BACKGROUND/PURPOSE: Betel quid (BQ) chewing is popular in Taiwan and many other countries. There are about 200-600 million BQ chewers in the world. BQ chewing is one major risk factor of oral cancer and oral submucous fibrosis (OSF). While areca nut (AN), a main component of BQ, exhibits genotoxicity, its transformation capacity and its role in the initiation and promotion stages of carcinogenesis are not fully clear.

A case of fatal nut anaphylaxis in a 16-year-old atopic, steroid-dependent asthmatic male is reviewed. He had large positive skin tests (scratch) to walnuts and peanuts. Serum was positive for specific IgE antibody measured by solid-phase radioimmunoassay to pecans. The importance of the immediate use of epinephrine is emphasized. Accidental ingestion of foods to which patients are sensitive is a common error for which patients must be prepared with preloaded epinephrine syringes.

Areca nut (AN), commonly known as betel nut, has been used for centuries in ceremonies and rituals, consumed for unproven therapeutic effects, and abused as a recreational psychostimulant. Nuclear protein in testis gene rearrangement-associated midline carcinoma (NUT carcinoma) is a rare and highly invasive tumor. Here, we are reporting the image findings of a 38-year-old woman with lung NUT carcinoma. To evaluate the bone metastases, she underwent 68 Ga-labeled DOTA-ibandronic acid ( 68 Ga-DOTA-IBA) PET/CT examination. Betel nut chewing was associated with the alteration of human oral microbiome and with diseases such as oral cancer and periodontitis, but only in populations of Asian cultural background. We report a young pregnant woman with large midline thoracic mass and markedly elevated serum alpha-fetoprotein (AFP) levels. Initially suspected as a germ cell tumour (GCT) due to age, site, and high AFP levels, a biopsy unveiled a high-grade malignant tumour characterised by undifferentiated monotonous cells. Although tumour cells exhibited positive AFP, the overall immunoprofile did not provide additional evidence to support GCT. Further work-up showed positive for NUT (nuclear protein in testis) immunostaining and the presence of BRD4-NUT1 fusion, confirming the diagnosis of NUT carcinoma.
