Bimaxillary osteotomy (BiMax); and Type III from BiMax (with 50% of cases having preceding mandibular procedures, including patient-fitted prosthesis); as seen in Part 1. This leads to the question how maxillary and mandibular hypoplasia are correlated and influence the types of maxillary correction.
MATERIAL AND METHODS: A retrospective chart study was conducted including patients diagnos…

In LeFort I surgery, the separation of the pterygomaxillary junction is done by osteotomy. Major intraoperative or postoperative bleeding associated with Le Fort I osteotomies can be venous and/or arterial in nature. Arterial hemorrhage generally involves the maxillary artery and its terminal branches. Arterial hemorrhage tends to be more persistent and can be recurrent, which makes it more difficult to manage. Postoperative bleeding following Le Fort I osteotomies generally presents as epistaxis and usually occurs initially within the first 2 weeks following surgery. Treatment modalities that have been used to successfully arrest postoperative hemorrhage include anterior and/or pos

PURPOSE: In orthognathic surgery for skeletal mandibular prognathism, maxillary clockwise rotation with posterior impaction and mandibular setback is often performed. Change in airway volume after orthognathic surgery is one of the concerns. The present study was performed to evaluate the effects of maxillary clockwise rotation with posterior impaction and the influence of different mandibular techniques, including sagittal splitting ramus osteotomy (SSRO) and intraoral vertical ramus osteotomy (IVRO), on upper airway volume changes before and after bimaxillary surgery for mandibular prognathi

Sinus floor elevation is the most commonly used method for vertical bone augmentation in the maxillary posterior area. This clinical report describes a modified transalveolar approach to elevate the Schneiderian membrane when placing implants on a severely resorbed maxillary posterior ridge with a buccal-palatal width of more than 8.0 mm. In this approach, the osteotomy prepared on the crestal is bilaterally enlarged to 8.0-10.0 mm. The enlarged osteotomies provide better access, allowing instruments to be in direct contact with the bone of the sinus floor and thus elevate the Schneiderian mem

The problem of medical rehabilitation of patients with asymmetrical deformation of the facial skull, complicated by cross occlusion, is discussed. The authors suggest osteotomy of the maxillary complex with translocation of the osteotomied fragment in the sagittal, transverse, and vertical planes with osteotomy to a distance needed for elimination of deep cross occlusion. This method in combination with orthodontic treatment repairs the anatomic and functional abnormalities and brings about good esthetic results.

Removal of a maxillary third molar is a common dental procedure, and oral surgeons rarely experience accidental displacement of a maxillary impacted third molar into the maxillary sinus as a complication of this procedure. In the case in which such displacement occurs, the molar is removed via a transantral (Caldwell-Luc) approach through bone removal or window osteotomy of the anterior wall of the maxillary sinus. However, this approach is highly invasive because 2 surgical fields are required at the socket and the anterior wall of the maxillary sinus. Here, we report as a minimally invasive
