Clinical applications of palifermin: amelioration of oral mucositis and other potential indications.

Mucositis is one of the most significant toxicities in cancer patients undergoing cytotoxic treatment. It can have a negative impact on both quality of life and health economics. Severe oral mucositis can contribute to hospitalization, need for narcotic analgesics, total parentral nutrition, suboptimal delivery of anti-neoplastic treatment, and morbidity and mortality. Palifermin, a recombinant derivative of human keratinocyte growth factor, is the first active agent approved by the FDA for the prevention of severe oral mucositis in patients undergoing haematopoietic stem cell transplantation …

Prevention of chemotherapy-induced ulcerative mucositis by transforming growth factor beta 3.

Mucositis is a common, dose-limiting complication in patients receiving cancer chemotherapy, which appears to be a consequence of the rate of epithelial proliferation. The beta transforming growth factors have been shown to be negative regulators of epithelial cell proliferation. Here we show that transforming growth factor beta 3 administration reduced proliferation of oral epithelium in vitro and in vivo. Topical application of transforming growth factor beta 3 to the oral mucosa of the Syrian golden hamster prior to chemotherapy significantly reduced the incidence, severity, and duration of…

Oral Microbiome Dynamics in Patients with Acute Lymphoblastic Leukemia and Oral Mucositis.

The oral microbiome of patients with acute lymphoblastic leukemia (ALL) undergoes changes caused by the neoplasia as well as the antimicrobial activity of chemotherapy (CTX), which promotes the development of oral mucositis (OM). This study aimed to analyze the oral microbiome dynamics and salivary cytokine production in pediatric ALL patients before and during CTX, comparing children who did and did not develop OM. We conducted a longitudinal, observational, and analytical study including 32 newly diagnosed pediatric ALL patients (ages 2-16 years) undergoing CTX. Oral rinse and non-stimulated…

Huangqi Baihe Granules alleviate chemotherapy-induced intestinal mucositis in Drosophila and mice.

Chemotherapy-induced intestinal mucositis (CIM) is a significant dose-limiting adverse effect of cancer treatment. Huangqi Baihe Granules (HQBHG) derived from Dunhuang’s ancient medical texts could alleviate radiation brain injury and hypobaric hypoxia-induced acute lung injury. Here, the protective effect and mechanism of HQBHG against irinotecan (CPT-11)-induced intestinal mucositis were detected by using Drosophila melanogaster and mouse models. Oral administration of HQBHG could significantly ameliorate body injury caused by CPT-11, including increased survival rate, rescued locomotion, al…

The Efficacy of Episil® for Radiation-induced Oral Mucositis in Rats.

OBJECTIVE: Radiation therapy is an indispensable treatment for head and neck cancer; however, the development of radiation-induced oral mucositis causes erythema and ulcers in the patient's oral cavity, resulting in severe pain. The purpose of this study was to investigate the efficacy of Episil® in the treatment of radiation-induced oral mucositis. STUDY DESIGN: Seventy-six F344 rats were randomly divided into an Episil® group (n=38) and a control group (n=38). After irradiation, the rats were evaluated based on body weight measurements, the oral mucositis index (OMI), tissue myeloperoxidase…

Study on thyroid autoimmunity of oral lichen planus and oral lichenoid mucositis.

CONTEXT: Oral lichen planus (OLP) and oral lichenoid mucositis (OLM) are immune-mediated mucosal conditions with multifactorial etiology, with similar clinical and histological characteristics. Diagnosis should be confirmed considering both clinical and histological aspects. Some subsets are associated with immune-mediated thyroid dysfunction due to anti-thyroid peroxidase antibody (TPO) and anti-thyroglobulin antibody (TGA). AIMS: To ascertain the presence of thyroid autoantibodies in a cohort of patients with OLP and OLM from a South Indian population. METHODS AND MATERIALS: This observati…

Monte Carlo based dosimetry of extraoral photobiomodulation for prevention of oral mucositis.

Photobiomodulation therapy (PBMT) is recommended for prevention and treatment of oral mucositis, a painful condition that occurs in cancer patients. Intraoral PBMT is limited to treating distal oral mucosa and oropharynx. Extraoral PBMT may provide a more efficient intervention. The goal of this study was to develop a clinically viable protocol for extraoral PBMT. Monte Carlo modeling was used to predict the distribution of 850 nm light for four treatment sites, using anatomical data obtained from MRI and optical properties from the literature. Simulated incident light power density was limite…

Anakinra and dexamethasone treatment of idarubicin-induced mucositis and diarrhoea in rats.

Chemotherapy-induced mucositis, characterized by diarrhoea and villous atrophy, is a severe side effect contributing to reduced quality of life and premature death in cancer patients treated with cytostatics. Despite its high incidence, there is no effective supportive therapy available. The main objective of this study was to determine if the anti-inflammatory drugs anakinra and/or dexamethasone-which have different mechanisms-of-action-might be used to effectively treat idarubicin-induced mucositis in rats. Mucositis was induced through a single injection with 2 mg/kg idarubicin (with saline…

Radiotherapy-induced severe oral mucositis: pharmacotherapies in recent and current clinical trials.

INTRODUCTION: Oral mucositis (OM) is among the most common, damaging side effects of head and neck radiation therapy and may interfere with patients' ability to comply with optimal treatment. AREAS COVERED: The increasing unmet clinical need, recent clinical trial successes, and the commercial potential have catalyzed interest in the development of effective intervention for OM. A range of small molecules are under development - some still in the preclinical stage, but others close to NDA submission. This review will focus on those drugs which have recently been assessed in a clinical trial a…

Propolis mouthwash for preventing radiotherapy-induced mucositis in patients with nasopharyngeal carcinoma.

BACKGROUND: Nasopharyngeal carcinoma (NPC) is the most common head and neck cancer in Malaysia. The gold standard treatment of NPC is radiotherapy (RT), as NPC is a radiosensitive tumour. Although RT is successful in treating NPC, patients cannot avoid the resulting RT complications. Oral mucositis is the most frequently encountered debilitating complication of RT and has no specific preventive treatment. The aim of this study was to evaluate the efficacy and safety of a 2.5% propolis mouthwash for preventing RT-induced mucositis in patients with NPC. MATERIALS AND METHODS: The study was a pr…

Radiation-induced oral mucositis presenting as atypical vascular proliferation: a case report.

Radiation-induced oral mucositis is an oral mucosal injury caused by radiation ionizing radiation, which often manifests as oral mucosal congestion, erosion, and ulcers. Radiation-induced oral mucositis manifesting as vascular proliferative changes in the oral mucosa has not been reported. We report a case of oral mucosal atypical vascular proliferation after radiotherapy for a malignant maxillofacial tumor. We discussed the mechanism and treatment of aty-pical vascular proliferation in the oral mucosa secondary to radiotherapy, including diagnosis, treatment, and previous literature.

Compound Kushen injection reduces severity of radiation-induced gastrointestinal mucositis in rats.

Mucositis, or damage/injury to mucous membranes of the alimentary, respiratory, or genitourinary tract, is the major side effect associated with anticancer radiotherapies. Because there is no effective treatment for mucositis at present, this is a particular issue as it limits the dose of therapy in cancer patients and significantly affects their quality of life. Gastrointestinal mucositis (GIM) occurs in patients receiving radiotherapies to treat cancers of the stomach, abdomen, and pelvis. It involves inflammation and ulceration of the gastrointestinal (GI) tract causing diarrhea, nausea and…
