Treat-to-target in spondyloarthritis: implications for clinical trial designs.

Spondyloarthritis (SpA) is a chronic inflammatory disease involving the spine and peripheral joints, and extra-articular manifestations such as uveitis, psoriasis and bowel inflammation. The treatment goals for SpA are maintenance of physical function, control of disease activity and prevention of radiographic progression. However, unlike the well-established treat-to-target (T2T) guidance in rheumatoid arthritis, the T2T concept for treating SpA is still immature. Clinical evidence of T2T in SpA is still lacking. To develop evidence of T2T in SpA, several research agendas need to be accomplis

The structural, functional and electrophysiological assessment of paraspinal musculature of patients with ankylosing spondylitis and non-radiographic axial spondyloarthropathy.

Paravertebral muscles are affected in spondyloarthritis. Decreased mobility of spine may lead to atrophy and fatty degeneration of these muscles. The objective of this study was to compare the sonographic, electrophysiological and magnetic resonance imaging (MRI) features of paraspinal muscles between patients with ankylosing spondylitis (AS) and non-radiographic axial spondyloarthritis (nr-axSpA). The patients who were diagnosed as AS with modified New York criteria and those as nr-axSpA with ASAS 2009 criteria were enrolled. Clinical evaluation, electrophysiological examination including ner…

Advancements in Imaging Techniques for Early Diagnosis and Management of Axial Spondyloarthritis.

PURPOSE OF REVIEW: We aimed to introduce recent finding of imaging studies used in axial spondyloarthritis (axSpA). RECENT FINDINGS: Using low-dose whole spine CT (CT syndesmophyte score [CTSS]) improved diagnostic accuracy for evaluating spinal structural progression than previous method (modified Stoke Ankylosing Spondylitis Spinal Score [mSASSS]) in axSpA. The novel definition of positive finding of sacroiliac joint (SIJ) and spine magnetic resonance imaging (MRI) enabled to diagnose axSpA earlier than plain radiography. In addition, novel MRI protocol such as volumetric interpolated breat

Genetic and Molecular Distinctions Between Axial Psoriatic Arthritis and Radiographic Axial Spondyloarthritis: Post Hoc Analyses from Four Phase 3 Clinical Trials.

INTRODUCTION: Emerging evidence suggests psoriatic arthritis (PsA) with axial involvement (axPsA) and radiographic axial spondyloarthritis (r-axSpA) may possibly represent distinct disorders, with some differing clinical manifestations, genetic associations, and radiographic findings. Moreover, axPsA and r-axSpA may respond differently to therapies: guselkumab (interleukin [IL]-23p19 subunit inhibitor [i]) and ustekinumab (IL-12/23p40i) demonstrated improvements in axial symptoms in patients with PsA; however, neither risankizumab (IL-23p19i) nor ustekinumab demonstrated efficacy versus placeb

Do we need new trial designs in spondyloarthritis?

In the last two decades drug trials in ankylosing spondylitis (AS) have been extremely successful and many effective new drugs have been approved for the treatment of patients with AS. In 2009, new classification criteria for axial spondyloarthritis (axSpA) have been released that capture not only AS, but also patients with presumably earlier non-radiographic axSpA. These criteria have served as inclusion criteria for patients to be enrolled in drug trials for the indication of nr-axSpA. The theme to be discussed in this article pertains to changes in the design of clinical trials required to

Magnetic Resonance Imaging in Axial Spondyloarthritis: Beyond the Sacroiliac Joints-Focus on Facet Joint Involvement.

Axial spondyloarthritis (axSpA) is a chronic inflammatory disease of the sacroiliac joints (SIJs) and spine, encompassing non-radiographic axSpA (nr-axSpA) and radiographic axSpA (r-axSpA) or ankylosing spondylitis (AS). Early diagnosis is challenging, particularly in nr-axSpA, due to the absence of radiographic sacroiliitis and overlap with nonspecific back pain. Imaging plays a pivotal role in disease classification and monitoring. To synthesize current evidence on inflammatory and structural facet joint involvement in axSpA, with particular emphasis on MRI characterization, available scorin

Axial Spondyloarthritis: Evolving concepts regarding the disease's diagnosis and treatment.

Spondyloarthritis (SpA) is a chronic inflammatory disease that affects the axial skeleton (axSpA) and/or the peripheral joints (p-SpA) and entheses. The natural history of SpA in the decades of the 80 and 90 s involved a progressive disease with pain, spinal stiffness, ankylosis of the axial skeleton, structural damage of peripheral joints, and a poor prognosis. In the last 20 years, enormous advances in understanding and managing SpA have occurred. With the introduction of the ASAS classification criteria and MRI, early disease recognition is now possible. The ASAS criteria widened the spectr

Developments in therapies for spondyloarthritis.

First-line therapy for spondyloarthritis (SpA) has not yet altered in the wake of new classification criteria; NSAIDs and physical therapy are recommended. Anti-TNF agents can be used when NSAIDs fail, but their efficacy has potentially been limited in previous trials by inclusion criteria requiring the presence of established, active disease. Now, not only patients with axial SpA (axSpA) with radiographic signs of sacroiliitis (that is, with ankylosing spondylitis), but also patients in whom structural damage is not-yet-visible radiographically (non-radiographic axSpA) can be included in tria

[What is ascertained in the therapy of axial spondyloarthritis?].

The new term axial spondyloarthritis (axSpA) includes classic ankylosing spondylitis and non-radiographic (nr-) axSpA. The definition was introduced in 2009 as part of the Assessment of SpondyloArthritis international Society (ASAS) classification criteria for axSpA, where-apart from chronic back pain starting before the age of 45 years and the presence of HLA-B27-magnetic resonance imaging demonstrated bone marrow edema of the sacroiliac joints (osteomyelitis) or structural changes on x-rays may play an important role. These criteria can also be used for diagnosis. A major advantage of the ne

Background: Biologics and small-molecule drugs have become increasingly accepted worldwide in the treatment of axial spondyloarthritis (axSpA), including ankylosing spondylitis (AS) and non-radiographic axial spondyloarthritis (nr-axSpA). However, a quantitative multiple comparison of their efficacy and safety is lacking. This study aims to provide an integrated assessment of the relative benefits and safety profiles of these drugs in axSpA treatment. Methods: We included randomized clinical trials that compared biologics and small-molecule drugs in the treatment of axSpA patients. The primary…
