Intraocular medulloepithelioma - A review of clinical features, DICER 1 mutation, and management.

Intraocular medulloepithelioma is a nonhereditary neoplasm of childhood arising from primitive medullary epithelium. It most often involves the ciliary body. Most patients present between 2 and 10 years of age with loss of vision, pain, leucocoria, or conjunctival congestion. The mass appears as a grey-white ciliary body lesion with intratumoral cysts. Presence of a neoplastic cyclitic membrane with extension to retrolental region is characteristic. Secondary manifestations like cataract and neovascular glaucoma may be present in up to 50% and 60% patients, respectively. These could be the fir

Extraventricular Neurocytoma in the Left Frontal Lobe: A Case Report and Literature Review.

BACKGROUND: Neurocytoma is a rare brain neoplasm of neuroepithelial origin that occurs predominantly in the ventricular system adjacent to the interventricular foramen and septum pellucidum. However, extraventricular neurocytoma is an extremely rare entity, with poor clinical, radiologic, and histopathological characterization. Here we report a case of an extraventricular parafalcine neurocytoma in the left frontal lobe. We also examine previously reported cases of extraventricular neurocytoma in an attempt to provide an up-to-date summary of the condition. METHODS: A literature search was pe

Dysembryoplastic neuroepithelial tumors: magnetic resonance imaging and magnetic resonance spectroscopy evaluation.

BACKGROUND: Dysembryoplastic neuroepithelial tumor (DNT) is a rare benign neoplasm of the central nervous system affecting young people. A correct preoperative diagnosis is helpful for planning surgical strategies and improving prognosis. The purpose of this study was to characterize DNTs using magnetic resonance imaging (MRI) and magnetic resonance spectroscopy (MRS) and to analyze the value of these two techniques in the diagnosis of DNTs. METHODS: MR images of 13 patients with DNTs were reviewed retrospectively; and five of the patients also underwent MRS. Tumors were confirmed by surgery.

Rosette-forming glioneuronal tumor of the fourth ventricle--case report.

Rosette-forming glioneuronal tumor (RGNT) of the fourth ventricle is a rare tumor included as a novel glioneuronal neoplasm in the 2007 World Health Organization classification of brain tumors. A 15-year-old female patient presented with RGNT of the fourth ventricle manifesting as headache persisting for one month. Magnetic resonance imaging disclosed a nonenhanced solid mass in the fourth ventricle and vermis. The tumor was gross totally resected. Histological examination revealed biphasic neurocytic and glial architecture. The neurocytic component consisted of uniform neurocytes forming neur

Neuroepithelial tumors of the central nervous system with EWSR1::PATZ1 fusion: a case report and literature review.

Neuroepithelial tumors (NEpT) harboring EWSR::PATZ1 fusions remain an enigma. Initially described in sarcomas, these tumors display remarkable histomorphological diversity and unpredictable clinical behavior based on histologic or molecular features, with no established management protocols. To date, this subgroup of neoplasms has not been acknowledged as a sui generis entity by the WHO classification system, and it is currently designated as 'NEC'/'NOS'. This retrospective case series describes two young adults (32-35 years old) without cancer predisposition or risk factors, diagnosed with EW

Differentiating ictal panic with low-grade temporal lobe tumors from psychogenic panic attacks.

OBJECTIVE: Indolent low-grade temporal lobe tumors may present with ictal panic that may be difficult to differentiate from psychogenic panic attacks. The current study aims to demonstrate the differences between the two disorders and help physicians generate a diagnostic paradigm. METHOD: This was a retrospective study of 43 patients who underwent a temporal lobectomy between 1981 and 2008 for the treatment of intractable temporal lobe epilepsy secondary to low-grade neoplasms at Rush University Medical Center. A total of 10 patients in this group presented with ictal panic who were previous

Outcome of adult patients with temporal lobe tumours and medically refractory focal epilepsy.

BACKGROUND: Tumours are a well-recognized cause of medically intractable epilepsies. Tumours represent the primary pathology in 10%-30% of patients undergoing surgical treatment for chronic epilepsy. This study examines the surgical and epileptological outcome of adults with temporal lobe tumoural epilepsy treated within a comprehensive epilepsy surgery programme. METHODS: Between 1999 and 2004, 99 consecutive patients have been operated for intractable temporal lobe epilepsy (TLE). Among these, 14 adult patients exhibited temporal lobe neoplasms associated with TLE. Every patient underwent a

Immunohistochemical studies on gastrin-releasing peptide- and adrenocorticotropic hormone-containing cells in the human lung.

Bronchial endocrine cells containing gastrin-releasing peptide (GRP), a mammalian analog of bombesin, and adrenocorticotropic hormone (ACTH) were immunohistochemically localized in paraffin sections of normal and pathologic human lungs. GRP-containing cells were present in fetal bronchi at the 12th gestational week and in "neuroepithelial bodies" about the time of delivery. In normal adult lungs, a few isolated GRP-containing cells were present in bronchial and bronchiolar mucosa. In bronchiectatic or fibrotic lungs, small clusters of GRP-containing cells were occasionally noted in basal bronc

Leptomeningeal glioblastoma presenting with multiple cranial neuropathies and confusion.

Glioblastoma multiforme (GBM) is the commonest primary malignant neoplasm of the CNS. Usually, patients present with seizures and headache but in the elderly, confusion and generalised cognitive decline are more frequently the initial features. Multiple cranial nerve lesions as a manifestation of leptomeningeal meningitis is a rare presentation of GBM. The diagnosis is not often suggestive on either brain computed tomography (CT) or magnetic resonance imaging (MRI) and is usually confirmed by cerebrospinal fluid (CSF) cytology or histology. We describe the case of an 80-year-old man, who prese

A case of recurrent epilepsy-associated rosette-forming glioneuronal tumor with anaplastic transformation in the absence of therapy.

Rosette-forming glioneuronal tumor (RGNT) most commonly occurs adjacent to the fourth ventricle and therefore rarely presents with epilepsy. Recent reports describe RGNT occurrence in other anatomical locations with considerable morphologic and genetic overlap with the epilepsy-associated dysembryoplastic neuroepithelial tumor (DNET). Examples of RGNT or DNET with anaplastic change are rare, and typically occur in the setting of radiation treatment. We present the case of a 5-year-old girl with seizures, who underwent near total resection of a cystic temporal lobe lesion. Pathology showed morp

[MRI-characteristics of epileptogenic supratentorial brain tumors in children].

OBJECTIVE: To analyze MR-images in patients with symptomatic epilepsy associated with the brain tumor. MATERIAL AND METHODS: MRI results of 52 patients with symptomatic epilepsy operated for tumors of supratentorial localization were analyzed. The most epileptogenic tumors with atypical MRI signs and subtle clinical presentation were identified. All patients with tumors were operated using different methods of surgical intervention. RESULTS: Dysembryoplastic neuroepithelial tumors (DNET), diffuse astrocytomas (DA) and gangliogliomas (GG) were the most frequent epileptogenic tumors. In all th

Lack of IDH1 mutation in astroblastomas suggests putative origin from ependymoglial cells?

Astroblastomas are extremely rare neuroepithelial tumors of uncertain histogenesis, affecting children and young adults, and constitute a new addition to the WHO 2000 classification of CNS tumors. We report the largest series of nine cases diagnosed in a single institute over the last 13 years and review published literature. Mean age at presentation was 12.8 years (range: 22 months to 27years). Seven out of nine cases were supratentorial (frontal/frontoparietal - three, parieto-occipital - three, parafalcine - one), one was intraventricular and another was optochaismatic/suprasellar. Five cas

[Algorithm identification of differential diagnostic clinical syndromes in patients with neuroepithelial tumors of the III ventricle].

An algorithm for identifying differential diagnostic clinical syndromes in neurooncologic patients was suggested. This technique involves: creating a database of cases - lists of symptoms, requests for selection of thematic cases with their subsequent division into the main and control groups; generation of single-symptom applicants; selection and verification of clinical differential diagnostic syndromes appropriate to sensitivity and specificity using a 4-dipole table and selection of the best of them. Based on the database of 368 patients with tumors of basal-diencephalic localization, we t

Pineal parenchymal tumors: an ultrastructural study with prognostic implications.

The pineal gland is host to a spectrum of neoplasms. Those considered to be derived from or differentiating toward pineal parenchymal cells are rare. Traditionally, pineal parenchymal tumors (PPTs) have been divided into 3 types: pineocytomas, pineoblastomas, and mixed or transitional tumors. Their characterization has been far from adequate and no firm diagnostic criteria, light microscopic or ultrastructural, have been established. In an attempt to provide more precise prognostic diagnostic criteria, we undertook a detailed ultrastructural analysis of 17 PPTs and found them to exhibit light

Co-Occurrence of Dysembryoplastic Neuroepithelial Tumor and Hodgkin Lymphoma in a Patient with Noonan Syndrome and Mutation in the PTPN11 Gene.

INTRODUCTION: Noonan syndrome (NS) is a genetic disorder which belongs to the RASopathy family, characterized by craniofacial dysmorphisms, short stature, congenital heart defects, and an increased predisposition to malignancies. Although hematologic malignancies, neuroblastomas, and certain solid tumors have been documented in NS, the co-occurrence of dysembryoplastic neuroepithelial tumor (DNET) and Hodgkin lymphoma has not been previously reported in the literature. CASE PRESENTATION: We present the case of an 11-year-old boy with NS caused by a pathogenic variant in the PTPN11 gene who de

Dysembryoplastic neuroepithelial tumor with rapid recurrence of pilocytic astrocytoma component.

Dysembryoplastic neuroepithelial tumor (DNT) rarely has aggressive behavior with recurrence. The authors describe a case of DNT with rapid re-growth and features of pilocytic astrocytoma. A 19-year-old girl presented with a 3-year history of complex partial seizure, which was presumably because of DNT located in the left parietal lobe. The tumor was resected, although a tiny residual lesion with cystic space enlarged with ring enhancement identified on MRI 14 days after surgery resulted in re-excision. The recurrent tumor was eventually proved to be predominantly composed of pilocytic astrocyt
