Mesocolic hernia following retroperitoneal laparoscopic radical nephrectomy: A case report.

INTRODUCTION: Small bowel obstruction (SBO) caused by an internal hernia through a mesocolon after retroperitoneal laparoscopic nephrectomy (RLN) is rare. PRESENTATION OF CASE: A 66-year-old man who had undergone RLN with bladder cuff excision for a left renal pelvic cancer. After the surgery, he experienced SBO repeatedly. Contrast-enhanced computed tomography (CT) and gastrografin contract radiography through a long tube showed an internal hernia through the mesocolon to the retroperitoneal space where the resected left kidney had been located. We performed a subsequent surgery for the inte

Spontaneous resolution of severe, symptomatic mesocolic panniculitis: a case report.

BACKGROUND: Mesenteric panniculitis is a rare chronic fibrosing inflammatory disease that typically affects the adipose tissue and mesentery of the small intestine but may also affect the mesosigmoid and the mesocolon. The pathology of this disease remains unclear despite association with some malignancies or inflammatory disorders. We report a case of mesocolic panniculitis and a literature review of its clinical presentation, imaging findings, associated conditions and treatment options. CASE PRESENTATION: A 64 year-old Caucasian man was admitted to the gastroenterology department for sever

Mesenteric cysts in children.

BACKGROUND: Mesenteric cysts are uncommon benign abdominal masses. Approximately one third of patients with these lesions are children. METHODS: We reviewed our clinical records for the past 14 years (corresponding to the period of time in which ultrasonography and computed tomography became reliable methods for imaging pediatric patients) and found 10 patients with mesenteric cysts. RESULTS: We were surprised to discover that abdominal pain was a presenting complaint in all but one patient. Five of the patients had the findings of an acute surgical abdomen and were thought to have appendici

Peritonitis caused by a ruptured, infected mesenteric cyst initially interpreted as an ovarian cyst. A case report.

BACKGROUND: Mesenteric cysts are rare intraabdominal lesions of childhood that may vary in presentation from an asymptomatic mass to an acute abdomen. CASE: We encountered a rare case of peritonitis caused by a ruptured, infected mesenteric cyst of the mesocolon in a female child that had been initially interpreted as a ruptured ovarian cyst. CONCLUSION: When confronted with a large unilocular or septated echolucent mass in the lower abdomen in female children, gynecologists should consider a mesenteric cyst in the differential diagnosis.

Mesenteric tears of the descending (small) colon as a postpartum complication in two mares.

Segmental ischemic necrosis of the descending colon associated with discrete tears of the mesocolon were diagnosed in 2 mares following foaling. After foaling, both mares had a slow onset of signs of abdominal pain and gradual deterioration of clinical status. Increases in peritoneal fluid total protein concentration and WBC count and a palpable impaction of the descending colon on examination per rectum were useful diagnostic aids. Surgical intervention was successful in one mare, but not in the other.

Standardization before radicality: Current evidence for complete mesocolic excision.

BACKGROUND: Despite the ongoing discussion around complete mesocolic excision (CME) during right colectomy, it remains a poorly defined concept, and its oncologic role in the treatment of right-sided colon cancer remains unclear. Since its original description, CME has been interpreted inconsistently, with multiple techniques and definitions used across studies. This heterogeneity, combined with a reliance on retrospective and observational data, has limited the meaningful assessment of efficacy. METHODS: This narrative literature review aimed to summarize recent data evaluating the oncologic

Efficacy of the "critical view/open book" concept for implementing standardized right hemicolectomy with complete mesocolic excision-results from a series of minimally invasive surgical training courses.

BACKGROUND: Since the introduction of right hemicolectomy with complete mesocolic excision (CME) the implementation of minimally invasive CME as standard of care still remains challenging. Among several established guidelines for minimally invasive CME, the "critical view/open book" concept has been developed to provide a stepwise standardization with safety checkpoints for teaching purposes. The present study evaluates this concept during its practical application in a large series of surgical training courses. METHODS: In this prospective observational study, seven consecutive 2-day surgica

Colon cancer: feeling the imbalance?

While neoadjuvant chemotherapy has become the standard of care for rectal cancers in most centres, there is much interest in neoadjuvant chemotherapy in colon cancer after the recent publication of the FOxTROT trial. The management of colon cancers seems to be heading down the same path as rectal cancer, where the radicality of surgery is replaced by chemotherapy intensification. The role of demanding procedures such as complete mesocolic excision with central venous ligation in this new paradigm of upfront chemotherapy remains uncertain and uninvestigated.

[Fascial anatomy of ligamentous structures associated with colon cancer surgery].

The ligamentous structures integral to the surgical management of colon cancer include the gastrocolic ligament, the phrenicocolic ligament, and the splenocolic ligament. Historically, the era of conventional open surgery was characterized by the use of large forceps for clamping and ligating these ligaments. However, the advent of fascial and mesenteric anatomy research has ushered in a paradigm shift. Aided by high-definition laparoscopy, colorectal surgeons have progressively clarified the fundamental anatomical structures, thereby refining surgical techniques in accordance with fascial and

[Membrane anatomy in right colon cancer: definition and identification of mesocolic completeness].

Complete mesocolic excision (CME) and D3 resection of right colon cancer have been widely implemented, but the definition and identification of the completeness of the mesentery have not been fully agreed, especially the dorsal and medial borders. In this paper, we proposed the dorsal fascia of the colonic mesentery as the dorsal border of the mesocolon and the line connecting the roots of the ileocolic artery and the middle colic artery (ICA-MCA line) as the medial border of the CME by systematically studying the relationship between the mesentery and the mesenteric bed from the theory of mem

Consensus statements on complete mesocolic excision for right-sided colon cancer-technical steps and training implications.

BACKGROUND: CME is a radical resection for colon cancer, but the procedure is technically demanding with significant variation in its practice. A standardised approach to the optimal technique and training is, therefore, desirable to minimise technical hazards and facilitate safe dissemination. The aim is to develop an expert consensus on the optimal technique for Complete Mesocolic Excision (CME) for right-sided and transverse colon cancer to guide safe implementation and training pathways. METHODS: Guidance was developed following a modified Delphi process to draw consensus from 55 internat

Mesocoloplasty for sigmoid volvulus: a preliminary report.

A technique of mesocoloplasty for the prevention of recurrence of sigmoid volvulus is described. The manoeuvre is simple, and does not open the gut. It can be performed confidently by junior doctors undergoing surgical training and appears to be an eminently safe technique as a routine procedure in any emergency service.

Right side fixation of sigmoid colon with a hepato-sigmoidocolic fistula in patient with hepatocellular carcinoma and midgut malrotation.

The location of the sigmoid colon varies within the abdominal cavity, but its mesocolon is fixed to the left side. Right side fixation of the sigmoid colon is a very rare congenital positional anomaly. In addition, it has been reported that hepatocolic fistula is also a very rare disease that may present lower gastrointestinal bleeding. Here, the authors describe a case of a 71-year-old man who underwent surgery for hepato-sigmoidocolic fistula complicated by hepatocellular carcinoma and the right side fixation of the sigmoid colon.

[A left paraduodenal hernia--a case report].

The left paraduodenal hernia obtains when the small intestine goes through the left paraduodenal fosset. It (the hernia) develops on the left, at the back of the descending mesocolon. The present authors study one case looking at its pathogeny and the clinical and therapeutic problems posed by such a variety of congenital internal hernia. The authors emphasize, in particular, its morbid associations with the volvulus of the pelvic colon. Finally they highlight the importance of a through and careful exploration of the abdominal cavity during any laparatomy following an occlusive syndrome.

Laparoscopic sigmoidectomy postopen aortic replacement for abdominal aortic aneurysm: a case report.

UNLABELLED: Colectomy for colorectal cancer after an open aortic replacement (OAR) for abdominal aortic aneurysms has high perioperative complication and mortality rates. CASE PRESENTATION: The authors report the case of an 87-year-old man who underwent laparoscopic sigmoidectomy. The patient presented with edema of the lower legs and face, and blood test results indicated anemia. The patient had a history of OAR for an abdominal aortic aneurysm 9 years prior, a left common iliac artery aneurysm, and a jump bypass graft. A colonoscopy revealed a type 2 lesion in the sigmoid colon; he was diag

Umbilical single incisional approach plus one port for partial transverse colectomy: initial operative experience.

BACKGROUND/AIMS: Although single incision laparoscopy surgery (SILS) has recently been applied to various kinds of disease, there are few reports on partial transverse colectomy by SILS. We have been performing SILS plus one port procedure, SILS plus One. SURGICAL PROCEDURE: After placing 3cm incision in the umbilical fossa, a SILS portTM was introduced, and a 5-mm port was inserted in the upper left quadrant of the abdomen, and laparoscopic procedure was started. After the greater omentum was divided, inferior margin of the pancreas was dissected in order to detect the gastrocolic trunk and

Transmesocolic ureteral intraperitonealization: a new approach for laparoscopic treatment of retroperitoneal fibrosis.

INTRODUCTION: Retroperitoneal fibrosis (RPF) is characterized by the presence of an inflammatory fibrotic process in the retroperitoneum causing compression of the retroperitoneal structures including the ureters. The ureterolysis is the liberation of the incarcerated portion of the ureter, from its proximal healthy portion to the distal portion, generally free of fibrosis, below the iliac vessels. We report the transmesocolic ureteral intraperitonealization as a new approach for laparoscopic treatment of RPF. PATIENT AND METHODS: A 52-year-old female patient diagnosed with idiopathic RPF was
