Management of a patient with autism following ingestion of a foreign body.

Patients with autism often find admission to hospital an anxious time. Awareness of the condition, a speedy diagnosis and flexibility in adapting treatment plans will facilitate early discharge and return to their familiar environment. We describe a patient with severe autism who presented with an acute abdomen secondary to ingestion of a foreign body which required laparotomy. Communication directly to medical staff was greatly limited due to severe autism and close liaison with family members was essential in interpreting scant clinical signs. At the time of surgery a rubber bottle teat was

Issues regarding insurance and other third-party reimbursement for behavioral management procedures.

Issues related to reimbursement for time and effort expended in providing behavior management services for children often result in confusion and frustration for the parent and practitioner. Multiple medical and dental insurance plans within a state or region frequently lack a common set of definitions that are fundamental in interpreting and applying contract language. Ambiguities and inconsistencies in state and federal programs and regulatory structures, as well as budget concerns, only exacerbate the problem. This paper reviews issues of third-party payment associated with dental care for

Prevalence of abnormal cases in an image bank affects the learning of radiograph interpretation.

OBJECTIVES: Using a large image bank, we systematically examined how the use of different ratios of abnormal to normal cases affects trainee learning. METHODS: This was a prospective, double-blind, randomised, three-arm education trial conducted in six academic training programmes for emergency medicine and paediatric residents in post-licensure years 2-5. We developed a paediatric ankle trauma radiograph case bank. From this bank, we constructed three different 50-case training sets, which varied in their proportions of abnormal cases (30%, 50%, 70%). Levels of difficulty and diagnoses were

How can a methadone and an opiate-positive immunoassay result be reconciled in a patient prescribed only OxyContin and Wellbutrin?

Appropriate management of patients in pain clinics can be complex and sometimes confusing because providers must correctly interpret multiple sources of patient information. The correct interpretation of laboratory results is essential to guide subsequent patient treatment and management; however, laboratory and clinical pictures can appear to be conflicting in cases of substance abuse. Incorrect interpretation of laboratory results can multiply negative impacts on clinical outcomes and may lead to patient harm or death. This report introduces the complex nature involved in understanding and i

A comparison of therapeutic oxygen versus medical air for the treatment of central sleep apnea in infants with Prader-Willi syndrome; A proof-of-concept randomized crossover trial.

BACKGROUND: Central sleep apnea (CSA) is common in infants with Prader-Willi syndrome (PWS). Although supplemental oxygen is frequently used to manage CSA, its mechanism of action remains unclear; comparison with medical air may help determine whether therapeutic benefit is mediated by inspired oxygen or by flow-related effects on respiratory control. STUDY DESIGN AND METHODS: In this randomized crossover trial, growth hormone-naïve infants <2 years with genetically confirmed PWS and CSA (CAHI >5 events/hr) underwent an overnight polysomnogram receiving medical air and supplemental oxygen in

The Dual-Identity Dilemma: A Comparative Study of Mentorship Structural Barriers in Military and Civilian Medical Education.

INTRODUCTION: Mentorship functions as a stabilizing relational framework in high-demand medical education environments. This international collaborative study between the Defence Services Medical Academy (DSMA) and Avalon University School of Medicine (AUSOM) examines the "Dual-Identity Dilemma"-the structural tension between professional medical identity and institutional military role expectations. While mentorship is central to professional development, hierarchical military structures may shape its psychosocial accessibility. METHODS: An explanatory sequential mixed-methods design (QUAN→Q

Defining Core Competencies for Generalists in Musculoskeletal Oncology: A Latin-American Consensus for Medical Education.

Inappropriate or delayed initial management of musculoskeletal neoplasms can lead to severe consequences, emphasizing the need to define core competencies for frontline practitioners. Such competencies can guide medical education and residency training objectives. This study aimed to identify core clinical competencies required by general practitioners and general orthopedic surgeons without oncologic training for the initial management of musculoskeletal tumors, with a focus on implications for medical education. A two-round modified Delphi method engaged 225 members of the Latin-American Tum

Language barriers in Canadian pharmacies: A review of interpretation services and health equity gaps in Newfoundland and Labrador.

Despite the Canada Health Act's emphasis on "reasonable access" to care, many newcomer patients for whom English is a second language face communication barriers when seeking medical treatment in Canada. This barrier is especially prevalent in Canadian pharmacies, where interpretation services are often limited or non-existent. With the expanding scope of pharmacy practice across Canadian provinces, accurate translation of clinical concerns and medication instructions is crucial to ensure patients' health and safety.Interpretation services in medical settings are inconsistently implemented nat

Affects of ableism in medical education: happiness, resignation, and the disabled killjoy.

Exclusion is not only an experience but a feeling. Yet, studies of disabled medical learners have paid little attention to their emotions. Affect theory provides a lens for studying how emotions are produced by socio-political ways of being and knowing. In this study, we investigated affects circulating through disabled student narratives to explore how these students perceived disability and agency within an ableist system. We conducted a reflexive thematic analysis of 674 open-text responses by disabled medical students from the 2019 and 2020 AAMC Year Two Questionnaire about their experienc

The Influence of Mistrust, Communication, and Advocacy on Black Women's Pregnancy-Related Healthcare Decisions.

Despite advancements in medical care, continued health inequities and a history of medical malpractice have perpetuated medical mistrust among Black communities. This is of particular concern in obstetrics, wherein Black women experience major disparities in outcomes in comparison to their White counterparts. To explore the relationship between mistrust and obstetric care, we conducted semi-structured interviews about obstetric-related healthcare experiences of pregnant and postpartum women who identified as Black or African-American and were currently pregnant or recently postpartum. Video Remote Sign Language Interpreting and Health Communication for Deaf Patients: A Randomized Clinical Trial.

IMPORTANCE: Current interpretation services for Deaf patients who use sign language are often ineffective or unacceptable. In-person interpretation is frequently unavailable, and while video remote interpreting (VRI) remains underused, its scalability may be a solution given interpreter shortages and cost barriers. Existing research focuses on user and interpreter preferences, leaving a critical gap in understanding how interpretation formats affect communication quality. OBJECTIVE: To evaluate the effectiveness of VRI in improving communication outcomes between Deaf patients and physicians c

Sign language ideologies in deaf education in Trinidad and Tobago.

This paper, representing a multilingual Global South-Global North deaf research collaboration, reports and critically analyzes findings from interviews with 5 deaf adult and 5 sign language interpreter participants regarding their educational and training experiences in the postcolonial Caribbean environment of Trinidad and Tobago. Caribbean sign languages are frequently said to be varieties of American Sign Language (ASL) and/or to have been displaced by ASL in schools with deaf learners. In the anglophone Caribbean context, deaf children have not had access to early childhood care and educat

Citizen interpreters in crisis response: Social capital, ethical trade-offs, and hybrid quality control in emergency language services-A comparative analysis of volunteer-led practices in COVID-19 pandemic and climate disasters.

This study investigates the dual role of citizen interpreters in addressing emergency language gaps during crises, combining social capital theory and crisis ethics. Through comparative case studies of coronavirus disease 2019 responses in Montreal's multilingual communities and Hurricane Ida relief efforts within Louisiana's Haitian-Cajun networks, this research identifies three core tensions: the paradox of relational proximity, trade-offs between immediacy and accuracy in terminology translation, and challenges in scaling informal volunteer networks. The study introduces a hybrid quality co

Measuring concurrency using a joint multistate and point process model for retrospective sexual history data.

Understanding the impact of concurrency, defined as overlapping sexual partnerships, on the spread of HIV within various communities has been complicated by difficulties in measuring concurrency. Retrospective sexual history data consisting of first and last dates of sexual intercourse for each previous and ongoing partnership is often obtained through use of cross-sectional surveys. Previous attempts to empirically estimate the magnitude and extent of concurrency among these surveyed populations have inadequately accounted for the dependence between partnerships and used only a snapshot of th

Incorporating retrospective clustering into a prospective Cusum methodology for anthrax: evaluating the effects of disease expectation.

We analysed livestock anthrax in Kazakhstan from 1960-2006, using a prospective CUSUM to examine the affects of expectation on the detection of spatio-temporal clusters. Three methods for deriving baselines were used for CUSUM; a standard z-score, AVG, a spatially-weighted z-score derived from Local Moran's I, LISA, and a moving-window average, MWA. LISA and AVG elicited alarm signals in the second year that did not return below threshold during the 47-year period, while MWA signaled an alarm at year four and relented at year fifteen. The number of spatial clusters elicited varied: LISA n=16,

Observer agreement in assessing flexion-extension X-rays of the cervical spine, with and without the use of quantitative measurements of intervertebral motion.

BACKGROUND CONTEXT: Flexion-extension X-rays are commonly used to identify abnormalities in intervertebral motion, despite little evidence for the reliability of the information that clinicians derive from these test. PURPOSE: Quantify observer agreement on intervertebral motion abnormalities assessed with and without the use of computer-assisted technology. STUDY DESIGN: Assess interobserver agreement among clinicians when they evaluate cervical flexion-extension X-rays using the methods they now use in clinical practice, and compare this to observer agreement when the same clinicians reass
