Out of a population of 188 unrelated narcoleptic probands, we identified 14 probands (7.44%) with a family history of narcolepsy, 23 (12.23%) with a family history of isolated repeated episodes of naps and/or lapses into sleep and 151 (80.31%) without a family history of either condition. Since the discovery of an almost 100% association of HLA-DR2 with narcolepsy-cataplexy, many efforts have been made to demonstrate the intervention of immune factors in the pathogeny of the disease. Fibrositis is often associated with sleep disturbances and with an alpha nREM abnormality on sleep electroencephalogram. We describe a case occurring during the course of a typical longstanding narcolepsy-cataplexy. Modafinil, that is an effective treatment of hypersomnia, did not alleviate the symptoms of fibrositis in the short term.

Eight pairs of subjects (each consisting of a narcoleptic and a control matched on the basis of age, sex, educational background and job) were evaluated under the following double-blind, randomized treatment conditions: baseline, placebo, low dose and high dose methamphetamine. Subjects were drug-free for 2 weeks prior to beginning the protocol. Five patients with narcolepsy-cataplexy (aged 31-69, mean age at onset 15 years) and their 47 relatives were examined. They were interviewed in detail, all patients and 16 relatives (including all symptomatic cases) were examined neurologically and a multiple sleep latency test and HLA-typing were done. One case of narcolepsy and two cases of idiopathic hypersomnia were identified. All the other 45 relatives were symptom-free. The observed rate of familial cases (1/5 for narcolepsy and 2/5 for idiopathic hypersomnia) corresponds the results of other recent studies.

Narcolepsy is characterized by excessive daytime sleepiness and cataplexy. It had been reported that 100% of Japanese and Caucasian narcoleptic patients are HLA-DR2 and HLA-DQw1 positive; last year, however, exceptionally rare cases of DR2-negative narcolepsy were reported. Tissue typing was performed on 14 narcoleptics as defined by both strict sleep laboratory and clinical criteria. Six of these patients were blacks from North America, a race underrepresented in previous studies. All patients were HLA-DR2-antigen positive and had the same HLA-DR2 subtype. Eighteen black patients with narcolepsy underwent human leukocyte antigen (HLA) typing. Eleven of the 18 had cataplexy. Twelve patients (66.6%) had HLA-DR2; 7 patients with cataplexy had DR2. All patients had DQw1. In contrast to white and Japanese patients studied to date, not all black narcoleptics have DR2.

We reviewed the records of 755 patients in whom narcolepsy was diagnosed between 1979 and 1983. In a subset of 405 previously undiagnosed and untreated patients, the following variables were assessed: the subspecialty in which the condition was diagnosed, age and sex of the patients, family history, expressed symptoms in the total group, symptoms in patients whose condition was diagnosed by ophthalmologists, laboratory aids used for diagnosis, treatment, and outcome.

The effectiveness of codeine as a treatment for the excessive daytime sleepiness of narcolepsy was studied in two experimental trials. In an open trial of codeine in five narcoleptic subjects, dramatic clinical improvement was reported. The antihypertensive drug clonidine was given in an open trial to two treatment-resistant (psychostimulants and tricyclic antidepressants) narcoleptic patients. When given acutely, clonidine suppressed REM sleep. However, patients became tolerant after repeated doses but did not lose the drug's beneficial clinical effects. This suggests that REM suppression may not be necessary for improvement in narcolepsy. Assessments of both clinical and polysomnographic variables before and after the trial indicate that clonidine may have a place in the treatment of some patients with narcolepsy.

Behavioral, electrophysiologic, and pharmacologic tests were performed on a 2 1/2-year-old Brahman bull suspected of having narcolepsy. Placement on a tilt table or electroejaculation induced apparent sleep and cataplexy. We obtained medical and psychological assessments and 48-hr polysomnographic recordings on five sisters, three of whom had narcolepsy. Of the three, two were identical twins.
