Whenever a critically ill gravida presents for care, consideration should be given to consulting an obstetrician or a maternal-fetal medicine specialist. The technological advances of the past two decades permit these critical care obstetrical specialists to immediately assess fetal health with ultrasonography or a fetal monitor. By so doing, maternal and fetal care can be optimized through a team approach.

Infections of the female upper genital tract are usually polymicrobic, often involving mixed aerobic (facultative) and anaerobic bacteria. Optimal therapy provides coverage against aerobes (both gram positive and gram negative and especially the Enterobacteriaceae) and anaerobes (especially the beta-lactamase-producing gram-negative species, such as Bacteroides). Escherichia coli is still the most common bacterial pathogen associated with urinary tract infections in women. Fundamental and clinical studies on gynecological use of cefotetan (CTT), a new cephamycin antibiotic, were performed with following results. Following the intravenous administration of 2.0 g of CTT, T 1/2 beta in serum was 3.1 hours and longer than the previous cephamycin antibiotics. The yields of CTT from serum to various uterine tissues and discharge from retroperitoneum were about 30 approximately 50%.
