[Eye muscle surgery].

Eye muscle surgery is as a rule surgery on healthy eyes. It therefore requires particularly high standards in terms of a lack of complications and predictability of surgical results. Misalignment of the eyes, binocular diplopia and ocular torticollis are all indications for eye muscle surgery as are enlargement of the binocular visual field, asthenopic complaints in heterophoria or the stabilization of binocular functions. Eye muscle surgery is binocular surgery and differs from monocular surgery. It has its own complications and requires specific skills.

Congenital upper eyelid coloboma: embryologic, nomenclatorial, nosologic, etiologic, pathogenetic, epidemiologic, clinical, and management perspectives.

PURPOSE: To review the recent literature and describe the authors' experience with congenital upper eyelid coloboma. METHODS: In this review, we will summarize the embryologic and etiopathogenetic bases of congenital upper eyelid coloboma, and study the published clinical reports. We will also attempt to briefly shed some light on the rarer syndromic curiosities associated with upper eyelid coloboma. RESULTS: Congenital upper eyelid colobomas are one of the few nontraumatic oculoplastic emergencies that may occasionally present in the first few days of life with a corneal ulcer and may even

[Periocular surgery and the impact of split-thickness skin grafts].

Oculoplastic surgery has to consider the specific characteristics of the periocular region and possible resulting functional consequences. Motility of eyelids, limited skin thickness with excellent wound healing conditions, lacrimal duct system and eyelid ligaments are important criteria for the individual choice of the surgical procedure. The use of split-thickness skin grafts represents an established method to repair periocular defects. Important medical conditions for split-thickness transplantation are colour of the skin, skin structure, photodamage of the skin, hair growth and other dege

Improving Operating Room Access and Utilization at an Academic Ophthalmology Department.

BACKGROUND AND OBJECTIVE: A quality improvement project was conducted to improve operating room (OR) access and utilization for elective ophthalmic cases at a university-affiliated eye center. PATIENTS AND METHODS: This prospective, interventional case series assessed OR utilization across three ORs during a baseline 3-month period. Case scheduling protocols were reviewed, and key drivers of inefficiency were identified. Targeted interventions were implemented to address these barriers. RESULTS: During the baseline period, a mean of 290 surgeries were performed monthly. Scheduling delays wer

Influence of orbital decompression on upper eyelid retraction in Graves' orbitopathy: a systematic review and meta-analysis.

Graves'-associated upper eyelid retraction (GAUER) is the commonest manifestation of orbitopathy in adults. Surgical management typically follows a 3-step staged approach commencing with orbital decompression. The rationale behind this is that certain surgical interventions can influence the parameters and outcomes of subsequent procedures. We performed a systematic review and meta-analysis evaluating the effects of orbital decompression on GAUER in adult patients with Graves' orbitopathy. All original English, non-pediatric studies meeting the study inclusion criteria from the last 20 years w

Acute thyroid eye disease (TED): principles of medical and surgical management.

The active inflammatory phase of thyroid eye disease (TED) is mediated by the innate immune system, and management is aimed at aborting this self-limited period of autoimmune activity. In most patients with TED, ocular and adnexal changes are mild and management involves controlling thyroid dysfunction, cessation of smoking, and addressing ocular surface inflammation and exposure. In patients with acute moderate disease, this being sufficient to impair orbital functions, immunosuppression reduces the long-term sequelae of acute inflammation, and adjunctive fractionated low-dose orbital radioth

The anterior eyelid crease approach to the orbit.

PURPOSE OF REVIEW: The anterior lid crease approach to the orbit allows the surgeon access to the superior orbital compartments through an upper eyelid incision giving excellent exposure with minimal damage to normal structures. Because the incision is in the lid crease, scarring is minimal. This route into the orbit has gained popularity in recent years and has been used by multiple different surgical subspecialties. RECENT FINDINGS: Historically, the eyelid approach to the orbit has been used mainly by ophthalmic plastic surgeons for removal of intraconal and extraconal orbital tumors, orbi

Perioperative Management of Coagulation Disorders in Ophthalmic Surgery.

Disorders of blood coagulation can lead to manifest spontaneous bleeding and an increased risk of bleeding during surgical procedures and interventions. Pathophysiologically, a distinction can be made between defects in primary haemostasis, which lead to impaired platelet adhesion and platelet aggregation, and disorders of secondary (plasmatic) haemostasis, which are characterised by impaired fibrin formation or fibrin stabilisation. Aetiologically, a distinction can be made between rare genetically-determined hereditary defects and common acquired coagulation disorders, which may be based on

A New Surgical Approach for the Treatment of Conjunctivochalasis: Reduction of the Conjunctival Fold with Bipolar Electrocautery Forceps.

Aim. To report a new surgical technique for the treatment of conjunctivochalasis. Methods. A new surgical technique in which specially designed bipolar electrocautery forceps facilitate the complete reduction of the conjunctival folds without creating lesions near the corneoscleral limbus was designed. A retrospective revision of the medical records of patients treated with this technique between the years 2011 and 2013 was made, and eighteen eyes of sixteen patients with conjunctivochalasis treated with this new technique were included. Results. All the eyes treated showed a significant impro

Study on the treatment of traumatic orbital apex syndrome by nasal endoscopic surgery.

Orbital apex syndrome (OAS) is a complex disease caused by a variety of pathological factors, and trauma is one of the main factors/causes. Clinical data of 17 cases of traumatic OAS treated by nasal endoscopic surgery in our department from January 2002 to April 2009 were gathered and reviewed. Among them, the six patients presented with OAS after injury to the lateral wall of orbital apex. Seven other patients exhibited OAS after injury to the medial wall of orbital apex, two displayed OAS after zygomatic trauma, while OAS manifested in the other two patients with craniocerebral trauma 3 day

Using the eye fixation speculum as an adjunct to pterygium surgery.

A technique is described that overcomes the two biggest problems facing the surgeon when dissecting a pterygium from the cornea-bleeding and eye movement. Our technique however, requires only minimal anesthesia (topical and subconjunctival) and the use of a disposable speculum and suction ring. An added advantage is this particular speculum gives good exposure of the superior bulbar conjunctiva; this facilitates harvesting a conjunctival autograft.

Controlling of Applied Force and Cornea Displacement Estimation in Robotic Corneal Surgery With a Gripper Surgical Instrument.

BACKGROUND: The human eye consists of highly sensitive, hydrated, and relatively thin tissues, making precise control and accurate force estimation crucial in robotic eye surgery. This paper introduces a novel control method and state observer designed for a gripper surgical instrument used on the external ocular surface during robotic eye surgery. METHODS: A novel state observer, operating in tandem with the controller, estimates the applied force. The proposed control approach, termed the Fixed-time Observer-based Sliding Mode Control (FOSMC), estimates the applied force by determining the

[Transposition flaps for reconstruction of deep defects of the medial eyelids. Fasciocutaneous rotations- and transposition flaps and modified pure fascial transposition flap].

Tumor resection in the medial canthal area may result in deep defects with involvement of the ethmoidal bone and destruction of the lacrimal ducts. While shallow defects with little involvement of bone can be covered with the classic glabellar flap, deep defects require larger fasciocutaneous flaps from the forehead that can also be used for the reconstruction of the involved medial parts of the eyelids. The preferred techniques of the median or paramedian transposition flaps and a modified paramedian fascial flap for reconstruction of the medial orbital wall and support of the orbit are descr
