In a retrospective study at Hospital del Niño in Panama City, Panama, 577 neonates with culture-proved sepsis and/or meningitis were identified during an 18-year period (1975 to 1992). Overall there was an incidence of 3.5 cases/1000 live births. Three hundred thirty-three patients (58%) were of low birth weight (< 2500 g) and 260 (45%) were premature. Gram-negative bacilli, particularly species of Klebsiella and Escherichia coli, were responsible for 61% of infections, whereas Gram-positive isolates (especially staphylococci) and Candida strains accounted for 37 and 2%, respectively. The patt

Antimicrobial resistance is a growing threat in neonatal intensive care units (NICUs) worldwide, challenging the management of neonatal sepsis for decades. The aim of this narrative review is to compare the epidemiology and resistance patterns of neonatal sepsis in NICUs between two periods, 2000-2005 and 2020-2025, and to identify key insights that may inform future practices to limit the emergence and dissemination of antimicrobial resistance in NICUs. During the early 2000s, resistant pathogens, including extended-spectrum beta-lactamases (ESBL)-producing Enterobacterales, vancomycin-resist

INTRODUCTION: Neonatal sepsis is a severe clinical syndrome that continues to be a common and significant health care burden. Knowledge of the local epidemiology allows for a better empirical treatment and improves morbidity and mortality. The aim of this study was to determine the prevalence, epidemiology, and etiology of blood culture-proven sepsis in neonates and to determine whether there are differences between preterm and term newborns.
METHODS: A retrospective study was carried out in a tertiary hospital in Madrid, Spain, during 2021, including 1443 patients at risk of developing sepsi

BACKGROUNDSymptoms of early-onset neonatal sepsis (EOS) in preterm infants are nonspecific and overlap with normal postnatal physiological adaptations and noninfectious pathologies. This clinical uncertainty and the lack of reliable EOS diagnostics results in liberal use of antibiotics in the first days to weeks of life, leading to increased risk of antibiotic-related morbidities in infants who do not have an invasive infection. METHODSTo identify potential biomarkers for EOS in newborn infants, we used unlabeled tandem mass spectrometry proteomics to identify differentially abundant proteins

INTRODUCTION: Neonatal sepsis remains a major contributor to morbidity and mortality worldwide, with the highest burden in low- and middle-income countries (LMICs). Generating accurate estimates of disease burden is critical for setting research priorities, informing health policy, and resource allocation. However, in many LMICs, limited access to timely and reliable diagnostic tools severely limits case detection, undermines epidemiological surveillance, and impedes efforts to improve clinical outcomes.
AREAS COVERED: This review examines the clinical, scientific, and health system implicati

Neonatal sepsis remains a major cause of neonatal deaths globally. Despite advances, accurate and timely diagnosis is hindered by the limited performance of the current clinical approaches, imperfect laboratory biomarkers, and long turnaround time of blood cultures. Artificial intelligence (AI), with its ability to identify patterns and learn continuously (machine learning), seems promising. Basic steps in model development include data filtration, train: test split, feature selection, choosing appropriate algorithms, and evaluating performance using a reference standard. In neonatal sepsis, t

Neonatal sepsis is a major cause of mortality in preterm infants, with Escherichia coli as one of the leading pathogens. Few studies have examined the interplay between virulence factors, resistance profiles, phylogroups, and clinical outcomes in this population. We analyzed 52 E. coli strains isolated from 49 preterm neonates diagnosed with sepsis at a tertiary-level hospital in Mexico. Strains underwent phylogenetic classification, virulence gene profiling, and antimicrobial resistance testing. PFGE was used to assess genetic relatedness and outbreak clusters. Clinical data were correlated w

BACKGROUND: Neonatal sepsis is a deadly disease with non-specific clinical signs, delaying diagnosis and treatment. There remains a need for early biomarkers to facilitate timely intervention. Our objective was to identify neonatal sepsis gene expression biomarkers that could predict sepsis at birth, prior to clinical presentation.
METHODS: Among 720 initially healthy full-term neonates in two hospitals (The Gambia, West Africa), we identified 21 newborns who were later hospitalized for sepsis in the first 28 days of life, split into early-onset sepsis (EOS, onset ≤7 days of life) and late-on

Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) exhibits a spectrum of clinical manifestations, spanning from asymptomatic carriage to fatal outcomes. Among young infants, the incidence of severe disease is notably high. The pathogenesis of multisystem inflammatory syndrome (MIS) in neonates associated with SARS-CoV-2 remains elusive, although post-infective immune dysregulation is posited as a significant contributor. Recent cohorts have highlighted the transplacental transfer of immunoglobulins, potentially exacerbating immune dysregulation due to the co-transfer of inflammatory

The United Nations Inter-Agency Group for Child Mortality Estimation (UNIGME) estimates that every year 2.5 million neonates die in their first month of life, accounting for nearly one-half of deaths in children under 5 years of age. Neonatal sepsis is the third leading cause of neonatal mortality. The worldwide burden of bacterial sepsis is expected to increase in the next decades due to the lack of effective molecular therapies to replace the administration of antibiotics whose efficacy is compromised by the emergence of resistant strains. In addition, prolonged exposure to antibiotics can h

Early-onset sepsis (EOS) remains a substantial contributor to neonatal morbidity and mortality. Continued epidemiologic surveillance of incidence, risk factors, and microbiology is paramount to developing new prevention strategies and optimizing antibiotic administration. Understanding the risks and benefits of maternal antibiotic exposure and neonatal risk assessment can inform clinical management. Maternal vaccination during pregnancy is a promising avenue for EOS prevention, particularly against group B Streptococcus. When EOS is suspected, ampicillin and gentamicin are the appropriate rout

OBJECTIVE: To evaluate trends in organisms causing early-onset neonatal sepsis (EONS). Congruent with recent reports, we hypothesized there would be an increase in EONS caused by Escherichia coli.
STUDY DESIGN: National data on infants admitted to neonatal intensive care units from 2009 to 2014 were compared to previously reported data from 2003 to 2008. We report 430 cases of EONS from 2009 to 2014. Bivariate analyses were used to analyze the distribution of causative organisms over time and differences by gestational age. Linear regression was used to estimate trends in causative organisms.
