We describe the ophthalmic manifestations of 3 cases of infantile Refsum's disease. The gravity and the aspect of the retinal disorders which we have observed by ophthalmoscopy and electroretinography were quite different from one case to another, including two siblings. We then go on to discuss the pathogeny and the genetic basis of diseases due to a deficiency of the peroxisomal biogenesis.

A visually asymptomatic 27-year-old man was found to have inferior altitudinal visual field defects binocularly. Ophthalmoscopy revealed superior segmental optic pallor with superior nerve fiber layer atrophy, nicely highlighted in red-free photographs. The patient's mother had insulin-dependent diabetes mellitus. Recognition of this entity is important for prognosis and for avoidance of unnecessary diagnostic studies.

Atrophy of the nerve fibres in the retina visible by direct ophthalmoscopy using red-free illumination, together with corresponding scotomata in the visual fields, signified retrograde degeneration of retinal nerve fibres from multiple sclerosis. Defects in medullated retinal nerve fibres were continous with arcuate nerve fibre bundle defects. These myelin defects exemplify secondary demyelination of optic axons.

BACKGROUND: Sudden loss of vision is an ophthalmological emergency.
OBJECTIVE: This article explains how history and examination, particularly ophthalmoscopy, will greatly assist the general practitioner to diagnose the cause.
DISCUSSION: Vision loss may be transient or prolonged. It is important to consider whether the eye is painful or comfortable, inflamed or not. An accurate diagnosis will direct the emergency management, which frequently determines the prognosis for vision.

Aicardi syndrome is a rare neurodevelopmental disease characterised by congenital chorioretinal lacunae, corpus callosum dysgenesis, seizures, polymicrogyria, cerebral callosum, chorioretinopathy and electroencephalogram abnormality. We present a case of Aicardi syndrome with callosal hypogenesis in a 4.5-month-old baby who presented with infantile spasms. Ophthalmoscopy revealed chorioretinal lacunae. The clinical and magnetic resonance imaging features were diagnostic of Aicardi syndrome.

OBJECTIVE: Improved survival of low birth weight, premature babies have increased the incidence of retinopathy of prematurity. This hospital-based, prospective, study was undertaken to determine its incidence and risk factors in our neonatal unit.
METHODS: Neonates with gestational age < or = 35 weeks and/or birth weight < or = 1500 gm born over a one-year period were examined by indirect ophthalmoscopy between 2 to 4 weeks after birth, and followed up till retinal vascularisation was complete.
