Nystagmus: Diagnosis, Topographic Anatomical Localization and Therapy.

Nystagmus is defined as rhythmic, most often involuntary eye movements. It normally consists of a slow (pathological) drift of the eyes, followed by a fast central compensatory movement back to the primary position (refixation saccade). The direction, however, is reported according to the fast phase. The cardinal symptoms are, on the one hand, blurred vision, jumping images (oscillopsia), reduced visual acuity and, sometimes, double vision; many of these symptoms depend on the eye position. On the other hand, depending on the etiology, patients may suffer from the following symptoms: 1. perman

HINTS Examination in Acute Vestibular Neuritis: Do Not Look Too Hard for the Skew.

BACKGROUND: An ocular tilt reaction (OTR) is a triad of a skew deviation, head tilt, and ocular counter-roll that can be partial or complete. An OTR can occur anywhere along the utriculo-ocular motor pathways from the labyrinth to the interstitial nucleus of Cajal but is almost always central in origin. In acute vestibular neuritis (AVN), case reports have described patients with an OTR due to AVN, although it is unclear whether this examination finding is common or rare. METHODS: The vestibular and ocular motor features of 7 patients presenting with AVN are described. RESULTS: Each of the 7

Eye movement changes in autosomal dominant spinocerebellar ataxias.

Oculomotor abnormalities are common findings in spinocerebellar ataxias (SCAs), a clinically heterogeneous group of neurodegenerative disorders with an autosomal dominant pattern of inheritance. Usually, cerebellar impairment accounts for most of the eye movement changes encountered; as the disease progresses, the involvement of extracerebellar structures typically seen in later stages may modify the oculomotor progression. However, ocular movement changes are rarely specific. In this regard, some important exceptions include the prominent slowing of horizontal eye movements in SCA2 and, to a

[Ocular torticollis is a diagnostic and surgical challenge].

Torticollis is an abnormal head posture caused by ocular or non-ocular factors. Ocular torticollis (OT) is a compensatory head posture adopted to avoid double vision or dampen nystagmus. In this review, we argue, that causes for OT comprise a wide range of congenital or acquired ocular conditions. These patients are at risk of being mis-cat-egorised as having non-ocular torticollis, which may delay treatment and cause amblyopia in children. OT can be symptomatically relieved by prismatic correction incorporated in the patient's glasses or treated by strabismus surgery, which also improve ocula

Opsoclonus in a child with neuroborreliosis: Case report and review of the literature.

Opsoclonus consists of massive erratic rapid eye jerks. They may occur in isolation or in association with myoclonus and ataxia, i.e., opsoclonus-myoclonus syndrome (OMS). We report the case of a 9-year-old girl who suffered from headaches for several days and was shown to have opsoclonus and left peripheral facial palsy. Work-up excluded the diagnosis of neuroblastoma, but CSF analysis showed aseptic meningitis, and serology for Borrelia burgdorferi (Lyme) was positive. The outcome was favorable with complete regression of symptoms after treatment with ceftriaxone 2g/day for 3 weeks. Although

Successful extraocular muscle re-resection for a strabismus surgery complication: a "snapped" [severed, inadvertently] and retrieved inferior rectus muscle: a case report.

A 26- year-old woman who had a left head tilt since childhood was undergoing left inferior rectus resection to correct her left inferior rectus paresis. During the surgery, when the inferior rectus was isolated and engaged with an eye muscle hook, the muscle tore (snapped) completely into two pieces, 8 mm posterior to insertion. Fortunately, we were able to find the proximal portion of the muscle and, after a 3 mm resection, of the distal yet attached 8 mm portion, the proximal and distal portions were sutured together with a non-absorbable suture. After nine months followup there was signific

Pharmacotherapy of vestibular and ocular motor disorders, including nystagmus.

We review current pharmacological treatments for peripheral and central vestibular disorders, and ocular motor disorders that impair vision, especially pathological nystagmus. The prerequisites for successful pharmacotherapy of vertigo, dizziness, and abnormal eye movements are the "4 D's": correct diagnosis, correct drug, appropriate dosage, and sufficient duration. There are seven groups of drugs (the "7 A's") that can be used: antiemetics; anti-inflammatory, anti-Ménière's, and anti-migrainous medications; anti-depressants, anti-convulsants, and aminopyridines. A recovery from acute vestibu

Monocular partial/sector occlusion therapy: a procedure to inhibit diplopia in Brown syndrome.

BACKGROUND: Brown syndrome is recognized clinically as an absence or restriction of elevation in adduction. It often causes intermittent diplopia. CASE REPORT: This article includes discussion of a child, age 3 years, 11 months, who underwent surgery of the right eye for a left hypertropia. By age 5 years, he complained to his mother of daily, intermittent diplopia, eye pain in both eyes (OU), and frontal headaches. Additional strabismus surgeries OU were done at ages 6¾ years and 8¼ years. At age 10½ years, the ophthalmologist first noted suspicion of acquired right Brown syndrome, which was

[Kearns-Sayre syndrome : a mitochondrial disease (OMIM #530000)].

Kearns-Sayre syndrome is a rare mitochondrial disease which usually occurs sporadically with the presence of ptosis and the clinical triad of chronic progressive external ophthalmoplegia, atypical retinitis pigmentosa and cardiac conduction disorders. We show on the example of a young patient with unexplained atypical acquired ptosis and eye movement disorders, the correct interpretation of the diagnostic findings. Of importance is the early detection of potentially life-threatening complications which can lead to sudden cardiac death.

WEBINO and the return of the King's Speech.

We present a 69 year-old man with hypertension who developed the sudden onset of horizontal binocular diplopia and stuttering of speech. On examination, bilateral exotropia (i.e. 'wall-eyed') was observed in the primary position. Attempted horizontal saccades revealed bilateral internuclear ophthalmoplegia; all consistent with the wall-eyed bilateral internuclear ophthalmoplegia (WEBINO) syndrome. Convergence, vertical saccades and vestibular ocular reflexes were likewise impaired. Pupillary and levator palpebrae superioris functions were intact. Mild left-sided dysmetria, intention tremor and

[Postsurgical WEBINO, a new form of this syndrome].

CASE REPORT: A 20 year-old woman was submitted to resection of an ependymome of the fourth ventricle. After surgery the patient began to refer diplopia and oscillopsia. WEBINO syndrome (wall-eyed bilateral internuclear ophthalmoplegia), vertical nystagmus and skew deviation were diagnosed. DISCUSSION: WEBINO syndrome is considered a special form of bilateral internuclear ophthalmoplegia. It is characterized by bilateral absence of adduction, nystagmic abduction of both eyes, convergence deficiency and frequently exotropia. Ischemic, demyelinating and infectious etiologies have been described,

[Abnormal head turn in a patient with Brown's syndrome].

BACKGROUND: We report on an eight-year-old boy, who was presented in our clinic because of head turn. The cause of the tortecollis (ocular or general) in this case was and still cannot be explained. Only by applying extensive prism adaptation tests it was possible to prove the ocular character of the head turn. CASE REPORT: An eight-year-old boy with Brown's syndrome was referred to us because of a head tilt to the left side. Six months previously surgery on the M. obl. superior of the right eye was performed in another clinic. No improvement of the head tilt could be observed after the opera

A case of poststreptococcal opsoclonus-myoclonus syndrome.

High antistreptococcal antibody titer (ASOT) was measured in a 31-year-old Caucasian lady presenting with opsoclonus and myoclonus. She was treated with oral steroids and 8 weeks after the onset of symptoms she had a normal ASOT and only mild residual symptoms. This is one of the first cases of opsoclonus-myoclonus syndrome developing, following a streptococcal infection in adults.

[The control of gaze (3). Neurological defects].

Eye movements serve vision, which has two different aims: changing images using saccades, i.e. rapid eye movements, and stabilizing new images on the retina using slow eye movements. Eye movements are performed by ocular motor nuclei in the brainstem, on which supranuclear pathways--originating in the cerebral cortex, cerebellum and vestibular structures--converge. It is useful for the neurologist to know the clinical abnormalities of eye movements visible at the bedside since such signs are helpful for localization. Eye movement paralysis may be nuclear or infranuclear (nerves), involving all

Ocular motor features of alternating hemiplegia of childhood.

A 14-month-old boy with alternating hemiplegia of childhood, an idiopathic disorder of early childhood causing episodic hemibody tonic spasms and hemiplegia, showed repetitive jerks of abduction of the ipsilateral eye during the spells. The mechanism of this ocular motor abnormality is unknown but may be unique to this disorder.

[Superior oblique sharpening in Brown's syndrome "plus". Results].

PURPOSE: To study the results found in patients with congenital and constant Brown's Syndrome who were surgically treated with superior oblique sharpening technique. METHOD: Nine patients were selected among nineteen cases diagnosed of Brown's Syndrome in our Department for 4 years. Indications for surgery were vertical deviation in primary position and/or torticollis. The procedure of choice was the superior oblique sharpening in all cases, associated to its recession in 3 patients. RESULTS: The limitation of elevation in adduction was notably marked in 100% of the cases, improving in all o
