Current State of Brazilian Neurosurgery: Evaluation of Burden of Care and Case Volume.

BACKGROUND: Low- and middle-income countries (LMICs) bear a heavier burden of neurosurgical diseases than high-income countries. Brazil, a growing middle-income country, holds promise as a health care market. However, there exists a lack of information to characterize the state of neurosurgical practice and access to care in Brazil. This study aimed to characterize neurosurgical practice in Brazil and identify barriers to care. METHODS: A collaborative survey was developed with Brazilian neurosurgeons and distributed by the Brazilian Society of Neurosurgery. The survey gathered demographic in…

A model for global surgical training and capacity development: the Children's of Alabama-Viet Nam pediatric neurosurgery partnership.

INTRODUCTION: Training capable and competent neurosurgeons to work in underserved regions of the world is an essential component of building global neurosurgical capacity. One strategy for achieving this goal is establishing longitudinal partnerships between institutions in low- and middle-income countries (LMICs) and their counterparts in high-income countries (HICs) utilizing a multi-component model. We describe the initial experience of the Children's of Alabama (COA) Global Surgery Program partnership with multiple Vietnamese neurosurgical centers. METHODS: The training model developed by…

Arne Torkildsen and the ventriculocisternal shunt: the first clinically successful shunt for hydrocephalus.

Arne Torkildsen was a pioneering Norwegian neurosurgeon who introduced the ventriculocisternal shunt, the first clinically successful shunt for CSF diversion in hydrocephalus. The procedure, usually referred to as ventriculocisternostomy (VCS), Torkildsen's operation, orTorkildsen's shunt, became internationally recognized as an efficient operation for the treatment of noncommunicating hydrocephalus. The operation gained widespread use in the 1940s and 1950s before the introduction of extracranial shunts. In this paper, the authors look more closely at Torkildsen's development of the VCS and e

Grief in the circle of neurosurgery: observing through the lens of science, spirituality, and philosophy.

The experience of grief within neurosurgery extends across a diverse range of individuals, including patients, but also caregivers, neurosurgeons, trainees, advanced practice providers, and the nursing staff who care for neurosurgical patients. This congregation of individuals constitutes the sacred thread woven in the circle of neurosurgery. This interconnected circle is united by a string of life-changing experiences, most of which are profoundly rewarding. However, some experiences leave a deep, regretful mark on the "self," even when the highest standards are upheld in the operating room. …

Meges of Sidon: The "Most Learned" of Surgery's Founding Fathers.

This paper aims to bring back to life an underrated, even forgotten surgeon of the late first century B.C.E., Meges of Sidon. He was an experienced surgeon of his time and was considered the most erudite Roman surgeon before Galen. He belonged to the Alexandria School of Medicine and later migrated to Rome to practice. Although most of his work did not survive, he was mentioned by notable ancient figures, such as Celsus and Galen. He excelled in various surgical specialties, not limited to neurosurgery, orthopedics, ophthalmology, and urology. Galen cited Meges in his surgical book on head inj

The future of neurosurgery: a white paper on the recruitment and retention of women in neurosurgery.

PREFACE: The leadership of Women in Neurosurgery (WINS) has been asked by the Board of Directors of the American Association of Neurological Surgeons (AANS) to compose a white paper on the recruitment and retention of female neurosurgical residents and practitioners. INTRODUCTION: Neurosurgery must attract the best and the brightest. Women now constitute a larger percentage of medical school classes than men, representing approximately 60% of each graduating medical school class. Neurosurgery is facing a potential crisis in the US workforce pipeline, with the number of neurosurgeons in the US…

Intraorbital optic nerve coloboma: Neurosurgical considerations from two rare cases.

BACKGROUND: Coloboma or colobomatous cyst of the optic nerve coloboma (ONC) is a rare congenital anomaly commonly associated with microphthalmia or anophthalmia. Neurosurgeons usually treat orbital lesions surgically; however, in some centers, neuro-ophthalmologists and other surgical specialties may also be involved in these procedures. Given this context, neurosurgeons must be familiar with various intraorbital lesions, including ONC. Here, emphasizing the clinical, pathological, and radiological aspects, we report two uncommon pediatric cases of ONC to enhance neurosurgeons' awareness and r…

Systematic reviews and meta-analyses in neurosurgery part I: interpreting and critically appraising as a guide for clinical practice.

Neurosurgeons are inundated with the Herculean task to keep abreast with the rapid pace at which clinical research is proliferating. Systematic reviews and meta-analyses (SRMAs) have consequently surged in popularity because when executed properly, they constitute the highest level of evidence, and may save busy neurosurgeons many hours of combing the literature. Well-executed SRMAs may prove instructive for clinical practice, but poorly conducted reviews sow confusion and may potentially cause harm. Unfortunately, many SRMAs within neurosurgery are relatively lackluster in methodological rigo…
