Moral flexibility without mutual benefits: From change to disagreement.

Resource-rational contractualism claims that moral flexibility leads to mutually beneficial agreements through renegotiation when new situations arise. We challenge this view using COVID-19 evidence, where moral change leads to persistent disagreement rather than consensus. We demonstrate that three psychological factors - social identity, confirmation bias, and emotional responses - systematically prevent convergence, revealing limitations in the resource-rational contractualism model.

Dynamic complexity of moral motivation: the roles of duty and self-interest.

Insights from moral philosophy are identified and presented to explicate the conception of moral motivation as a complex dynamic system based on multiple motivational tendencies that have competitive and cooperative relationships with each other. As part of process-relational metatheory, the perspective of complex dynamic systems is applied. Self-interest and duty are conceptualized as motivational tendencies. Two specific forms of duty are examined: justice as duty and care as duty. The psychological construction and regulation of duty and self-interest are discussed as fundamental processes

Moral distress and attitudes toward intramural goal-of-care discussions.

BackgroundAdvance directives (ADs) document the patient's preferences regarding future medical care and are used to ensure goal-concordant care and patient autonomy when the patient becomes incapacitated. In a clinical setting, goals of care (GOC) discussions are a tool that helps health care providers (HCPs) provide goal-concordant care. A lack of documentation of the patient's wishes, either as AD or following GOC discussions, can, especially in emergency or critical situations, lead to moral distress among HCPs, a psychological disequilibrium that stems from the perceived inability to act i

The REAL journey: a meaning-oriented approach to moral injury care.

This paper describes three theoretical models that focus on meaning-making as a foundation for moral injury conceptualization and care, as manifested in the Reclaiming Experiences and Loss (REAL) moral injury group therapy. The philosophical foundation of the REAL group is Viktor Frankl's logotherapy, which emphasizes the role of the human spirit and posits that meaning is achieved through living out one's fundamental responsibility to actualize whatever values are available in a given moment. Constructive psychotherapy operates from core assumptions about client autonomy, an inherent drive to

Identity scaffolding as a moral need in the caretaking of unaccompanied refugee minors.

BACKGROUND: Unaccompanied refugee minors (URMs) represent one of the most vulnerable groups within the global refugee population, having fled armed conflict without familial support. Although policies in high-income countries secure URMs' basic rights-housing, education, healthcare and legal protection-these standardised frameworks frequently neglect the sociocultural and relational dimensions of identity integral to their well-being. OBJECTIVE: To articulate the moral imperative of identity scaffolding in URM care, demonstrating how current approaches enforce assimilation, exacerbate identit

Clinical Ethics in Name Only? The Case for Moral Realism at the Bedside.

As Clinical Ethics Consultation (CEC) becomes more institutionalized, concerns about its legitimacy and efficacy have grown. We defend a realist orientation, according to which CEC has normative force only if its recommendations answer to stance-independent reasons, not to individual preferences and institutional policies. Public justification can and should proceed through familiar means (e.g., shared mid-level norms and transparent procedures) so that stakeholders can scrutinize the reasoning. We show how neutrality, admirable as a procedural virtue, hollows CEC when treated as a master idea

Autistic perspectives on the moral and ethical considerations of genetic testing for autism.

This study aimed to characterize the nuanced perspectives expressed by Autistic adults about genetic testing for autism. In this secondary analysis of an existing dataset, we used inductive content analysis of free-text responses that Autistic adults wrote in response to questions asking about perceptions of genetic testing for autism that had been included in a previous survey. Participants who completed the survey had been recruited online. Three-hundred eighty-nine participants wrote a total of 28,774 words in response to the questions of interest. While some respondents wrote about potenti

Maternity providers' moral experiences addressing maternal drug use.

BackgroundAddressing drug use among pregnant and postpartum women is complicated by the social, ethical, and legal interplay between maternal and child needs. The moral experiences of providers as they care for this population are poorly understood. The purpose of this review is to synthesize the qualitative literature on the moral experiences of prescribing maternity providers managing the care of pregnant and postpartum women who use drugs (PPWUD), specifically focused on drug testing and child welfare reporting decisions.MethodsThe Hunt and Carnevale (2011) bioethics framework was used to d

Identity in turmoil: Investigating the morally injurious dimensions of minority stress.

Background: Sexual and gender minorities (SGMs) are at an increased risk for developing mental health disorders due to their socially stigmatized identities. Minority stress (i.e. discrimination, identity nondisclosure, internalized stigma) has been shown to impact mental health outcomes among SGMs. Both distal and proximal minority stressors may serve as potentially morally injurious events (PMIEs), which may lead to moral injury and trauma/stressor-related symptoms. Critically, minority stress-related moral injury among SGMs has never before been explored using a mixed-methods approach.Metho

The anatomy of ontological ambiguity: exploring moral distress in cadaveric dissection.

This study investigates sources of distress experienced by some medical students performing cadaveric dissection in the anatomy lab. Using a novel theoretical framework adapted from the humanities, faculty at the University of Rochester sought to understand the connection between distress, dissection, and the ontological assumptions students hold about cadavers. Between 2019 and 2021, the authors engaged in an iterative inductive thematic analysis to better understand student experiences. We conducted 14 semi-structured individual interviews with medical students about their experiences. We fo

Veterans' experiences of moral injury, treatment and recommendations for future support.

INTRODUCTION: Moral injury (MI) significantly impacts the lives of many UK military veterans however, there is a lack of manualised treatment to address the needs of this population. To develop future treatments that are acceptable and well tolerated, veterans should be consulted on their experiences of existing psychological treatments and suggestions for future treatments. METHODS: 10 UK military veterans were interviewed about their experiences of receiving treatment for psychological difficulties after MI, and beliefs about core components of future treatments. Thematic analysis of these

Moral Panic and Electric Micromobilities: Seeking Space for Mobility Justice.

This article makes the case that electric micromobilities (EMMs) are the site of a moral panic and employs the lens of mobility justice to explain it. Through analysis of scholarly and media discourse, interviews with, and social media content produced by, EMM riders (eriders), and the auto ethnographic experiences of the lead author as an electric unicycle rider in daily life, as a participant in online and offline "erider" communities, and as a food delivery worker, we reinforce the conclusion that alternate mobilities face an uphill battle in gaining legitimacy and inclusion in transportati

Pregnancy and superior moral status: a proposal for two thresholds of personhood.

In this paper, I suggest that, if we are committed to accepting a threshold approach to personhood, according to which all beings above the threshold are persons with equal moral status, there are strong reasons to also recognise a second threshold that would be reached through human pregnancy, and that would confer on pregnant women a temporary superior moral status. This proposal is not based on the moral status of the fetus, but on the moral status of the pregnant woman. It is not only the fetus which is an organism sui generis: the pregnant woman, also, is a unique being. Following almost

Confirming and Expanding the Moral Incongruence Model of Compulsive Sexual Behavior.

Compulsive sexual behavior (CSB) is a clinical syndrome in which repetitive sexual behaviors, urges, or thoughts cause significant distress and impairment. Moral incongruence (MI), or conflict between an individual's sexual behaviors and moral beliefs, is highly associated with self-reported problematic pornography use, a common behavioral manifestation of CSB. However, it is currently unknown whether MI is associated with other self-reported compulsive sexual behaviors, such as masturbation, sexual fantasy, and sex with multiple partners. Additionally, research on MI has not explored whether
