Trauma and wound healing: role of the route of nutritional support.

Following severe trauma and surgical injury, nutritional support via the enteral route has been shown to lead to increased survival and decreased complications when compared to the parenteral route. We hypothesized that the route of nutrient delivery may affect cutaneous wound healing following severe traumatic insult. Forty-six Sprague Dawley rats underwent bilateral closed femoral fractures, central venous catheterization, gastrostomy placement and dorsal skin incision with placement of polyvinyl alcohol sponges into subcutaneous pockets. Identical nutritional infusates of 25% dextrose, 4.25…

Adding food for thought. Structuring nutritional support for elderly trauma patients.

Of all elderly patients admitted to the acute setting, orthopaedic patients have been shown to be most severely undernourished or malnourished. The delivery of nutrition as part of patient care is undervalued and underdeveloped. Creating strategies within which effective treatment can be achieved is vital to its efficient delivery. Effective management of nutrition for those at risk will speed recovery, reduce the incidence of secondary complications and mortality and reduce nursing workload in both hospitals and community settings.

Improved nasogastric intubation technique for administration of nutritional support in dogs.

Established procedures for nasogastric intubation are difficult to perform in dogs because of anatomic variation of nasal passages and turbinate structures. Twenty-five clinically normal dogs were used to improve and refine the technique of nasogastric tube placement, and 13 clinical patients at our teaching hospital were evaluated for malnutrition and were considered candidates for nutritional support by nasogastric tube feeding. An improved method for the placement of nasogastric tubes in dogs was used. By pushing the external nares dorsally while advancing the tube in a caudoventral, medial…

Malnutrition and the role of nutritional support in alcoholic liver disease.

Malnutrition adversely effects hepatic function and structure. In certain instances, histological changes indistinguishable from alcoholic liver disease may be seen. Alcohol is clearly a direct hepatic toxin and its results are not prevented by normal nutritional intake. The majority of patients admitted to the hospital with alcoholic liver disease are moderately to severely malnourished but find it difficult or impossible to resume an intake sufficient to repair and maintain a normal nutritional state. Malnutrition may be considered a complication of alcohol abuse which occurs simultaneously

The metabolic response and problems with nutritional support in acute tetanus.

Thirteen patients with severe tetanus were studied in the first and second week of illness in order to assess metabolic changes and nutritio … itable in such patients unless the metabolic response can be suppressed or more aggressive forms of nutritional support (eg, total parenteral nutrition including sufficient insulin to maintain normoglycemia) are employed.

Energy and tissue metabolism in patients with cancer during nutritional support.

The present study evaluates the energy and skeletal muscle metabolism in malnourished patients, with and without cancer, in response to nutrition. The energy balance was positive in all patients receiving nutritional support. This led to an increase in body weight and body potassium levels. Glucose turnover increased in all patients. In patients with cancer, elevated glucose turnover reflected increased utilization of glucose preferentially for synthetic pathways rather than for oxidation. Protein synthesis and RNA content in skeletal muscles increased during nutrition. Nutritional support imp…

Nutritional support for pressure injury: a bibliometric analysis of research trends.

INTRODUCTION: Pressure injury (PI) is a common complication closely associated with malnutrition. Nutritional support is critical for PI prevention and healing, but the overall research trends in this field remain unclear. PURPOSE: This bibliometric analysis aimed to explore trends in research on nutritional support for Pressure Injury (PI) patients, providing insights for healthcare professionals and researchers. METHODS: This study is a bibliometric analysis. Relevant English articles were retrieved from the Web of Science Core Collection (WOSCC) and PubMed databases, with the search perio…

[Nutritional support in cancer patients - Perceptions of healthcare professionals and patients].

Introduction: malnutrition in patients with cancer negatively impacts their quality of life, treatment response, survival, and other clinical outcomes. Despite its importance, the access to specialized nutritional support remains limited in clinical practice. Objective: to analyse the perceptions and practices of healthcare professionals and patients regarding nutritional support in oncology in Spain. A secondary objective was to identify the main barriers and strengths in its implementation. Methods: an observational study was conducted using a structured survey, targeting healthcare professi…

Nutritional Support for Moderate-to-Late-Preterm Infants - A Randomized Trial.

BACKGROUND: Most moderate-to-late-preterm infants need nutritional support until they are feeding exclusively on their mother's breast milk. Evidence to guide nutrition strategies for these infants is lacking. METHODS: We conducted a multicenter, factorial, randomized trial involving infants born at 32 weeks 0 days' to 35 weeks 6 days' gestation who had intravenous access and whose mothers intended to breast-feed. Each infant was assigned to three interventions or their comparators: intravenous amino acid solution (parenteral nutrition) or dextrose solution until full feeding with milk was es…

Isocaloric nutritional support reduces ventilator duration time in major trauma patients.

AIMS: Major trauma patients need adequate nutrition for recovery. This study aimed to evaluate the adequacy of nutritional supply and the correlation between nutritional supply and clinical outcome. METHODS: A single-centre retrospective observational study was undertaken, describing the amounts of energy and proteins provided to 320 critically ill trauma patients during the first 10 days after admission. The data were collected from the electronic medical records of patients admitted to the trauma intensive care unit during the study period and descriptive statistical analyses were performed

Nutritional Support in Lung Cancer Patients: The State of the Art.

Lung cancer (LC) represents the most commonly diagnosed neoplasm worldwide for both sexes and is the leading cause of cancer mortality. Malnutrition is a comorbidity frequently found in neoplastic patients, but it remains often underestimated and thus undertreated. In this review, we aimed to investigate the incidence of malnutrition among LC patients according to different screening and assessment tools, to evaluate the impact of weight loss and body composition on survival, and to analyze the efficacy of different nutritional interventions in this setting. Although malnutrition, weight loss,

Nutritional Support in Head and Neck Radiotherapy Patients Considering HPV Status.

Malnutrition is a common problem in patients with head and neck cancer (HNC), including oropharyngeal cancer (OPC). It is caused by insuffic … ssibility of eating through the mouth in 20-30% of patients. These patients usually require enteral nutritional support. Feeding tubes are a commonly used nutritional intervention during radiotherapy, most frequently percutaneous gastrostomy tube. Recently, a novel HPV-related type of OPC has been described. Patients with HPV-associated OPC are different from the HPV …

OBJECTIVE: We evaluated the efficacy of muscle-targeted nutritional support on the functional outcomes of multidisciplinary intensive rehabilitation treatment (MIRT) in patients with Parkinson disease (PD) or parkinsonism. METHODS: We conducted a pragmatic, bicentric, randomized (1:1), assessor-blind controlled trial (Protein, Leucine and Vitamin D Enhancing Rehabilitation [PRO-LEADER]; April 2017 to January 2018) in cognitively intact patients with PD or parkinsonism and undergoing a 30-day MIRT. Metabolic and nutritional support of critically ill patients: consensus and controversies.

The results of recent large-scale clinical trials have led us to review our understanding of the metabolic response to stress and the most appropriate means of managing nutrition in critically ill patients. This review presents an update in this field, identifying and discussing a number of areas for which consensus has been reached and others where controversy remains and presenting areas for future research. We discuss optimal calorie and protein intake, the incidence and management of re-feeding syndrome, the role of gastric residual volume monitoring, the place of supplemental parenteral n
