Statewide assessment of surgical outcomes and the acute care surgery model.

BACKGROUND: The acute care surgery (ACS) model has been widely implemented with single institution studies demonstrating improved outcomes. Recent multicenter studies have raised questions about the economics and efficacy of ACS. This study compares traditional and ACS outcomes across an entire state. METHODS: A retrospective review of Virginia's Health Information administrative database was completed. Adults admitted with appendicitis or cholecystitis between 2008 and 2014 were included. Hospital administration was contacted to determine surgical model. To compare patient characteristics, t

Immunization costs and programmatic barriers at an urban HIV clinic.

BACKGROUND: Although the increasing number of recommended immunizations is essential for patients infected with human immunodeficiency virus (HIV), the potentially uncompensated costs of expanded immunizations will present significant challenges for clinics and health systems serving HIV-infected patients. METHODS: We estimated costs of providing Gardasil, Prevnar, and Zostavax to eligible patients at a US Ryan White Part C academically affiliated HIV clinic in 2013. Clinic expenditures were calculated using vaccine price and administrative fees. Revenue was calculated using insurance reimbur

Impact of Social Determinants of Health in the Care of Moebius Syndrome: A Case Report.

Moebius syndrome is a rare congenital disorder characterized by nonprogressive uni- or bilateral abducens nerve (VI) and facial nerve (VII) palsy. Other cranial nerves (CN) such as CN III, IV, and IX-XII may be involved leading to varied presentations. Speech development, communication, and psychomotor complications are common. Given the complexity of the disease, patients require an individualized, multidisciplinary care plan involving many medical specialists. Accessing this level of care without insurance poses seemingly insurmountable challenges and places immense financial strain on both

Unmet mental health needs of women in public-sector gynecologic clinics.

OBJECTIVE: We examined the mental health needs of poor young women seen in public-sector gynecologic settings. STUDY DESIGN: Participants were 205 ethnically diverse women, mean age 29 years, seen by gynecologists at San Francisco General Hospital, all either uninsured or receiving public medical assistance. An interview with an instrument designed to diagnose mood, anxiety, alcohol, and eating disorders in medical patients, the Prime-MD, was used to assess current mental disorders. Any history of sexual or other physical assault was recorded. Receipt of primary care was scored for comprehens

The Emergency Medical Treatment and Active Labor Act: Clinical Practice and Ethical Considerations.

The Emergency Medical Treatment and Active Labor Act (EMTALA), enacted in 1986, established the legal foundation for access to emergency medical care in the United States. This review examines EMTALA's historical origins, core requirements, and implications for pediatric emergency practice. The law emerged in response to widespread patient dumping practices in the 1980s, when hospitals routinely transferred or refused care to uninsured patients. In the 4 decades since, amendments and regulatory clarifications have expanded the law's scope and strengthened enforcement mechanisms. The modern ite

Effects of health insurance on out-of-pocket payments: a coarsened exact matching analysis using Kenya's 2022 demographic and health survey.

OBJECTIVES: Out-of-pocket payments remain a major source of healthcare financing in Kenya, yet it is unclear whether health insurance coverage is associated with lower out-of-pocket spending due to non-random enrolment. This study uses coarsened exact matching (CEM) to compare out-of-pocket costs (OOPCs) between insured and uninsured individuals. DESIGN, SETTING AND PARTICIPANTS: This study used cross-sectional data from the 2022 Kenya Demographic and Health Survey which covered 80 780 household members. Participants with missing or incomplete information on health insurance status and OOPC w

Justice system experiences and health risks among Medicaid and uninsured populations receiving treatment for substance use disorder.

INTRODUCTION: We explored the relationship between justice system experiences and health risks using a sample of Medicaid and uninsured patients in addiction treatment, focusing on four health behaviors correlated with premature mortality: (1) opioid misuse, (2) overdose, (3) multi-substance use, (4) recurrent treatment admissions. METHODS: From 2021 to 2023, electronic in-depth life-course history surveys were administered in-person with adult patients selected from a community-based public health case management system while undergoing addiction treatment in Michigan, USA. Healthcare Expenditure Burden Among Non-elderly Cancer Survivors, 2008-2012.

INTRODUCTION: There is increasing concern regarding the financial burden of cancer on patients and their families. This study presents nationally representative estimates of annual out-of-pocket (OOP) burden among non-elderly cancer survivors and assesses the association between high OOP burden and access to care and preventive service utilization. METHODS: Using the 2008-2012 Medical Expenditure Panel Survey, 4,271 cancer survivors and 96,780 individuals without a history of cancer were identified, all aged 18-64 years. High annual OOP burden was defined as spending >20% of annual family inc

Physician unawareness of serious substance abuse.

PURPOSE: To describe and assess the prevalence of perceived physician unawareness of serious substance abuse. PATIENTS AND METHODS: We report an observational study with validation of multivariable results of data collected by interview from persons presenting for addictions treatment in the public system who reported having a physician. RESULTS: Of 3,253 patients interviewed, 87% (2,843) responded to the question about having a physician. Of 1,440 patients who stated that they had physicians, 45% (651) reported that the physician who cared for them was unaware of their substance abuse. In m

It takes a village: a multidisciplinary model for the acute illness aftercare of individuals experiencing homelessness.

Homeless individuals are often uninsured and are more likely than the housed to utilize acute health care services and experience longer hospitalizations. Currently in the United States, there are fragmented services available for the aftercare of these patients to ensure continuum of care, promote healing, and avoid re-entry into the acute care system. The Fourth Street Clinic Respite Program was created to address these issues. Patients are referred to the program from local hospitals and other service providers. Based on the acuity of illness and need for nursing care, patients are admitted

Insurance, self-reported medication adherence and LDL cholesterol: The REasons for Geographic And Racial Differences in Stroke study.

BACKGROUND: Lack of health insurance may adversely impact medication adherence and the control of cardiovascular risk factors. We examined if the association between insurance and LDL-C is due to self-reported low medication adherence. METHODS: This cross-sectional study included 8685 black and white men and women aged 45 and older who participated in the REasons for Geographic And Racial Differences in Stroke (REGARDS) cohort and used statins. Medication adherence was assessed using the 4-item Morisky Medication Adherence Scale (MMAS-4). Mean differences in LDL-C between participants with an

Insurance-associated disparities in cancer screening and adult immunization in a predominantly Asian immigrant community: a cross-sectional survey at two community health fairs in Garland, Texas.

BACKGROUND: Preventive screening and adult immunizations reduce morbidity and mortality, yet uptake remains uneven. Immigrant communities, particularly those facing language barriers, limited insurance coverage, and preventive health hesitancy, experience persistent gaps in preventive care. Asian immigrant populations, including Vietnamese immigrants, remain underrepresented in preventive health research. METHODS: A cross-sectional survey was conducted at two community health fairs held in September and October 2025 in Garland, Texas: one at a Vietnamese church and one at the Dallas Police As

The Shadow Price of Uncertainty: Consequences of Unpredictable Insurance Coverage for Access, Care, and Financial Security.

UNLABELLED: Policy Points Health insurance reform in the United States has fostered enrollment to promote access to care and reduce financial insecurity. However, enrollees' inability to reliably predict what insurance will cover (a.k.a. "coverage uncertainty") impedes these goals, often as much as being uninsured. The Patient Protection and Affordable Care Act initially expanded enrollment and reduced coverage uncertainty. After the mid-2010s, trends in coverage uncertainty plateaued, and it now impedes access to care for four times as many households as lack of health insurance. A variety of

Disparities in refusal of surgery for gynecologic cancer.

OBJECTIVE: To identify sociodemographic and clinical factors associated with refusal of gynecologic cancer surgery and to estimate its effect on overall survival. METHODS: The National Cancer Database was surveyed for patients with uterine, cervical or ovarian/fallopian tube/primary peritoneal cancer treated between 2004 and 2017. Univariate and multivariate logistic regression were used to assess associations between clinico-demographic variables and refusal of surgery. Overall survival was estimated using the Kaplan-Meier method. Trends in refusal over time were evaluated using joinpoint re

Exacerbating disparities?: Cervical cancer screening and HPV vaccination.

The human papillomavirus (HPV) vaccine is a powerful tool in cancer prevention. In combination with cervical cancer screening programs, this vaccine has the potential to nearly eliminate death from cervical cancer. However, this remarkable public health success can only be realized if vaccines reach those most at risk-unscreened women. Vaccinating only those already well-screened would have little impact on cervical cancer mortality, exacerbate disparities, and be a striking public health failure. We use data from the Behavioral Risk Factor Surveillance System optional Adult HPV Vaccination mo
