We report eye movement findings in 30 patients with latent nystagmus and who were found to have a variety of associated oculomotor disorders. Latent nystagmus is defined clinically as nystagmus which appears on covering one eye and beats towards the uncovered eye. Recordings showed that the latent nystagmus in 28 patients had slow phases with linear or exponentially decreasing velocity. This nystagmus is termed 'LN'. In 13 of these patients certain manoeuvres (e.g. pursuit) provoked nystagmus with exponentially increasing slow phase velocities characteristic of the congenital form of nystagmus

CONCLUSIONS: The presence of orthotropic nystagmus (ON), in multiple positions, appears to predict the success of canalith repositioning maneuvers (CRMs). In this sense CRMs under video-Frenzel are informative for the immediate follow-up, although not related to the degree of disability or emotional distress. Still, attention should be given to psychological assistance even in cases of clinically cured benign paroxysmal positional vertigo (BPPV).
OBJECTIVE: The aim of the study was to highlight the prognostic value of nystagmus in CRMs for BPPV and its relevance for emotional and patient-perc

OBJECTIVE: To verify if the superior semicircular canal (SCC) can be stimulated using the modified "bilateral simultaneous caloric test" (BSCT).
STUDY DESIGN: Prospective study in a tertiary care center.
METHODS: We performed a simultaneous irrigation of both ears with cold water at 24°C. Simultaneous irrigation of both ears inhibits the response of the horizontal SSC and avoids vegetative reaction. Modified BSCT was completed on 12 healthy individuals with no previous otologic history and on 12 patients with unilateral superior SCC obliteration for dehiscence syndrome using a middle fossa a

OBJECTIVE: The study explored the hypothesis that spontaneous nystagmus (Ny) in migraine patients can be triggered or modulated by painful trigeminal stimulation, providing evidence of a functional connection between vestibular and trigeminal systems.
BACKGROUND: Vertigo attacks are reported by subjects with migraine or a familiar history of migraine, also independently of headache episodes. Idiopathic vertigo is three times more frequent in migraine patients than in controls. Vestibular investigations in migraine patients have consistently demonstrated spontaneous Ny both of central and peri

The characteristics of vertigo in the pathology of vestibular peripheral lesions are differentiated and described. Symptomatological and clinical aspects are also discussed in the light of the physiopathological mechanisms of vestibular compensation. The therapeutic problem of vertigo is treated on the basis of experimental research on the inhibitory effect of some drugs on the vestibulo-ocular reflex.

The vestibular function was extensively investigated in 75 patients suffering from migraine. Pathological findings were present in 62 patients (82.6%). With the exception of position nystagmus, vestibular abnormalities were not related to migraine characteristics. Fifty-six patients were treated with flunarizine 10 mg daily for three months. A favourable effect on headache was obtained in 44 patients (78.5%). Flunarizine therapy influenced significantly gaze nystagmus and position nystagmus. The latter tended to be related to anti-migraine efficacy. Other electronystagmographic parameters were

PURPOSE: Diagnosis of Menière's disease (MD) relies on subjective factors and the patients diagnosed with MD may have heterogeneous pathophysiologies. This study aims to stratify MD patients using two objective data, nystagmus videos and contrast-enhanced magnetic resonance imaging (CE-MRI).
METHODS: This is a retrospective cross-sectional study. According to the Japan Society for Equilibrium Research criteria (c-JSER), adults diagnosed with definite MD and who obtained videos recorded by portable nystagmus recorder immediately following vertigo attacks and underwent CE-MRI of the inner ear w

The aims of the study were to characterize the vibration-induced nystagmus (VIN) in patients with unilateral vestibular neuritis (VN) and Ménière's disease (MD), and to clarify the clinical significance of VIN by comparing it with caloric results in patients with VN and MD. We recorded eye movements from 22 VN patients and 24 MD patients using unilateral 100-Hz vibration on the mastoid bone. Eye movements were analyzed and the maximum value of slow-phase eye velocity was obtained during vibration on each mastoid. The average value of slow-phase velocities was calculated. Spontaneous nystagmus

We reported a case of Fisher's syndrome with upbeat nystagmus. The syndrome manifested itself with the appearance of diplopia and ataxic gait preceded by cold-like symptoms. Although slight blephaloptosis, marked bilateral upward gaze palsy, and restricted abduction of the right eye were observed, light reflexes and Bell's phenomenon were normal. A slight decrease in the muscular power of the distal extremities, bilateral fingertip dysesthesia, and diminished sensation of vibration were observed. All muscle stretch reflexes were absent, and no pathologic reflexes were noted. Truncal ataxia and

Postrotatory responses of nystagmus were analysed in an exponential model by utilizing linear regression analysis. Four nystagmus qualities (velocity and duration of slow and fast phases) were studied in 10 patients with vestibular peripheral lesions, 10 patients with frontal lobe lesions and 10 patients with brain-stem lesions, together with 10 control subjects. In addition, pauses during the responses were quantified. Patients with frontal lobe lesions differed from other groups by scoring higher values of slow phase velocity and by exhibiting more pauses. The time constant was significantly

BACKGROUND AND AIMS: Dizziness, impaired balance and fear of falling are common complaints in the elderly. We evaluated the association of vestibular symptoms with vestibular findings in the elderly by posturography and video-oculography (VOG).
METHODS: We studied 38 oldest old subjects (≥ 85 yrs, mean age 89) living in a residential home. Vestibular symptoms were taken with a structured questionnaire, the Mini Mental State Examination (MMSE) was scored and any falls were recorded over a period of 12 months. Posturography was measured with a force platform and eye movements were measured by v

OBJECTIVE: In the past, various attempts were made to perform a quantitative analysis of nystagmographic findings but their diagnostic value was limited. Therefore, the authors present a multivariate analysis of nystagmus findings with the aim of increasing the precision of diagnostic differentiation in cases of vestibular dysfunction.
MATERIAL AND METHODS: A group of 387 patients and 40 healthy volunteers were examined over a 14-month period using electronystagmography after stimulation by bithermal, bilateral irrigation of the labyrinth. Amplitude, slow-phase velocity, frequency and directi

PURPOSE: Nystagmus is an involuntary jerky eye movement. It is an oculomotor sign of asymmetry in the vestibular pathway. With the rapid advancement of deep learning and its widespread applications, numerous deep learning-based methods for nystagmus detection have been proposed based on eye movement videos. This would enhance diagnostic efficiency. We propose a horizontal nystagmus detection model with joint SAM segmentation and time series classification.
METHODS: In this study, a convolutional neural network is employed to eliminate the invalid interference frames in the video. Subsequently

A 61-year-old woman presented with episodic dizziness that met diagnostic criteria for vestibular migraine. Vestibular and ocular motor examination was normal, aside from a 1°/second downbeat nystagmus (DBN) that was only seen with removal of fixation (in the dark) using video oculography. Two years later, lithium was initiated for treatment of depression and visual bouncing and jumping developed even when her head was still. On repeat examination, she had clear spontaneous (20°/second) DBN even with fixation in room light. Lithium therapy was discontinued, and for days and weeks, there was su

Positional downbeat nystagmus (pDBN) is a common finding in dizzy patients, with etiologies ranging from benign paroxysmal positional vertigo (BPPV) to central vestibular lesions. Although peripheral pDBN often presents with distinct clinical features that differentiate it from BPPV, diagnosing its etiology can be challenging. A thorough clinical evaluation, including the physical characteristics of the nystagmus, response to positional maneuvers, and neurological findings, is often sufficient to diagnose conditions that provoke pDBN such as anterior canal BPPV, atypical posterior canal BPPV,

Whereas several studies have reported on quantitative oculomotor and vestibular measurements in spinocerebellar ataxia type 6 (SCA6), selecting the most suitable paradigms remains challenging. We aimed to address this knowledge gap through a systematic literature review and providing disease-specific recommendations for a tailored set of eye-movement recordings in SCA6. A literature search (MEDLINE, Embase) was performed focusing on studies reporting on quantitative oculomotor and/or vestibular measurements in SCA6-patients. Oculomotor and vestibular parameters were extracted and correlations

BACKGROUND: Severe hypomagnesemia is an increasingly recognized cause of acute and reversible cerebellar ataxia, often accompanied by cerebellar oculomotor signs such as jerky horizontal or downbeat nystagmus and very rarely ocular flutter.
PHENOMENOLOGY SHOWN: This video illustrates horizontal pendular nystagmus in a patient with acute onset cerebellar ataxia associated with severe hypomagnesemia.
EDUCATIONAL VALUE: Acquired pendular nystagmus can be distinguished from macrosaccadic oscillations and ocular flutter in that the former is composed of two slow phases of equal velocity and the l

BACKGROUND: Accurate diagnosis of efferent visual system disease in neuro-ophthalmology involves the classification of clinical signs as prenuclear, nuclear, or infranuclear in origin. Over many years, I have come to recognize ocular torsional instability as a clinical sign of prenuclear disease.
METHODS: Retrospective chart review of patients in whom ocular torsional instability was diagnosed using indirect ophthalmoscopy.
RESULTS: Twenty patients were diagnosed as having ocular torsional instability (OTI). Eight had neuro-ophthalmologic disease caused by structural injury to prenuclear ocu

Original texts and expert translations from various ancient cultures covering a time span from the 2nd millennium BC to the ninth century AD were searched for historical observations of involuntary eye movements. Abnormal, spontaneous eye movements are an easily recognisable neuro-ophthalmological symptom that can be both congenital and acquired. Ocular oscillations termed 'hippos' by Hippocrates (460-370BC) and Galenos (129-216AD) are synonymous with nystagmus, a term first introduced in the eighteenth century. The original description of hippos suggests an innate onset, which retrospectively
