[Adrenal gland necrosis following acute pancreatitis].

Acute pancreatitis is a rare pathology, whose incidence tends to increase in Western countries, and whose severity is variable; severe forms (10-20 %) are associated with significant mortality, ranging from 13 to 35 % depending on the studies. Alongside the usual complications of such afflictions, rarer complications, especially related to an adjacent or distant organ involvement, can cause unusual symptoms and contribute to life-threatening risks for the patient. This case illustrates one of these uncommon complications, in the form of necrosis of an adrenal gland in the onset of severe necro…

Jejunogastric Invagination Complicated With Intestinal Necrosis.

Jejunogastric intussusception (JGI) is an uncommon complication of gastric surgery. Its causes are varied, and it is classified as acute or chronic depending on the time of onset. The clinical presentation is usually nonspecific, with symptoms such as epigastric pain, nausea, vomiting (with or without blood), and signs of upper gastrointestinal obstruction, which can be mistaken for neoplastic disease or peptic ulcer disease. The diagnosis is established by endoscopy or radiology, with computed tomography (CT) being particularly useful for defining the site of intussusception by demonstrating

Epipericardial fat necrosis: A case report.

Epipericardial fat necrosis is a rare cause of acute pleuritic chest pain and is a benign and self-limiting condition. It is important to distinguish epipericardial fat necrosis from other diseases that cause acute chest pain, such as acute myocardial infarction, pulmonary embolism, and acute pericarditis, because conservative treatment is recommended for epipericardial fat necrosis. This report presents the case of a 25-year-old man with severe pleuritic chest pain located on the left anterior side that was associated with dyspnea. Electrocardiogram and laboratory data were normal, except for…

Post radiotherapy femoral head avascular necrosis.

Osteonecrosis is the death of bone cells due to insufficient blood supply; radiotherapy for various underlying malignancies is one of the uncommon causes. Microvascular damage or underlying tissue fibrosis, which leads to an ischemic environment and cell death, is a proposed mechanism. Factors influencing risk of radiation induced AVN include type of radiation whether external beam radiotherapy or brachytherapy, age of the patient, included body part and concomitant additional steroid or chemotherapy treatment. In this case report we brought a case of 40-year-old male patient with right proxim…

Acetaminophen-Induced Hepatic Necrosis: A Reminiscence.

In the early 1970s, Dr. B. B. Brodie, head of the Laboratory of Chemical Pharmacology, National Heart Institute, National Institutes of Health, initiated a program to elucidate the mechanism of hepatic necrosis induced in rats by bromobenzene. These studies showed a crucial role for its 3,4-epoxide intermediate, known in part, to collapse to 4-bromophenol. To examine a possible contribution of this phenol to tissue toxicity, some rats were coadministered a high dose of acetaminophen to suppress phenolic clearance by glucuronidation and sulfation. Subsequent examination of liver slices showed t…

Endoscopic therapies for walled-off necrosis.

BACKGROUND AND AIMS: Management of walled-off necrosis (WON) has focused on a step-up approach using minimally invasive drainage and debridement techniques. This document reviews the technical aspects of endoscopic management of WON, technical and clinical success, adverse event rates for endoscopic drainage, and dedicated endoscopic devices for direct endoscopic necrosectomy (DEN). METHODS: The MEDLINE database was searched through April 2022 for relevant articles using the key words walled-off necrosis, endoscopic necrosectomy, interventional EUS, severe acute pancreatitis, necrotizing panc…

Colon necrosis in acute severe pancreatitis.

A 78 year-old woman was admitted for biliary acute pancreatitis (AP). Fluid and analgesia were initially administered. Her clinical course was poor with persisting abdominal pain, intestinal paresis and fever development. On her 7th admission day a contrast-enhanced computed tomography scan was performed where a huge necrotic peripancreatic collection was found with gastric compression .

Avascular Necrosis in Renal Transplant Patients.

OBJECTIVES: Kidney transplant recipients are at increased risk for avascular necrosis due to steroid use and accompanying comorbidities. Concerning risk factors, uncertainty still exists. We evaluated the clinical characteristics and risk factors of avascular necrosis in kidney transplant recipients. MATERIALS AND METHODS: Symptomatic avascular necrosis was found by magnetic resonance imaging in 33 of 360 kidney transplant patients between 2005 and 2021. The patients' clinical characteristics, biochemical testing, and medications were evaluated. RESULTS: We found the frequency of avascular n…

Progressively confluent monkeypox lesions with necrosis.

Human monkeypox virus became a burgeoning global health issue when outbreaks were identified in over 100 countries beginning in early 2022. We describe the case of a 38-year-old male with acquired immunodeficiency syndrome who presented one month after the development of painful anal lesions, subsequently confirmed to be monkeypox. The patient was unsuccessfully treated outpatient with multiple courses of oral tecovirimat before presenting to the emergency department for continual lesion progression. Given his AIDS, the patient was at-risk for poor response to oral treatment due to the potenti

Unusual case of acute oesophageal necrosis.

Acute oesophageal necrosis is a rare clinical entity that commonly affects the distal oesophagus, giving it a characteristic black appearance. It is associated with a high mortality and predominantly impacts critically ill patients. In this case report, we present a male patient in his 50s with multiple comorbidities admitted for management of ketoacidosis. The patient was overall well appearing and remained haemodynamically stable throughout the entirety of his hospital course. Despite this, necrosis was found in the proximal, middle and distal portions of the patient's oesophagus. The presen…

Mitochondrial permeability transition pore-dependent necrosis.

Mitochondrial permeability transition pore (mPTP)-dependent cell death is a form of necrotic cell death that is driven by mitochondrial dysfunction by the opening of the mPTP and is triggered by increases in matrix levels of Ca2+ and reactive oxygen species. This form of cell death has been implicated in ischemic injuries of the heart and brain as well as numerous degenerative diseases in the brain and skeletal muscle. This review focuses on the molecular triggers and regulators of mPTP-dependent necrosis in the context of myocardial ischemia reperfusion injury. Research over the past 50 years…

Swine Inflammation and Necrosis Syndrome (SINS).

Tail biting is a prevalent and undesirable behaviour in pigs and a major source of significant reduction in well-being. However, focusing on biting considers only one part of the solution, because tail damage can be found with a high prevalence without any action by other pigs. The lesions are not limited to the tail but can also be found in the ears, heels, soles, claw coronary bands, teats, navel, vulva, and face. Environmental improvement alone often fails to overcome the problem. This review addresses a new inflammation and necrosis syndrome in swine (SINS). It shows the clinical signs and…

Right Epipericardial Fat Necrosis After Thymectomy.

A 62-year-old male patient underwent median sternotomy and thymectomy for thymoma (World Health Organization classification type B1+B3, Masaoka classification stage II). A 10-month follow-up postoperative computed tomography showed a 22-mm nodule in the anterior mediastinal lipid tissue just above the right diaphragm. The patient was followed for 8 years, and the nodule's capsule eventually became calcified. Considering the possibility of postoperative recurrence of thymoma, we excised the nodule. The pathologic diagnosis was epipericardial fat necrosis. In some cases, fat necrosis is difficul…

Tracheal necrosis following thyroid radiofrequency ablation.

INTRODUCTION: Radiofrequency ablation for benign thyroid nodules has a low rate of complications in experienced hands for selected indications, but tracheal necrosis is a major complication. CASE REPORT: A 60 year-old female patient underwent percutaneous radiofrequency ablation of an unesthetic benign isthmic thyroid nodule. The procedure was performed with a cooled electrode, using the "moving shot" technique on a trans-isthmic approach, under general anesthesia. Postoperative course was complicated by dysphonia and cervical pain implicating a third-degree skin burn of the medial cervical r…

Infected Pancreatic Necrosis Mimicking Pancreatic Cancer.

Although infected pancreatic necrosis can develop as a result of rare conditions involving trauma, surgery, and systemic infection with an uncommon pathogen, it usually occurs as a complication of pancreatitis. Early phase of acute pancreatitis can be either edematous interstitial pancreatitis or necrotizing pancreatitis. The late complications of pancreatitis can be divided into pancreatic pseudocyst due to edematous interstitial pancreatitis or walled-off necrosis due to necrotizing pancreatitis. During any time course of pancreatitis, bacteremia can provoke infection inside or outside the p…
