From the archives of the AFIP: Pediatric orbit tumors and tumorlike lesions: nonosseous lesions of the extraocular orbit.

The histologic spectrum of nonosseous tumors and tumorlike lesions of the extraocular orbit in children differs from that in adults, and the appearance of these lesions at imaging reflects their pathologic features. Rhabdomyosarcoma is the most common extraocular orbital tumor in children. This neoplasm usually manifests in young children, grows quite rapidly, and is fairly vascular. Vasculogenic lesions are common orbital lesions in newborns and young infants. The most prevalent of these are infantile hemangioma, a true neoplasm, and venous-lymphatic malformation, a developmental anomaly. Hem…

Photonic spectral horizons for spin-orbit conversion.

We present a theoretical and computational framework for metasurface mirrors that switch optical angular momentum as a function of wavelength. The approach is based on controlling the phase retardance and amplitude balance of anisotropic meta-atoms to create a sharp wavelength threshold, termed a spectral horizon, where the reflected light reverses its spin and orbital angular momentum. The paper develops an operational definition of this spectral horizon, supported by analytical modeling, large-scale Monte Carlo validation, and full-field simulations of spatial intensity and phase profiles in…

Coherent control of optical spin-orbit interactions.

Optical spin-orbit interactions (SOI) are an intriguing physical phenomenon where the spin (circular polarization) of light influences and governs its spatial degrees of freedom. Yet, achieving real-time and flexible control over SOI remains a formidable challenge. Here, we propose a coherent control method for the flexible manipulation of optical SOI using a thin isotropic crystal. The output horizontally and vertically polarized waves from the crystal exhibit strong and distinct dependencies on the phase delay between the input signal and control waves, leading to substantial spin-orbit beam…

Geosynchronous orbit-based Earth radiation measurement system.

We have designed a set of Earth radiation measurement systems based on a segmented aperture method, which can be deployed in geosynchronous orbit. This system includes a four-channel radiometer and a nine-channel multispectral camera. The four-channel radiometer adopts a Cassegrain telescope system as the initial structure and combines segmented aperture technology to achieve simultaneous measurements of four Earth radiation bands (0.3∼50µm, 0.3∼4.3µm, 4.3∼50µm, and 0.7∼4.3µm). The radiometer is equipped with two linear array thermopile detectors to capture images from the four channels, with …

Recurrent Solitary Fibrous Tumors in the Orbit.

BACKGROUND: Solitary fibrous tumors (SFTs) of the orbit are rare mesenchymal neoplasms that typically exhibit indolent behavior. However, a subset may recur, sometimes many years after initial treatment, posing significant diagnostic and therapeutic challenges. Because of the rarity of recurrent orbital SFTs, there is limited literature addressing their clinicopathologic features, diagnostic evolution, and optimal management strategies. OBJECTIVE: This study aims to analyse the clinical, radiologic, histopathologic, and immunohistochemical characteristics of 4 patients with recurrent orbital …

Phosphaturic Mesenchymal Tumor Originating in the Orbit.

Phosphaturic mesenchymal tumors (PMTs) are rare benign neoplasms that are often associated with tumor-induced osteomalacia. The authors present a rare case of a PMT with primary origin in the orbit. A 53-year-old female presented with diffuse bone pain and deficiency fractures throughout the body on x-ray imaging. Antinuclear antibody, immunoglobulin G4, and parathyroid hormone were elevated at that time. An incidental apical orbital mass with medial rectus and superior oblique involvement was found on MRI of the brain. Histologic examination confirmed fibroblast growth factor 23 positivity fo…

Transverse Magnetization in Spin-Orbit Coupled Antiferromagnets.

Some antiferromagnets under a magnetic field develop magnetization perpendicular to the field as well as more conventional ones parallel to the field. So far, the transverse magnetization (TM) has been attributed to either the spin canting effect or the presence of cluster magnetic multipolar ordering. However, a general theory of TM based on microscopic understanding is still missing. Here, we construct a general microscopic theory of TM in antiferromagnets with cluster magnetic multipolar ordering by considering classical spin Hamiltonians with spin anisotropy that arises from the spin-orbit…

Spindle Cell/Sclerosing Rhabdomyosarcoma of the Orbit.

A 3-year-old boy developed proptosis over 3 weeks. CT and MRI disclosed a 3.2 × 1.9 cm soft-tissue mass of the right extraconal and intraconal orbit with sphenoid bone erosion. After debulking through an upper eyelid crease incision, the tumor was diagnosed as a spindle cell/sclerosing rhabdomyosarcoma. DNA sequencing was negative for an L122R mutation in MyoD1 . Spindle cell/sclerosing rhabdomyosarcoma is an uncommon variant of this neoplasm, and only 2 patients with orbital tumors have been reported in 2 case series. Spindle cell/sclerosing rhabdomyosarcomas confined to the orbit are conside…

Low-grade fibromyxoid sarcoma of the orbit.

A 70-year-old male presented with diplopia and painless proptosis of the left eye for 5 months. Examination showed 6 mm of axial proptosis and restriction of supraduction, abduction and adduction, and mild limitation of infraduction of the left eye. Magnetic resonance imaging demonstrated a large, moderately well-circumscribed intraconal mass in the left lateral orbit, and excisional biopsy was performed. Histopathologic features of mixed fibrous and myxoid areas in a whorl-like pattern and immunohistochemical staining for MUC4 confirmed the diagnosis of low-grade fibromyxoid sarcoma (LGFMS). …

Acute Leukemia Presenting in the Pediatric Orbit.

We present a case series to evaluate the clinical features of acute leukemia presenting with primary orbital manifestations. We undertook a retrospective case review of primary orbital presentations of acute myeloid leukemia (AML) and acute lymphocytic leukemia (ALL) over a 10-year period at two hospital sites (Hereford County Hospital and Leicester Royal Infirmary). Our case series included four patients - two with AML and two with ALL. Patients were young (mean age of four years and five months) at presentation, all with unilateral disease, and presented with orbital signs. Although there wa…

Renal Cell Carcinoma Metastasis to Anterior Orbit.

Renal cell carcinoma (RCC) is a rare malignancy that often metastasizes to the lung, bones, liver, and brain. Only a few cases of RCC metast … no systemic symptoms. This report also reviews existing literature on RCC metastasis to the eye and orbit. Ultimately, RCC metastasis to the eye in patients with a history of known RCC should be considered in the differential diagnosis in those presenting with an atypical mass in periocular regions.

[Endoscopic decompression of orbit in Graves' orbitopathy].

Graves' orbitopathy (GO) is an extrathyroidal manifestation of Graves' disease (GD). The majority of patients has mild form of the disease, with no need of additional treatment. A few percent of patients can have a severe or very severe course of disease. In severe forms of GO there might occur considerable exophthalmos complicated in some cases with corneal ulceration or pressure on optic nerve leading to neuropathy (DON, dysthyroid optic neuropathy). In therapy of severe forms of GO different types of treatment are used depending on diagnosis and activity of disease. The pharmacological (amo…

A giant dermoid cyst of the orbit.

To describe a unique case of a non-germinomatous germ cell tumor, a giant orbital dermoid, present in adulthood. A 31-year-old male presented in our clinic with gradual, painless, progressive proptosis in the left eye since 28 years associated with gradual loss of vision. The left eye showed gross proptosis. Ocular structures could not be made out. Computed tomography scan showed a well-circumscribed oval heterogeneous mixed solid and cystic lesion completely filling the left orbit with calcification. The differential diagnosis was that of either teratoma or optic nerve glioma. The patient und…

Vascular Anomalies of the Orbit--A Reappraisal.

The recent International Society for the Study of Vascular Anomalies (ISSVA) classification of vascular anomalies can be applied to orbital lesions, dividing them into vascular tumors and vascular malformations. Orbital cavernous hemangiomas are probably best considered cavernous venous malformations under this classification. Management of symptomatic lesions can be with surgical excision or stereotactic fractionated radiotherapy in selected cases. Beta-blockers including propranolol and topical timolol maleate represent first-line therapy for infantile hemangiomas, although surgery has a rol…
