P.J. Desault and the birth of nephrology (between 1785 and 1795).

Desault's genius and methods opened up a field: the medical diseases of the urinary tract. Though he was a surgeon and therefore an anatomic … rvived this physiological eruption into renal disorders would have been a definitive step of modern nephrology. From a historical point of view, one could suspect that Bichat shifted from anatomy and surgery to medicine and physiology after having posthumously compiled and edited the Desault's book, "Traité des Maladies des voies urinaires".

[Pierre J. Desault and the birth of nephrology 1785-1795].

Desault's genius and methods opened up a new field, the pathology of those medical conditions which afflict the urinary apparatus. Though he … this physiological eruption into renal disorders would have been the first decisive step of modern nephrology. His message, though excellent, was delivered too soon and on barren soil, to poorly educated physicians who paid little attention. But then, one has to remember that Desault was just a surgeon!

[Effect of praziquantel on the uro-nephrologic complications of urinary bilharziasis].

This study shows the effect of praziquantel as a 12 month treatment on the uro-nephrological consequences of Schistosoma haematobium chronic infection. This was done in a hyperendemic setting in the middle west of Madagascar. 435 people with ova in their urine filtration test were followed up with clinical examination, ultrasonography, urinary sticks and creatininemia. The prevalence of macroscopic hematuria decreased significantly from 32.5% (153 patients) to 4.3% (20 patients) (p < 0.05). Other abnormalities decreased but not significantly. The prevalence of proteinuria decreased from 62.3%

[The prenatal development of nephrological abnormalities detected during pregnancy].

OBJECTIVE: The objective of the present study was to evaluate the conformity between the pathological prenatal ultrasound findings of the kidney and urinary tract during fetal life and in the neonatal period. PATIENTS AND METHODS: In a period of four years (1991-1994) 14,938 pregnant woman were controlled in our hospital by performing at least one prenatal ultrasound during the third trimester of their pregnancy to detect alterations of the kidneys and fetal urinary tract. In those newborns with a positive fetal ultrasound, a renal ultrasonography study was performed in the first week of life

[Benefits of early nephrological management in chronic renal failure].

OBJECTIVES: We evaluated whether early nephrological referral of patients with chronic renal failure (CRF) resulted in improved condition of patients at initiation of maintenance dialysis and in better outcome on dialysis. PATIENTS AND METHODS: We prospectively recorded clinical status, laboratory parameters, length of hospital stay and outcome of 900 CRF patients who started maintenance dialysis at Necker hospital between January 1989 and December 1996. We compared patients who benefited regular nephrological follow-up, and patients who were referred in emergency conditions at the ultimate s

[Clinical experiences with Wegener's granulomatosis from the nephrologic viewpoint].

Immunosuppressive treatment with cyclophosphamide and prednisolone has markedly improved the prognosis of Wegener's granulomatosis (WG). In patients with WG the detection of anti-neutrophil-cytoplasmic autoantibodies (ANCA or ACPA) has become an important parameter to confirm or even routinely establish the diagnosis of WG over the past few years. From 1985 to 1990 we observed and treated 12 patients (5 males and 7 females) aged 14-64 years (mean 41.1 years) with WG. In the same time span an analysis of 35 patients with rapidly progressive glomerulonephritis showed that the octiology was idiop

Trends in nephrology: from "supportive care" to CKD combination therapy.

Chronic kidney disease (CKD) management has evolved from supportive care with renin-angiotensin system inhibitors (RASi) alone to multi-target combination therapies. While RASi remain foundational, their limited efficacy in fully preventing disease progression (e.g. residual albuminuria) and safety concerns, such as hyperkalemia, have underscored the need for novel therapeutic agents. Sodium-glucose cotransporter-2 inhibitors (SGLT2i), glucagon-like peptide-1 receptor agonists (GLP-1RA) and nonsteroidal mineralocorticoid receptor antagonists (nsMRA) demonstrate complementary nephroprotective e

Advocacy without Borders: A Collaborative Journey in Nephrology Nursing Leadership.

Advocacy is an essential component of professional nursing practice, yet opportunities to experience advocacy across international health care systems are uncommon. This article describes a collaborative journey between leaders of the American Nephrology Nurses Association (ANNA) and the Canadian Association of Nephrology Nurses and Technologists (CANNT), beginning with a conversation at the 2025 CANNT Conference and concluding in participation at ANNA's 2026 Advocacy Forum in Washington, D.C. Through shared experiences, the authors explore similarities and differences in nurse advocacy within…

Modeling Fabry nephropathy: insights from experimental systems towards precision nephrology.

Fabry nephropathy is a major cause of kidney failure in Fabry disease, caused by pathogenic GLA variants that reduce α-galactosidase A activity and lead to globotriaosylceramide (Gb3) accumulation. In recent years, rapid advances in experimental modeling and emerging therapeutic strategies have dramatically reshaped understanding and treatment prospects for Fabry nephropathy. Experimental models - including cellular systems, patient-derived kidney organoids, and animal models such as zebrafish, Drosophila, mice and rats - have been pivotal in elucidating classic Gb3-dependent and novel Gb3-ind

Lessons from implementation of a nephrology key performance indicator programme.

BACKGROUND: We review the impact of a nephrology key performance indicator (KPI) program implemented over a ten-year period in Victoria, Australia. The program enabled transparent comparison of performance across nephrology units, however improvement against performance targets was not consistent and better understanding of KPIs is required to promote quality improvement. AIM: We aimed to review the impact of the Victorian nephrology KPI programme over a 10-year period. METHODS: The KPI programme involved six KPIs related to the continuum of care for kidney replacement therapy: pre-dialysis …

Lung ultrasound in nephrology: Basics, applications, limitations, and future directions.

Point-of-care ultrasonography (POCUS) is increasingly recognized as a valuable extension of the physical exam, offering real-time bedside insights to support clinical decision-making. In nephrology, lung ultrasound (LUS) is gaining prominence for its ability to assess extravascular lung water and guide fluid management, especially in patients with end-stage renal disease. This narrative review highlights current applications, technical aspects, and limitations of LUS in nephrology. Studies such as the Lung Water by Ultrasound-Guided Treatment in Haemodialysis Patients trial indicate that LUS-g…

Artificial intelligence in nephrology: predicting CKD progression and personalizing treatment.

Chronic kidney disease (CKD) represents a major and expanding global health challenge, with prevalence rising due to aging populations, diabetes, hypertension, and environmental factors. Conventional risk assessment tools such as the CKD Epidemiology Collaboration equation and the Kidney Failure Risk Equation are limited in precision, generalizability, and their ability to identify rapid progressors in early stages. This review examines the transformative role of artificial intelligence (AI), encompassing machine learning, deep learning, natural language processing, and multimodal data integra

Endoplasmic reticulum stress and monogenic kidney diseases in precision nephrology.

The advent of next-generation sequencing (NGS) in recent years has led to a rapid discovery of novel or rare genetic variants in human kidney cell genes, which is transforming the risk assessment, diagnosis, and treatment of kidney disease. Mutations may lead to protein misfolding, disruption of protein trafficking, and endoplasmic reticulum (ER) retention. An imbalance between the load of misfolded proteins and the folding capacity of the ER causes ER stress and unfolded protein response. Mutations in nephrin (NPHS1), podocin (NPHS2), laminin β2 (LAMB2), and α-actinin-4 (ACTN4) have been show

Nephrology Update: End-Stage Renal Disease and Renal Replacement Therapy.

End-stage renal disease (ESRD) is associated with high rates of morbidity and mortality, and increased health care use. Optimal management of patients with ESRD requires close collaboration among primary care physicians, nephrology subspecialists, and other subspecialists. Critical issues for the family physician include helping patients transition from chronic kidney disease to ESRD, recognizing and managing common issues in patients receiving dialysis or after kidney transplantation, and understanding palliative care for patients with ESRD. Dialysis typically is initiated for patients with a…

A new initiative in nephrology: 'Kidney disease: improving global outcomes'.

The burden of kidney disease: Improving global outcomes. Chronic kidney disease (CKD) is a worldwide public health problem with an increasing incidence and prevalence of patients requiring replacement therapy. There is an even higher prevalence of patients in earlier stages of CKD, with adverse outcomes such as kidney failure, cardiovascular disease, and premature death. Patients at earlier stages of CKD can be detected through laboratory testing and their treatment is effective in slowing the progression to kidney failure and reducing cardiovascular events. The evidence-based care of these pa
